Purpose: This study aims to investigate major traumatic events experienced by nurses in regional trauma centers and explore the relationship among their traumatic events experience, perceived stress, and stress coping. Methods: Data were collected from 208 nurses in the trauma emergency room (trauma-bay) and trauma intensive care unit at four regional trauma centers. Results: The mean score of the traumatic events experience was 44.3 out of 76 points. The scores for physical injuries caused by traffic accidents or falls as well as patient care with abnormal behaviors were high. Significantly positive correlations among traumatic events experience, perceived stress, and stress coping were identified. Conclusion: Nurses working in the regional trauma centers experienced many various traumatic events, leading to high levels of stress. This study suggests that it is necessary to establish a regular surveillance system for nurses' traumatic events experience and perceived stress.

대상 데이터

본 연구는 권역외상센터 15개 중 2012~2013년 사이에 권역외상센터에 지정된 4개 병원의 권역외상센터 외상소생실과 외상중환자실에 근무하는 경력 6개월 이상의 간호사를 대상으로하였다. 이들 권역외상센터는 국가에서 시행한 운영 평가에서 적합성이 인정되어 개소식이 거행된 의료기관을 기준으로 선정하였다.
각 권역외상센터에 우편으로 설문지를 배부하고 회수할 예정이므로 높은 탈락률을 고려하여 설문지 230부를 2016년 10월 15일부터 10월 31일까지 배부하였다. 이중 221부가 회수되었으며, 응답에 부실한 13부를 제외하고 208부를 최종분석에 사용하였다.

데이터처리

Lazarus와 Folkman (1984)이 개발한 69개 문항으로 이루어진 스트레스 대처방식 척도를 Kim (1987)이 수정한 62개 문항의 한글 도구를 사용하였다. 적극적 대처와 소극적 대처의 2개 하위유형으로 크게 구분되며, 각 문항은 ‘사용하지 않음’, ‘약간 사용’, ‘어느 정도 사용’, ‘아주 많이 사용’의 Likert식 4점 척도이다.
지각된 스트레스 도구는 Cohen, Kamarck와 Mermelstein(1983)에 의해 개발되어 1988년 개정한 10개 문항의 Perceived Stress Scale (PSS)을 Lee 등(2012)이 번역한 한글 도구를 사용하였다. 각 문항의 내용을 전혀 느끼지 않은 0점부터 매우 자주 느낀 4점까지의 Likert 척도이고, 부정 문항은 역코딩하여 총점을 구하였다(Lee et al.

성능/효과

본 연구는 권역외상센터 간호사 208명을 대상으로 권역외상센터 간호사의 외상사건 경험 정도와 주요 외상사건을 분석하고 지각된 스트레스 및 스트레스 대처방식과의 관계를 파악하기 위해 시도되었다. 분석결과, 외상사건 경험 정도는 중간 이상의 높은 수준이었고, 주요하게 경험하는 외상사건은 교통사고로 인한 심각한 신체 손상(자동차, 열차, 비행기, 선박, 오토바이, 자전거 등), 낙상으로 인한 신체적 손상, 비정상적인 행동(소리지름, 난동, 섬망 등)을 보이는 환자 간호로 나타났다. 외상사건 경험은 지각된 스트레스와 적극적 및 소극적 스트레스 대처방식과 유의한 상관관계가 있었다.
연구대상자의 외상사건 경험 정도는 총점 76점에 평균 44.3점이었다. 선행연구와의 비교를 위해 총점 100점으로 환산하여 비교해 본 결과 본 연구의 외상사건 경험 정도는 평균 58.
분석결과, 외상사건 경험 정도는 중간 이상의 높은 수준이었고, 주요하게 경험하는 외상사건은 교통사고로 인한 심각한 신체 손상(자동차, 열차, 비행기, 선박, 오토바이, 자전거 등), 낙상으로 인한 신체적 손상, 비정상적인 행동(소리지름, 난동, 섬망 등)을 보이는 환자 간호로 나타났다. 외상사건 경험은 지각된 스트레스와 적극적 및 소극적 스트레스 대처방식과 유의한 상관관계가 있었다. 따라서 권역외상센터에서 급성기 환자간호를 담당하는 간호사는 외상사건 경험의 정도가 높고 이로 인한 지각된 스트레스를 겪을 수 있으므로, 정기적인 감시체계와 중재전략을 마련하는 것이 필요하다.

후속연구

특히, 근무경력이 많을수록 많은 외상 환자를 간호하게 되어 외상사건 경험은 증가할 수 밖에 없으므로, 외상사건 경험이 집중되지 않도록 권역외상센터 내 인력 보강이 필수적이며 높은 스트레스 수치를 감소시키기 위한 대처 중재와 함께 이루어져야 함을 시사한다. 그러나 권역외상센터 간호사를 대상으로 이에 대해 살펴본 선행연구가 부족한 상태이므로, 스트레스의 원인과 외상사건의 영향 정도를 구체적으로 살펴볼 수 있는 질적 연구를 비롯한 추후 연구가 필요하다.
둘째, 권역외상센터 간호사가 외상사건 경험에 대처할 수 있도록 직무교육 시 활용할 수 있는 외상사건 경험 대처 프로그램 개발을 제언한다.
첫째, 본 연구는 전국 15개 권역외상센터 중 4개의 권역외상센터 만을 대상으로 하였으므로 전체 권역외상센터를 대상으로 하는 반복연구를 제언한다.
