Results: We identified three latent classes of drinking behaviors: current non-drinkers (CND), binge drinkers (BD), and problem drinkers (PD). Compared to the CND class, both BD and PD classes were strongly associated with higher academic year, lower academic performance, higher levels of stress, suicidal ideation, lifetime conventional or electronic cigarette use, and lifetime use of other drugs. Conclusion: Health professionals should develop and implement intervention strategies targeting individual subgroups of drinking behaviors to obtain better outcomes. In particular, health professionals should consider different characteristics across subgroups of adolescent drinking behaviors when developing the interventions, such as poor mental health status and other substance use among binge and problem drinkers.

제안 방법
SAS 소프트웨어는 결측치가 포함된 지표를 이용하여 잠재계층의 유형 파악 시 무작위 결측(missing at random)이라는 가정에 근거하여 완전정보최대우도법(full-information maximum likelihood)을 이용하여 잠재계층모형을 추정함에 따라 [24], 본 연구는 ‘음주 시작 시기’ 지표에 결측치가 있는 대상자를 포함하여 분석할 수 있었다.
대표성이 있는 연구결과를 얻기 위해 본 연구에서는 제13차 청소년건강행태온라인조사 표본선정 시 사용된 층화, 집락 및 가중치 정보를 반영하여 대상자의 특성을 분석하였다. 둘째, 본 연구에서 청소년기 음주행동 잠재계층모형 파악을 위하여 잠재계층분석을 시행하였다. 잠재계층모형의 판별을 위해, 잠재계층 개수가 1개부터 5개까지인 모형별로 모형적합도, 모형의 간명성(parsimony), 이론적 해석 가능성과 모형 판별(model identification) 수준을 비교하여 모형을 결정하였다[24-26].

혼자 음주한 경험은 지난 12개월 동안 혼자 술을 마신 경험으로, 음주시작 시기는 초등학교 6학년 이전에 처음으로 1잔 이상의 술을 마신 경험으로 파악하였다. 음주로 인한 부정적 경험은 지난 12개월 동안 가족이나 친구로부터 절주하라는 충고를 들은 경험, 음주 후 직접 운전하거나 술을 마신 사람이 운전하는 차에 동승한 경험, 음주 후 기억을 잃은 경험 및 음주 후 타인과 싸운 경험으로 조사하였다.
이 때, 대상자의 일반적 특성, 정신건강 특성 및 약물사용 특성을 공변량으로 포함시켜 본 연구의 잠재계층모형과의 연관성을 분석하였으며, ‘현재 비음주자’ 계층을 준거 집단으로 이용하였다.

대상 데이터
잠재계층모형의 판별을 위해, 잠재계층 개수가 1개부터 5개까지인 모형별로 모형적합도, 모형의 간명성(parsimony), 이론적 해석 가능성과 모형 판별(model identification) 수준을 비교하여 모형을 결정하였다[24-26]. 본 연구에서 사용된 모형적합도는 the likelihood-ratio statistic (G2), Akaike information criterion (AIC), Bayesian information criterion (BIC), Adjusted BIC, the log-likelihood, Entropy가 있다. Entropy 값은 클수록, G2, AIC, BIC, Adjusted BIC와 the log-likelihood 값은 작을수록 모형이 자료에 적합하다는 것을 의미한다.
본 연구의 대상은 제13차 조사 참여자 중 평생 술을 한 잔이라도 마신 경험이 있다고 응답한 중·고등학생 24,417명이었다.

