Acute ankle sprain is the most common injury in the lower extremities, and approximately 10% to 40% of acute lateral ankle ligament injury causes chronic pain or instability. For chronic symptoms lasting after an acute sprain, the possibility of joint damage, such as bony structures, ligaments, cartilage, and nerves around the ankle joint, should be considered. Patients with chronic lateral ankle instability usually complain of repeated sprains or giving way sensations. There has been steady progress in the treatment options until recently, however new treatments are still being attempted. This paper describes the causes, diagnosis, and recent trends in the conservative and operative treatment of chronic lateral ankle instability.

제안 방법

관절운동범위를 검사하는데 관절의 강직 여부뿐만 아니라 슬과절 신전위와 굴곡위에서 각각 족관절의 족배굴곡 각도가 정상인지, 즉 비복근의 충분한 이완 여부를 Silfverskiöld 검사를 통해 관찰한다.
그 외에 전외측 부종과 후외측 부종은 전거비인대 손상이나 비골 건지대의 손상 후에 발생하기도 하고 족근동의 압통이나 부종, 족관 절 자체의 활액막 비후나 활액 증가가 동반되었는지도 관찰한다. 족 관절 후외측의 만성 부종, 뻑뻑한 느낌은 족관절 만성 불안정에 잘 동반되는 증상이다.
비골근과 후경골근 등 내외번과 관련된 근육의 근력을 확인한다. 족관절 중립위에서 검사자가 한 손으로 족부 외측에 저항을 가한 상태에서 이를 극복하여 외번시켜 비골근의 근력을 평가한다.
부하 검사(stress test)는 방사선학적 검사 외에도 신체검사로도 판 단할 수 있는데 명확한 판단 기준이 없으므로 건측과의 비교가 필요 하다. 슬관절을 굴곡시켜 족관절의 과도한 긴장을 피한 후 전방 전 위와 내반 부하 검사를 시행한다. 전방 전위 검사는 검사자가 환자의 후족부를 전방으로 당겨 전위 정도를 관찰하기도 하지만 뒤꿈치를 바닥에 대고 경골을 후방으로 밀어 검사할 수도 있다.
비골근과 후경골근 등 내외번과 관련된 근육의 근력을 확인한다. 족관절 중립위에서 검사자가 한 손으로 족부 외측에 저항을 가한 상태에서 이를 극복하여 외번시켜 비골근의 근력을 평가한다. 근력이 현저하게 떨어져 있거나 근위축이 발견되면 다른 신경학적 검사가 필요하다.

이론/모형

관절운동범위를 검사하는데 관절의 강직 여부뿐만 아니라 슬과절 신전위와 굴곡위에서 각각 족관절의 족배굴곡 각도가 정상인지, 즉 비복근의 충분한 이완 여부를 Silfverskiöld 검사를 통해 관찰한다. 이때 족배굴곡 시 후족부가 내번 혹은 외번되지 않는지와 전신 관절의 과한 이완 여부도 Beighton scale 등을 통해 관찰한다.

성능/효과

그 외 에 컴퓨터 단층촬영(computed tomography), 자기공명영상(magnetic resonance imaging) 등을 통해 인대의 결손 위치나 인대조직의 비후나 잔존 여부, 연골 병변 등 동반 손상 등을 발견할 수 있다.¹³⁾ 최근에는 동적인 평가가 가능한 초음파 검사가 많이 쓰이고 있으며 특히 비골건 손상이 동반 경우 유용한 검사가 될 수 있고 부하 검사와 더불어 시행하면 관절 불안적과 인대의 병변을 유기적으로 확인할 수 있다.
비수술적 재활운동은 4단계로 구성되는데, 관절가동운동(range of motion exercise), 근력 강화 운동, 균형감각 회복 및 기능적 운 동이다. 이 중 균형감각 훈련이 환자의 자가 기능 평가에서 가장 효과적인 것으로 보이며 염좌의 재발 방지에도 효과가 있다. 그러나 염좌의 병력이 뚜렷하지 않은 경우에는 그 효과 역시 명확하게 증명되지 않았다.
