Purpose: The purpose of study was to provide basic data for developing interventions that could help middle-aged adults prepare for dignified death in the future by examining their attitudes toward death and advance directives (AD), and knowledge of AD. Methods: Data were collected through a survey of 211 middle-aged adults from January 28 to February 28, 2019, in a city located in Gangwon Province. To analyze the data, descriptive statistics, t-test, one-way ANOVA, Scheffé test, Pearson's correlation coefficient and stepwise linear regression were utilized using SPSS/WINdows 21.0. Results: The average score of the participants' attitudes toward death and AD, and knowledge of AD was 91.82±10.89, 15.53±4.27, 46.00±9.45, respectively. There were positive correlations between attitudes toward dignified death and attitudes toward AD, and knowledge of AD. Factors that influence attitudes toward AD were shown in the order of attitudes toward dignified death, knowledge of AD, and intentions to write AD (Adjusted R2=.24). Conclusion: For the dignified death in the future, it is necessary to provide middle-aged adults with an opportunity to think about the need of AD. In addition, extensive education and promotion of AD are required to correct the misunderstanding of AD.

본 조사 시행 전 중년기 성인 20명을 대상으로 사전 조사를 시행하여 이해도 및 소요시간을 확인하였다. 본 조사는 연구자가 관공서에 직접 방문하여 직원 또는 민원실을 이용하는 민원인 중 중년기 성인을 편의 추출하여 연구의 목적과 참여 방법에 대해 직접 구두로 설명한 후 서면 동의한 경우에 설문 조사를 시행하였다. 설문지 작성은 약 20~25분 정도 소요되었으며, 탈락률을 줄이기 위해 대상자들이 혼동하거나 궁금해하는 부분은 다시 설명하였고, 빠진 문항이 없는지 확인하도록 하였다.

대상 데이터

연구대상자는 강원도 소재 일개 도시에 거주하는 중년기성인을 표적 모집단으로 하고 두 개의 관공서에 근무하는 근무자 또는 민원실을 이용하는 민원인 중 40세 이상 65세 미만의 중년기 성인 중 스스로 일상활동을 영위하며, 지역사회에 거주하고, 의사소통이 가능하며, 연구의 목적을 이해하고 스스로 결정하고 동의한 대상자이다. 표본의 크기는 대상자의 사전연명의료의향서에 대한 태도에 미치는 영향을 파악하기위해 유의수준 .
자료수집은 강원도 C시에 소재한 대학병원 임상연구심의위원회(Institutional Review Board, IRB)에서 승인을 얻은 후 연구자가 임의 선정한 C시의 두 개의 관공서에 직접 방문하여 연구의 목적을 설명한 후 자료수집에 관한 허락을 받아2019년 1월 28일부터 2월 28일까지 이루어졌다.
연구대상자는 강원도 소재 일개 도시에 거주하는 중년기성인을 표적 모집단으로 하고 두 개의 관공서에 근무하는 근무자 또는 민원실을 이용하는 민원인 중 40세 이상 65세 미만의 중년기 성인 중 스스로 일상활동을 영위하며, 지역사회에 거주하고, 의사소통이 가능하며, 연구의 목적을 이해하고 스스로 결정하고 동의한 대상자이다. 표본의 크기는 대상자의 사전연명의료의향서에 대한 태도에 미치는 영향을 파악하기위해 유의수준 .05, 효과 크기 .15, 검정력 95%, 회귀분석(독립변수: 14개)에 필요한 대상자 수를 G*Power를 이용해서 산출한 결과 최소 192명이었으며, 탈락률 10.0%를 고려하여212명에게 설문 조사를 시행하였고 응답이 부실한 설문지 1부를 제외한 총 211명의 자료를 최종 분석하였다.

데이터처리

0 프로그램을 이용하여 다음과 같이 분석하였다. 1) 대상자의 특성은 빈도 분석을 이용하였고, 2) 대상자의 품위 있는 죽음에 대한 태도, 사전연명의료의향서에 대한 지식 및 태도는 평균과 표준편차를 이용하였으며, 3) 대상자의 특성에 따른 품위있는 죽음에 대한 태도, 사전연명의료의향서에 대한 지식 및 태도의 차이 비교는 t-test, one way ANOVA 및 사후 분석(Scheffé test)을 이용하였다. 4) 대상자의 품위 있는 죽음에 대한 태도, 사전연명의료의향서에 대한 지식 및 태도 간의 상관관계는 Pearson's correlation coefficient를 이용하였고, 5) 사전연명의료의향서에 대한 태도에 영향을 주는 요인 분석은 단계적 회귀분석을 이용하여 분석하였다.

성능/효과

중년기 성인인 본 연구대상자의 품위 있는 죽음에 대한 태도와 사전연명의료의향서에 대한 태도는 비교적 긍정적이며, 자연스러운 죽음을 품위 있는 죽음이라고 생각하는 것으로 나타났다. 사전연명의료의향서에 대한 지식은 비록 이전에 비해 높아졌으나 여기에서 사용되는 용어나 이에 포함된 의료 행위나 서비스, 문서에 대한 법적인 것들에 대해서는 정확히 알지 못하고 있었다. 이를 개선하기 위해 품위 있는 죽음에 대한 태도, 사전연명의료의향서에 대한 지식 및 태도는 서로 상관관계가 있을 뿐만 아니라 사전연명의료의향서의 작성 의사가 사전연명의료의향서에 대한 태도에 영향을 미치는 것을 고려하여 평소 이들이 관심을 가질 수 있도록 품위 있는 죽음에 대하여 고민할 기회를 만들고, 사전연명의료의향서에 대한 잘못된 정보나 오해를 바로잡기 위한 체계적인 교육과 홍보가 필요하며, 상담 시 가족과 의료진의 역할이 중요할 것으로 보인다.
중년기 성인인 본 연구대상자의 품위 있는 죽음에 대한 태도와 사전연명의료의향서에 대한 태도는 비교적 긍정적이며, 자연스러운 죽음을 품위 있는 죽음이라고 생각하는 것으로 나타났다. 사전연명의료의향서에 대한 지식은 비록 이전에 비해 높아졌으나 여기에서 사용되는 용어나 이에 포함된 의료 행위나 서비스, 문서에 대한 법적인 것들에 대해서는 정확히 알지 못하고 있었다.

후속연구

이를 개선하기 위해 품위 있는 죽음에 대한 태도, 사전연명의료의향서에 대한 지식 및 태도는 서로 상관관계가 있을 뿐만 아니라 사전연명의료의향서의 작성 의사가 사전연명의료의향서에 대한 태도에 영향을 미치는 것을 고려하여 평소 이들이 관심을 가질 수 있도록 품위 있는 죽음에 대하여 고민할 기회를 만들고, 사전연명의료의향서에 대한 잘못된 정보나 오해를 바로잡기 위한 체계적인 교육과 홍보가 필요하며, 상담 시 가족과 의료진의 역할이 중요할 것으로 보인다. 본연구결과를 토대로 죽음이나 연명의료에 대하여 좀 더 자유롭게 이야기할 수 있는 사회적 문화를 조성하기 위한 생애 주기에 맞는 교육 프로그램을 개발하고 그 효과를 검증하는 연구를 제언하는 바이다.