성능/효과
‘문제 음주자’ 계층에 속할 위험성은 중학생에 비해 고등학생이 2.78배 높았고, 학업성취도는 교차비가 0.52~0.81배로 조사되어, 학업성취도가 낮은 청소년에 비해 보통 이상인 청소년은 ‘문제 음주자’ 계층에 속할 위험성이 낮은 것으로 나타났다.
8개 변수를 독립변수로 포함하여 다변량 분석을 시행한 결과, 성별과 주관적 경제수준을 제외한 모든 변수가 ‘폭음자’와 ‘문제 음주자’ 계층을 유의하게 예측하는 것으로 나타났다(Table 5).
계층간 특성을 비교하기 위하여 8개 변수를 이용하여 단변량 분석을 시행한 결과, 모든 변수가 잠재계층 간에 유의한 차이가 있었다(Table 4). 음주 수준이 높은 ‘문제 음주자’ 계층에서 남학생과 고등학생의 비율이 대체적으로 높았다.
본 연구에서 학년 변수는 통계적으로 유의하게 음주행동 유형을 예측하였다. 구체적으로, 중학생에 비해 고등학생이 폭음과 문제 음주를 경험할 위험성이 높았다. 이러한 연구 결과는 학년이나 연령이 청소년기 음주행동 관련 잠재계층모형을 유의하게 예측한 선행연구 결과와 일치한다[10,13,14].
구체적으로, 폭음자 계층에 속할 위험성은 중학생에 비해 고등학생이 2.48배 높았고, 학업성취도는 교차비가 0.82~0.88배로 조사되어, 학업성취도가 낮은 청소년에 비해 보통 이상인 청소년은 ‘폭음자’ 계층에 속할 위험성이 낮은 것으로 나타났다.

88배로 조사되어, 학업성취도가 낮은 청소년에 비해 보통 이상인 청소년은 ‘폭음자’ 계층에 속할 위험성이 낮은 것으로 나타났다. 또한 폭음자 계층에 속할 위험성은 스트레스를 많이 느낀다고 응답한 청소년이 1.12배, 자살을 생각해본 청소년이 1.24배, 흡연 경험이 있는 청소년이 3.13배, 평생 담배/술 이외의 약물 사용 경험이 있는 청소년이 3.26배 높은 것으로 나타났다.
마지막으로, 본 연구에서 평생 궐련/전자담배 사용 경험과 담배/술 이외의 약물 경험이 있는 청소년은 폭음자 계층이나 문제 음주자 계층에 속할 위험성이 높았으며, 그 영향력이 다른 예측 요인에 비해 높았다. 이는 미국 청소년 중 알코올 섭취 수준이 높고, 음주로 인한 부정적 결과를 많이 경험하는 집단이 평생 흡연 및 약물사용, 지난 30일간 흡연 및 약물사용의 위험성이 높다고 밝힌 선행연구와 유사하다[13,14].

본 연구에서 음주행동 유형 파악에 사용된 지표 및 음주행동 유형 예측요인 중 ‘음주 시작 시기’ 지표 1개 변수만이 62개(0.3%)의 결측치를 포함하고 있었다.
본 연구에서 음주행동 유형별 특성을 분석한 결과, 성별과 주관적 경제수준을 제외한 나머지 6개 요인이 음주행동 잠재계층을 유의하게 예측하는 것으로 나타났다. 첫째, 일반적 특성 중 성별은 다른 예측 변수들의 효과를 통제한 상태에서 음주행동 유형을 유의하게 예측하지 않는 것으로 나타났다.
본 연구에서 주관적 경제수준은 음주행동 유형을 유의하게 예측하지 않는 것으로 나타났다. 변수중심 접근법을 사용하여 청소년기 사회경제적 수준과 음주문제의 관계를 조사한 연구들을 체계적으로 고찰한 선행연구에 따르면, 두 변수의 관계에 대한 결과는 혼재되어 있으나 대다수 연구에서 청소년기의 음주 사용과 사회경제적 수준은 연관성이 없었다[33].
이러한 결과는 청소년이 집에서 보내는 시간보다 친구와 보내는 시간이 증가하여 친구 관련 요인이 청소년의 음주행동에 더 큰 영향을 주기 때문에, 부모나 청소년 자신의 사회경제적 수준의 영향력이 감소되는 것으로 일부 설명될 수 있다[35]. 본 연구에서는 주관적 학업성취도가 낮을수록 폭음자나 문제 음주자 계층에 속할 위험성이 높아지는 것으로 나타났다. 이는 여자 청소년을 대상으로 음주행동 잠재계층분석을 시행한 연구에서 음주 행동 수준이 높은 잠재계층일수록 학업성적이 유지되지 않거나 결석이나 숙제 불이행과 같은 학업적 문제를 많이 경험한다고 한 연구[16]와 유사하였다.
본 연구의 결과, 대상자의 음주행동 유형은 현재 비음주자, 폭음자와 문제 음주자 집단으로 나뉘었다. 대상자의 3분의 2 이상이 현재 비음주자 집단으로 가장 많았으며, 다음은 폭음자, 문제 음주자 집단 순이었다.
둘째, 잠재계층 숫자가 4개 이상인 모형보다 3개인 모형이 간명하고 이론적으로 해석이 용이하였다. 셋째, 무작위 초기값을 이용하여 동일 모형을 20번 추정한 결과에서도 잠재계층이 3개인 모형의 80.0%가 최적합 모형으로 수렴된 반면, 잠재계층이 4개와 5개인 모형에서는 최적합 모형으로 수렴되는 백분율이 40.0%와 25.0%였다. 이는 잠재계층이 4~5개인 모형에 비해 3개인 모형의 모형 판별 수준이 높음을 의미한다.
스트레스 수준이 높거나 자살생각을 경험한 학생의 비율은 ‘현재 비음주자’ 및 ‘폭음자’ 계층에 비해 ‘문제 음주자’ 계층에서 높았다.
81배로 조사되어, 학업성취도가 낮은 청소년에 비해 보통 이상인 청소년은 ‘문제 음주자’ 계층에 속할 위험성이 낮은 것으로 나타났다. 스트레스 수준이 높은 청소년과 자살을 생각해본 청소년은 문제 음주자 계층에 속할 위험성이 각각 1.54배, 2.08배 높았다. 마지막으로, 흡연 경험과 평생 담배/술 이외의 약물 사용 경험이 있는 청소년은 문제 음주자 계층에 속할 위험성이 각각 23.

