Purpose: This descriptive correlational study examined the acculturation, health literacy, and factors affecting the health literacy of Vietnamese and Chinese female marriage immigrants. Methods: The sample consisted of 90 Vietnamese and 89 Chinese female marriage immigrants who participated in Multicultural Family Support Centers' programs in metropolitan city A. The health literacy was measured by HLI-FMI (Health Literacy Index for Female Marriage Immigrants), acculturation was measured using the Acculturation Index for female marriage immigrants. Descriptive statistics, t-test, Chi-square, one-way ANOVA, Scheffé test, Pearson correlation coefficient, and multiple linear regression were performed for data analysis. Results: The Chinese group (M=9.80±2.72) showed a significantly higher health literacy than the Vietnamese group (M=8.07±3.57). The factors affecting health literacy among Chinese were the length of residence (β=.442 p<.001), and adaptation to Korean culture (β=.381, p=.007). Conclusion: To develope graded education programs for promoting the health literacy of Chinese and Vietnamese female marriage immigrants, the length of residence can be considered. Continuously providing a support system to help them adapt to Korean culture can also have a positive effect on health literacy.

건강문해력 측정도구는 Yang과 Chee(2017)가 skills(print, numeracy), tasks(access, understand, appraise, apply), health contexts(health promotion and disease prevention, health care maintenance and treatment, health system navigation) 등 3개의 구성개념을 기반으로 개발한 Health Literacy Index for Female Marriage Immigrants(HLI-FMI)를 사용하였다.
결혼이주여성의 문화적응은 Kim (2011)이 개발한 ‘여성결혼이민자 문화적응척도’를 사용해 측정했다. 이 척도는 국내 여성결혼이민자를 대상으로 개발되었으며, 한국문화적응 척도 26문항, 모국문화유지 척도 19문항으로 총 45문항이다.
본 연구의 설문지는 건강문해력 측정도구 HLI-FMI(Yang & Chee, 2017), 문화적응 측정도구인 여성결혼이민자 문화적응척도(Kim, 2011), 인구학적 특성 15문항으로 구성되어있다.

성능/효과

다중회귀분석 결과 베트남 결혼이주여성의 건강문해력에 유의하게 영향을 미치는 변수는 없는 것으로 나타났으며, 중국 결혼이주여성은 거주기간(β=.442 p<.001), 한국문화적응(β=.381, p=.007)이 건강문해력에 유의한 영향을 미치는 변수로 확인되었고 설명력은 47.5%이었다.

본 연구는 결혼이주여성의 사회·문화적 배경을 고려한 건강문해력 및 영향요인을 분석하고자 국내 결혼이주여성을 대상으로 이들의 사회문화적 배경을 고려해 개발된 도구를 사용하여 연구 결과가 보다 높은 의의를 가진다. 따라서 본 연구는 국내 거주 중인 베트남, 중국 결혼이주여성의 건강문해력에 영향을 미치는 영향 요인을 고려한 수준별 교육 및 프로그램의 개발에 필요한 기초 자료를 제공함으로써, 건강문해력 향상을 위한 효과적인 전략을 마련하는 데 도움을 줄 수 있다.
이는 결혼이주여성을 대상으로 건강문해력과 관련된 교육이나 프로그램을 제공할 때 출신국가가 고려될 필요성이 있음을 시사한다. 따라서 출신국에 따른 건강문해력 수준을 고려한 건강 관련 정보의 제공 및 교육이 제공되어야할 것이다. 본 연구 결과 거주기간이 결혼이주여성의 건강문해력 수준에 유의한 영향을 주는 요인임을 확인하였으므로 이들의 수준별 건강 교육 및 프로그램을 개설할 때, 거주기간을 고려하는 것이 도움이 될 것이다.
이 밖에도 국적취득유무, 주관적 건강상태 등이 결혼이주여성의 건강문해력에 영향을 주는 요인으로 밝혀져(Kim & Koo, 2017), 이에 대한 추가 적인 연구가 더 필요할 것으로 사료된다. 본 연구는 일개 광역시의 다문화지원센터 프로그램에 참여중인 베트남, 중국 결혼이주여성만을 대상으로 하였으므로 추후 지역을 전국으로 확대하는 것이 필요할 뿐만 아니라, 다문화지원센터를 비롯한 지역사회 내의 기관을 이용하지 못하는 사각지대에 놓인 결혼이주여성을 포함할 수 있는 연구가 필요하다. 또한 본 연구는 베트남, 중국 출신 결혼이주여성만을 대상자로 하였으므로 소수국가를 포함한 결혼이주여성을 포함하는 연구가 이루어져야 할 것이다.
