Purpose: The purpose of this study was to identify the mediating effect of compassionate competence on the relationship between nurses' spirituality and attitude toward care of dying patients. Methods: A cross-sectional correlational research design was conducted. The total subjects were 172 nurses at one general hospital in Gangwon-do. Data were collected using self-report questionnaires and analyzed using t-test, one-way ANOVA, Scheffe's test, Pearson's correlation coefficients, and multiple regression analysis. Results: Compassionate competence showed the mediating effect (${\beta}=.19$, p=.025) on the relationship between nurses' vertical spirituality and attitudes toward care of dying patients. However, no mediating effect of compassionate competence on the relationship between nurses' horizontal spirituality and attitudes toward care of dying patients was found. Conclusions: Based on the findings of this study, development of training programs with a focus on spirituality and compassionate competence was highly recommended to improve and maintain nurses' positive attitudes toward caring of the dying patients.

제안 방법

대상 데이터

본 연구 대상자는 W시에 소재한 Y대학병원 간호사를 대상으로 하였으며, 연구대상자 선정기준의 근거는 본 연구의 설문응답에 신뢰성을 증가시키고자 임종간호가 가장 흔히 이루어지는 내과병동, 중환자실 및 응급실에서 근무하는 간호사로 근무경력이 6개월 이상이며 임종간호를 경험한 대상자로 하였다. 대상자 수는 G power 3.

데이터처리

첫째, 일반적 특성은 백분율, 실수, 평균과 표준편차를 이용하여 분석하였으며, 둘째, 영성, 공감역량, 임종간호태도 정도는 평균과 표준편차로 분석하였다. 셋째, 대상자의 일반적 특성에 따른 임종간호태도 차이는 t-test와 one way-ANOVA로 분석하였으며, 사후분석은 Scheffe test로 분석하였다. 넷째, 영성, 공감역량, 임종간호태도 간의 상관관계는 Pearson Correlation coefficient를 이용하여 분석하였다.

이론/모형

본 연구는 Frommelt [18]의 ‘Frommelt Attitudes toward Care of the Dying Scale [FATCOD]’를 번안한 Cho와 Kim [19]의 임종간호태도 도구를 사용하였다. 이 척도는 30문항으로 구성된 Likert 4점 척도로 ‘전혀 아니다’ 1점에서 ‘매우 그렇다’ 4점으로 구성되어 있으며, 환자에 대한 간호사의 태도 20문항과 환자의 가족에 대한 간호사의 태도 10문항으로 이루어져 있다.
본 연구는 Park [16]이 Howden [17]의 영성 척도를 Oh와 Kang [7]의 수직적 차원과 수평적 차원의 개념을 적용하여 재개발한 영성척도를 사용하였다. 영성척도는 관계성, 초월성, 통합성, 삶의 의미와 목적의 개념을 포함하여, 자신을 능가하는 힘을 지닌 어떤 것과의 초월적인 관계의 의미를 지닌 수직적 영성의 하위척도와 영적 영역의 삶의 의미와 목적과 관계되는 역동적인 기능으로 사회적 및 환경적 관계에서 실존적 의미를 내포하고 있는 수평적 영성의 하위척도로 구성되어 있다.

성능/효과

본 연구에서 임종환자를 돌보는 간호사의 영성과 임종간호태도와의 관계에서 공감역량의 매개효과를 확인한 결과, 수직적 영성과 임종간호태도와의 관계에서 공감역량의 매개효과가 있는 것으로 규명되었다. 이러한 결과는 초월적인 내재된 에너지를 가지고 삶의 의미를 추구하는 간호사는 공감역량을 통해 긍정적인 임종간호태도를 가지게 될 수 있음을 보여주었다.
본 연구에서 임종환자를 돌보는 간호사의 영성과 임종간호태도와의 관계에서 공감역량의 매개효과를 확인한 결과, 수직적 영성과 임종간호태도와의 관계에서 공감역량의 매개효과가 있는 것으로 규명되었다. 이러한 결과는 초월적인 내재된 에너지를 가지고 삶의 의미를 추구하는 간호사는 공감역량을 통해 긍정적인 임종간호태도를 가지게 될 수 있음을 보여주었다.

후속연구

본 연구의 대상자는 일개 종합병원에서 임종환자를 경험하는 간호사를 대상으로 하였으므로 연구결과를 일반화하는 데는 제한점이 있어 연구결과를 해석할 경우에는 신중을 기하여야 한다. 임종을 주로 경험하는 간호사뿐만 아니라 경험이 없는 간호사를 포함한 모든 임상간호사를 대상으로 간호사의 영성, 공감역량 및 임종간호태도간의 관계와 공감역량의 매개효과를 파악하는 반복 연구가 필요하다. 또한 임상간호사를 대상으로 임종간호태도에 영향을 미치는 다양한 요인을 파악하는 연구가 요구된다.