본 연구의 의의는 다음과 같다. 첫째, 우리나라 전국 단위의 대표성 있는 청소년 자료를 이용하였으므로, 본 연구에서 파악된 청소년기 음주행동 유형 결과는 한국 청소년에게 일반화가 가능하다. 둘째, 인간중심 접근법 이용을 통해 다차원적인 음주행동 관련 지표들을 반영하여 우리나라 청소년의 음주행동 유형을 규명하였다.
본 연구에서 음주행동 유형별 특성을 분석한 결과, 성별과 주관적 경제수준을 제외한 나머지 6개 요인이 음주행동 잠재계층을 유의하게 예측하는 것으로 나타났다. 첫째, 일반적 특성 중 성별은 다른 예측 변수들의 효과를 통제한 상태에서 음주행동 유형을 유의하게 예측하지 않는 것으로 나타났다. 본 연구결과와는 달리, 뉴질랜드 청소년 대상의 음주행동 잠재계층모형 연구에서 여학생에 비해 남학생이 저위험 음주 유형보다 중등도나 고위험 음주 유형에 속할 위험성이 유의하게 높은 것으로 나타났다[10].
본 연구데이터에 가장 적합한 잠재계층모형 결정을 위하여 잠재계층 개수가 1개에서 5개에 이르는 5종류의 모형을 비교하였고, 본 연구에서는 다음의 3가지 근거에 의하여 잠재계층이 3개인 모형을 최종적으로 채택하였다(Table 2). 첫째, 잠재계층의 숫자가 증가할수록 G2, AIC, BIC와 adjusted BIC 값이 감소하는 것으로 나타났다. 그러나 잠재계층이 3개 이상인 모형에서는 G2 , AIC, BIC와 adjusted BIC 값의 감소 폭이 크게 줄었으며, entropy 값 역시 잠재계층이 4~5개인 모형보다 3개인 모형에서 더 높았다.

더 나아가 청소년기에 혼자 술을 마신 경험은 성인기의 알코올 사용장애나 다른 약물사용 및 문제행동(물건 훔치기, 폭행)과도 유의한 연관성이 있었다[28,29]. 본 연구에서도 문제 음주자가 빈번하게 많은 양의 술을 마시며 혼자 음주하거나 음주로 인한 다양한 부정적 경험을 할 위험성이 모두 높은 것으로 나타나, 문제 음주 청소년에 대한 시급한 대책 마련이 필요하다.
본 연구의 결과에 근거하여 후속연구 방향을 다음과 같이 제언한다. 첫째, 현재 우리나라 청소년 음주행동 유형에 대한 이해가 부족하므로 인간중심 접근법을 이용하여 청소년기 음주행동 유형 판별을 위한 후속연구가 필요하다. 둘째, 청소년기 음주행동 유형의 특성을 심층적으로 파악하기 위한 연구가 필요하다.
