Objective : This case report presents the effect of Hoiyeok-tang on atopic dermatitis. A patient was diagnosed with Lesser Yin disease based on Shanghanlun provisions. Methods : According to the disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS), we diagnosed the patient with Lesser Yin disease. The patient was treated with Hoiyeok-tang for 87 days. The changes in symptoms were estimated by the SCORAD index. Results : After treatment, the SCORAD Index improved from 57.3 to 33.5. The patient also body weight from 89.1 kg to 76.5 kg. Conclusions : This case study shows the effectiveness of Hoiyeok-tang on atopic Dermatitis according to DPIDS.

본 증례의 환자는 25세 여성으로 초등학생 때 체중이 급격히 증가한 이후 아토피 피부염이 발생하여 증상 조절을 위해 스테로이드 외용제를 사용하다가 2018년 10월 스테로이드 외용제 중단 이후 양 볼, 목, 가슴, 등 부위의 피부염이 악화되어 본원에서 3개 월간 回逆湯으로 치료를 받아 아토피 피부염 호전 및 체중 감량에 성공하였다.

후속연구

다만 본 증례 연구는 단일 증례에 대한 연구로서, 통계적 유의성을 검토할 수는 없다는 연구방법상의 한계가 있다. 따라서 앞으로 추가적인 증례 보고, 또는 신뢰도 높은 연구를 통해 임상적 검증을 해야 할 것으로 사료된다.

	국소 스테로이드제의 부작용은?	특히 아토피 피부염 치료에 기본적으로 선택되고 있는 국소 스테로이드제는 장기적으로 사용시 내성, 표피 및 진피 조직의 위축과 약화로 인한 모세 혈관 확장, 자색반 또는 멍, 튼살, 피부 궤양 등의 부작용이 발생할 수 있으며, 여드름 모양 반응, 다모증, 색조 변화와 같은 의인성 피부질환이 발생할 수도 있다. 또한 다양한 2차 감염이 발생할 수 있으며, 쿠싱 증후군, 골밀도 감소, 백내장, 녹내장, 고혈압, 소아의 성장 억제가 유발 될 수 있다4, 5). 더욱이 성인형 아토피 피부염 환자는 질병에 이환된 기간이 길어서 이러한 스테로이드 제제의 지속 도포가 최선의 해결책이 될 수 없다.
