Purpose : The purpose of this study was to identify relation of the communication skills, humanistic knowledge and professional quality of life and to investigate the enhancing ways of professional quality of life in intensive-care unit nurses. Methods : The study design was a descriptive survey from 152 intensive-care unit nurses, from December 1 to 30, 2019. The data analysis was with ANOVA, Scheffé test, Pearson's correlation coefficient and Multiple regression using the SPSS/WIN 23.0 program. Results : The factors affecting compassion satisfaction were communication skills (β=0.42, p=.001), satisfaction for working (β=0.61, p=.001), marriage (β=0.23, p=.005), humanistic knowledge (β=0.47, p=.008) and these factors explained 35.6%. The factors affecting secondary traumatic stress were job stress (β=0.19, p=.011), number of patients per nurse (β=-0.36, p=.004) and these factors explained 9.4%. The factors affecting burnout were job stress (β=-0.25, p=.001), communication skills (β=-0.29, p=.001), working period(β=-0.18, p=.010), satisfaction for working (β=-0.31, p=.001), self care (β=-0.11, p=.033) and these factors explained 36%. Conclusion : Therefore, it is necessary to develop solution to improve professional quality of life in intensive-care unit nurses. It needs to develop programs to improve communication skills and humanistic knowledge in order to enhance compassion satisfaction and to improve communication skills to prevent burnout.

제안 방법

본 연구는 부산광역시에 소재하는 P 대학교의 기관 생명윤리위원회(institutional review board, IRB)의 승인을 받아 진행되었다(승인번호: 1041386-201907-HR-30-02). 자료의 수집은 구조화된 설문지를 이용하여 2019년 12월 1일부터 2019년 12월 31일까지 이루어졌으며, 자료 수집에 앞서 해당 병원 간호부에 연구의 목적과 방법을 설명하고 협조를 받았다. 연구 대상자의 윤리적인 측면을 고려하여 연구의 목적과 방법, 자료의 비밀보장 및 연구자료 관리, 설문지 작성방법에 대하여 충분한 설명을 제공하여 자발적인 연구 참여에 동의한 경우 체크표시를 하고 설문지에 응하도록 하였다.

대상 데이터

본 연구의 대상자는 B 광역시에 소재한 300병상 이상의 중환자실 5곳에서 중환자실에 근무한 지 1년 이상이 경과한 간호사로 연구의 목적과 방법을 이해하고 연구에 참여하기를 동의한 간호사를 편의 추출하였다. 표본 수는 G*power 3.
80을 기준으로 독립변수 17개를 설정하여 146명이 산출되었다. 이를 근거로 설문지 탈락률 10%를 고려하여 160명의 간호사에게 설문지를 배부하였으며, 이 중 불성실하게 응답한 8부를 제외한 총 152부(회수율 95%)를 분석하였다. ;

2) 대상자의 특성에 따른 의사소통능력, 인문학적 소양, 전문직 삶의 질 정도의 차이는 independent t-test, one-way ANOVA로 확인하였고, 사후 검정은 Scheffe test로 분석하였다. ;
3) 대상자의 의사소통능력, 인문학적 소양, 전문직 삶의 질 정도의 상관관계는 Pearson’s correlation coefficients로 분석하였다.

이론/모형

인문학적 소양이란 인문학의 분야를 많이 접하고, 지식을 많이 습득할수록 강화된다는 전제하에 인문학에 포함되는 역사, 철학, 예술 등의 요소들에 대해서 많이 알고 있는 정도와 관심 있는 정도를 의미한다(Kim, 2014). 본 연구에서 인문학적 소양은 Kim (2014)의 연구에서 사용한 도구로 측정된 점수를 의미하며, 도구 개발자에게 사용 허가를 받고 사용하였다. 본 도구는책 읽기 소양, 역사 소양, 문학⋅철학적 소양, 예술적 소양의 4개 하위영역의 총 13문항으로 이루어져 있으며, 각 문항은 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점으로 Likert 5점 척도로 측정되었다.
본 연구에서는 Hur (2003)가 개발한 포괄적 대인커뮤니케이션 능력 척도인 Global Interpersonal Communication Competency Scale (GICC)을 사용해 측정한 값을 의미하며, 도구 개발자에게 사용 허가를 받고 사용하였다. 의사소통 능력은 자기 노출, 역지사지, 사회적 긴장 완화, 주장력, 상호작용 관리, 표현력, 지지, 즉시성, 집중력, 효율성, 사회적 적절성, 조리성, 목표 간파, 반응력, 잡음 통제력의 총 15개 문항으로 구성되어있으며, 각 문항은 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점으로 Likert 5점 척도로 측정되었다.
전문직 삶의 질 측정을 위해 Stamm (2010)이 제작한 전문직 삶의 질 홈페이지에 공개된 Professional Quality of Life Scale: Compassion Satisfaction/ Fatigue Subscale Version 5 도구의 한국어판 공개버전을 사용하여 측정하였다. 이 도구는 공감 만족, 이차 외상성 스트레스, 소진으로 구성된 3개의 하위영역으로 구분되며 각 하위영역마다 10개의 문항으로 구성되어총 30개의 문항으로 이루어진다.

성능/효과

765로 2에 가까워 오차 항들 간에는 자기상관이 없는 것으로 나타나 잔차와 등분산성과 정규분포성 가정을 만족하였다. 분석결과 공감만족에 영향을 미치는 유의한 변수는 결혼상태(β =0.23, p=.005), 근무만족도(β=0.61, p=.001), 의사 소통능력(β=0.42, p=.001), 인문학적 소양(β=0.47, p=.008)으로, 이들 변수는 공감 만족을 35.6% 설명하였다(F=17.72. p<.001) (Table 5).

후속연구

둘째, 중환자실 간호사의 인문학적 소양과 의사소통 능력을 높여 전문직 삶의 질을 향상시키기 위한 중재 프로그램 개발과 그 효과를 제안하는 연구를 제언한다.
셋째, 중환자실 간호사의 전문직 삶의 질에 영향을 미치는 요인을 파악하기 위한 추가적인 연구를 제언한다.
