Objective: This study reviewed recent clinical research trends regarding the effects of herbal medicine treatment containing diuretic herbs on nonalcoholic fatty liver disease (NAFLD) in China. Methods: We reviewed clinical studies on NAFLD from the China Academic Journal of the China National Knowledge Infrastructure from January 1, 2016, to September 10, 2018. Results: Thirty-nine papers from 86 studies were reviewed. The most commonly used herbal prescriptions included Alismatis Rhizoma(澤瀉), Salviae miltiorrhizae Radix (丹蔘), Crataegii Fructus (山査), Polia Sclerotium (茯?), Bupleuri Radix (柴胡), and Cassiae Semen (決明子). The treatment effective rate, liver function test, serum lipid test, radiological examination, and symptom comparisons were used to analyze the treatments. In 32 of the papers, the effective treatment rate was 76.9%-100%, and the effectiveness in the experimental group was significantly higher than that in the control group (p<0.05). The risk ratio for the treatment effective rate was 1.26. The mean difference of ALT was -13.44 U/L (95% confidential interval, -24.45 U/L, -2.44 U/L, p=0.0166). Conclusion: Herbal medicine containing diuretic herbs has been demonstrated to be an effective and useful treatment for NAFLD. Clinical studies that are more scientific and systematic should be actively conducted in the future, and the results of the current study could be used as basic data in future clinical studies on herbal medicine treatment for NAFLD.

데이터처리

이질성 평가(Heterogenity test)를 수행하여 연구결과에 의미 있는 차이의 여부를 평가하였다. I2와 Cochrane Q-검사를 활용하여 이질성을 평가하였으며, I2가50% 이상이거나 Cochrane Q-검사의 결과가 p<0.1인 경우 이질성이 있다고 평가하였다. 통계적으로 유의미한 이질성이 발견되는 경우 변량효과모형(Random-effects model)을 활용하였으며 그렇지 않은 경우 고정효과모형(Fixed-effect model)을 활용하였다.

이론/모형

두 명의 연구자가 최종 선정된 39건의 RCT 문헌들을 대상으로 코크란 비뚤림 위험(Cochrane’s Risk of Bias, RoB) 평가표를 사용하여 비뚤림 위험 평가를 시행하였다. 비뚤림 위험 평가 항목에는 무작위 배정 순서 생성, 배정 순서 은폐, 연구 참여자, 연구자에 대한 눈가림, 결과평가에 대한 눈가림, 불충분한 결과 자료, 선택적 보고, 그 외 비뚤림 총 7가지가 있으며, 비뚤림 위험이 큰 경우에는 ‘높음(High risk of bias)’, 적은 경우에는 ‘낮음(Low risk of bias)’, 판단하기 어려운 경우에는 ‘불확실(Unclear risk of bias)’로 표기하였다.
1인 경우 이질성이 있다고 평가하였다. 통계적으로 유의미한 이질성이 발견되는 경우 변량효과모형(Random-effects model)을 활용하였으며 그렇지 않은 경우 고정효과모형(Fixed-effect model)을 활용하였다. 결과는 숲 그림(forest plot)과 MD, RR의 점 추정치와 그 신뢰구간으로 제시하였다.

성능/효과

치료방법은 39편의 논문 중 34편의 논문에서 탕약을 대상으로 연구되었고, 나머지 5편의 논문에서는 과립제를 대상으로 연구되었다. 처방 구성 약재 중 다용 약재는 澤瀉, 丹蔘, 山査, 茯苓, 柴胡, 決明子 순이었다.
7. 치료 유효율로 평가된 논문은 33편이었으며, 치료 유효율은 76.9~100%였다. 33편의 논문 모두 치료군은 대조군에 비해 유의성이 높은 것으로 나타났다.
8. 메타분석은 치료 유효율과 ALT 수치를 가지고 진행되었고, 두 평가항목 모두 통계적 유의미를 가지고 있으나, Cochrane Q-검정 결과 치료 유효율은 유의미한 이질성이 발견되지 않았으나 ALT 수치는 유의미한 이질성이 발견되었다.
이상의 중의학 논문을 통해 살펴보았을 때, NAFLD환자에게 利水 약재가 포함된 한약치료를 시행하였을 때 치료 유효율의 증가, 혈청지질 및 간 기능수치의 개선, 간 내 지방량의 감소 등 유의미한 효과가 있었음을 알 수 있다.

후속연구

본 연구는 利水 약재를 포함한 한약 처방 치료가 NAFLD에서 유효한 치료수단이 될 수 있다는 의학적인 근거를 제시할 수 있을 것으로 생각한다. NAFLD의 치료를 위해서 향후 利水 약재와 이를 포함한 한약에 대한 보다 과학적이고 체계적으로 설계된 임상 및 실험연구가 활발히 이루어질 필요가 있으며, 본 연구의 결과가 향후 한약 치료의 임상적 연구에서 기초가 되는 바탕 자료로 활용될 수 있을 것이라 생각한다.

	NAFLD의 치료 방법으로는 무엇이 있는가?	이러한 변화를 억제하기 위한 NAFLD의 치료 방법으로는 운동 및 생활습관의 변화, 체중 감량 등이 있다. 약물 치료제로는 Insulin sensitizing agents, Lipid lowering agents, Antioxidants 등이 있으며, 최근에는 Farsenoid X receptor agonists인 Obeticholic acid와 Tumor necrosis factor-α(TNF-α) antagonist 인 Pentoxifylline 등에 대한 임상 연구가 진행되고 있으나 양방에서는 아직 표준 치료제로 선정된 것은 없으며2,8 한방에서는 단일 약재와 복합약물로 나누어 관련 연구가 진행되고 있다.
