노년기에 달성해야할 발달 과업인 자아통합감은 무엇인가? 본 연구에서 근거이론적 접근을 통하여 도출한 노인의 암 진단 이후 삶의 경험 핵심범주는 ‘손상된 자아통합감을 회복하여 암과 함께하는 일상을 찾아가는 여정’이었으며, 삶의 과정을 축약하여 정리하면 위기의 단계, 재구조화의 단계, 자아통합의 단계로 전개되었다.Erikson 등[15]은 노년기에 달성해야 할 중요한 발달과업으로 자아통합감을 제시하였는데, 이는 일생을 후회 없이 수용하고 현재 생활에 만족하며 죽음에 대한 공포가 없는 상태로서 심리적 안녕상태를 반영하는 개념이다. 본 연구의 참여자들은 성공적인 노화를 이룬 노인의 일반적인 자아통합감의 개념에 더하여 암 투병이라는 특수한 상황 속에서 자아통합감을 정립해 나갔다.
노인 암환자를 대할 때 의학적인 중재에 초점을 두는 경우, 발생할 수 있는 문제점은 무엇인가? 노인 암환자를 돌보는 의료인은 암이 다양한 잠재적 결과를 가져올 수 있는 질환이기 때문에 환자를 대할 때 의학적인 중재에 초점을 두기 쉽다. 그러나 암을 진단받은 개인은 투병 상황에 대해 미처 준비하지 못하기 때문에 가장 취약하고 손상받기 쉬운 상태에 놓이게 된다[9]. 이를 고려할 때 간호사는 노인 암환자들이 암 진단과 치료과정에서 겪게 되는 어려움을 이겨내고 일상생활로 재적응 할 수 있도록 환자의 경험에 초점을 두고 환자의 요구에 따른 보다 효율적이고 개별적인 간호중재를 제공할 필요가 있다.
