Purpose: This study attempts to provide basic data for establishing and implementing an advanced directive (AD) system that helps identify dignified death attitudes and reduces death anxiety of the preliminary, through preparation of an AD. Methods: Data were collected from 135 preliminary elderly aged 55 to 64 years, recruited from health centers and welfare facilities. All data were analyzed using SPSS 22.0. Results: Overall scores of the participants were 2.26±0.64 for death anxiety scale, and 3.16±0.44 for dignified death attitude. Scores of individuals who prepared an AD were determined to be 2.19±0.64 for death anxiety, and 3.34±0.44 for dignity of death. The difference in scores obtained for dignified death attitude was significantly higher for the group that signed an AD, as compared to the group with no AD (F=14.81, p<0.001). Conclusions: Results of this study reveal that preliminary elderly who sign an AD have a higher dignified death attitude score as compared to subjects who do not sign an AD. Additionally, the former group of participants desire a dignified end to their life. This indicates a necessity to promote public campaigns for ADs, and to develop educational programs that assist the elderly to prepare for a dignified death and make autonomous decisions.

대상 데이터

본 연구는 만 55세 이상 만 65세 미만의 예비노인을 대상으로 하였으며 J도 소재의 사전연명의료의향서 등록기관에서 사전연명의료의향서를 작성한 예비노인과 보건소와 복지시설을 이용한 사전연명의료의향서를 작성하지 않은 예비노인 중 의사소통이 가능하며 설문의 목적과 내용을 설명한 후 본 연구의 목적을 이해하고 연구 참여에 동의한 자를 대상으로 선정하였다.
연구의 대상자 수 산정을 위해 G power 3.1.9.2 program을 이용하여 공변량분석(ANCOVA)을 기준으로 유의수준(α) .05, 검정력(1-β) .80, 중간정도의 효과 크기(f) .25, 자유도(df) 1로 설정하여 표본수를 산출하여 그 결과 각 그룹당 64명으로 최소 128명이 산출되었으나 탈락률을 고려하여 사전연명의료의향서를 작성한 그룹 70명, 작성하지 않은 그룹 70명 총 140명에게 설문지를 배부하여 모두 회수하였으나 그 중 불완전하게 응답한 5부를 제외하고 사전연명의료의향서를 작성한 그룹 68명, 작성하지 않은 그룹 67명의 총 135명을 최종 분석 대상자로 선정하였다.

데이터처리

∙ 대상자의 사전연명의료의향서 작성군과 비작성군의 죽음불안과 품위있는 죽음태도 정도는 두 군 간에 차이가 있는 변수에 대한 값을 공변량으로 하는 ANCOVA로 분석하였다.
∙ 대상자의 일반적 특성에 따른 죽음불안과 품위있는 죽음태도의 차이는 independent t-test, One-way ANOVA로 분석하고 사후검정은 scheffé test로 분석하였다.

이론/모형

죽음불안을 측정하기 위해 Collett와 Lester (1969)가 개발한 Fear Death Scale과 Templer (1970)가 개발한 Death Anxiety Scale을 토대로 Thorson과 Powell (1992)이 수정한 수정된 죽음불안 도구(Revised Death Anxiety Scale, RDAS)를 Park (1995)이 한국인을 대상으로 수정·보완한 죽음불안 도구를 저자의 동의를 얻어 사용하였다.
품위있는 죽음태도는 Jo (2011)가 한국 성인을 대상으로 개발한 품위있는 죽음태도 측정도구를 저자의 동의를 얻어 사용하였다. 본 척도는 총 30문항으로 정서적 안위유지(10문항), 사회적 관계정리(9문항), 고통 없음(3문항), 자율적 의사결정(4문항), 역할보존(4문항)의 5가지 하위영역으로 구성되어 있으며, ‘전혀 아니다’(1점)부터 ‘매우 그렇다’(4점)까지의 4점 Likert 척도로 점수가 높을수록 품위있는 죽음을 희망하고 품위있는 죽음에 대한 태도가 긍정적임을 의미한다.

성능/효과

연구 결과, 사전연명의료의향서 작성자와 비작성자의 죽음불안은 유의한 차이가 없었다. 그러나 사전연명의료의향서를 작성한 예비노인은 비작성 대상자에 비해 품위있는 죽음태도 점수가 더 높은 것으로 나타났다. 즉, 사전연명의료의향서를 작성한 대상자는 비작성 대상자보다 인간적 품위를 지니고 생을 마감하길 원하는 것을 알 수 있었다.

또한 죽음에 대한 수용적인 태도와 경제상태가 서로 관련이 있는 선행 연구 결과에 비추어 볼 때(Bae, 2018), 스스로 인지 하는 소득수준이 높을수록 자신의 죽음에 이르는 과정을 자율적으로 결정하여 수용하고자 하는 의지가 상대적으로 더 강해 그에 따른 마음가짐의 차이가 사전연명의료의향서 작성에도 영향을 미쳤을 것으로 여겨진다. 따라서 학력이 낮거나 소득수준이 낮은 대상자에게 사전연명의료의향서에 대한 적극적인 홍보와 품위있는 죽음에 대한 교육프로그램이 필요할 것으로 사료되며, 사전연명의료의향서 작성 및 등록기관을 확대하여 접근성을 높여야 할 필요성이 있다.
그러나 죽음불안과 성별의 관계에서 여성이 남성보다 죽음불안이 더 크다는 연구와(Kim & Kim, 2009), 그 반대의 결과를 보고한 연구(Kim & Lee, 2013), 연령이 높을수록 죽음불안이 낮다는 연구결과(Kang & Han, 2013)와 신앙심이 강한 사람은 죽음불안 정도가 낮다(Park, 1995)는 선행연구 결과와는 차이가 있다. 이러한 차이는 선행연구가 대부분 의료인과 노인을 대상으로 하였기 때문에 예비노인을 대상으로 한 본 연구 결과와 차이가 나타난 것으로 보이며, 향후 예비노인을 대상으로 죽음불안에 대한 반복적인 연구가 필요할 것으로 판단된다.
