본 연구는 혈액투석을 받는 노인환자를 대상으로 건강문해력, 자가관리지식, 자가간호행위 수준을 파악하고 각 변수간의 관련성을 확인하고자 진행되었다. 자료수집은 인천광역시 내 9개 병원에서 혈액투석을 받고 있는 60세 이상 193명을 대상으로 진행되었다. 연구도구는 건강문해력, 혈액투석에 대한 자가관리지식과 자가간호 행위도구를 사용하였다. 자료의 분석은 t-검정, ANOVA, Pearson 상관분석을 하였다. 연구결과 대상자의 건강문해력은 17점 만점에 평균 13.66(±2.09)점, 자가관리지식은 15점 만점에 평균 13.33(±1.85)점, 그리고 자가간호행위는 80점 만점에 평균 63.95(±10.32)점이었다. 건강 문해력과 자가관리지식 간에는 통계적으로 유의한 양의 상관관계를 보였고(r=.21, p=.003), 건강 문해력의 하위 척도인 독해력과 자가관리지식은 통계적으로 유의한 양의 상관관계를 나타내었다(r=.25, p=.001). 그리고 자가간호행위와 자가관리지식 간에는 유의한 양의 상관관계를 나타내었다(r=0.15, p<.05). 본 연구를 바탕으로 혈액투석을 받는 노인의 자가간호행위를 증진시키기 위해서는 개인별 건강문해력의 수준을 파악하여 이를 토대로 교육을 할 필요가 있겠다.

The purpose of this study was not only to identify the level of health literacy, self-management knowledge and self-care behavior, but also to identify the relations those variables in elders with hemodialysis. The subjects of this study are 193 patients from dialysis clinics of nine hospitals in Incheon who aging over 60 years old. Data collection was conducted by using Health Literacy Scale, Self-Management Knowledge Scale, and Self-Care Behavior Scale. The data were analysed by descriptive analysis, t-test, ANOVA, and Pearson correlation coefficient using SPSS statistics 23.0 program. In the results, average score of subjects' health literacy, self-management knowledge and self-care behavior were 13.66(±2.09), 13.33(±1.85) and 63.95(±10.32), respectively. There were significant positive correlation between self-management knowledge and health literacy (r=0.37, p<.001) and reading comprehension part which is subscale in health literacy (r=.25, p=.001). Also, there was significant positive correlation between self-management knowledge and self-care behavior (r=0.15, p<.05). According to the above results, for improving the self-care behaviors in elders with hemodialysis, it is necessary to provide tailored education based on his/her level of health literacy.

제안 방법

먼저 연구자가 직접 해당기관 인공 신장실을 방문하여 당일 내원환자 리스트와 의무기록을 토대로 60세 이상 노인으로 6개월 이상 이용하고 있는 대상자를 파악하였다. 이후 대상자의 지남력을 파악하기 위해 본인의 이름, 날짜, 장소, 내원 목적을 질문하여 연구 대상자를 선정하였다.
질병 관련 특성에 관한 자료는 설문조사를 마친 당일 오후에 의무 기록 열람을 통해 수집하였다. 수집된 자료는 잠금장치가 있는 연구자의 개인 캐비넷에 보관 후 연구보고가 종료되면 파쇄기를 이용하여 자료를 분쇄할 예정이다.

대상 데이터

자료 수집을 위해 인천광역시 I 대학 기관생명윤리위원회(IRB)에서 승인을 받고 자료를 수집하였다. 대상자모집 기관인 인천광역시 소재 9개 병원 인공 신장실 소속 기관장이나 행정 책임자에게 연구 목적과 방법, 연구 절차를 설명한 후 자료 수집에 대한 동의를 얻은 후자료를 수집하였다.
연구 대상은 인천광역시 소재 9개 병원의 인공신장실에서 만성신부전을 진단받고 혈액 투석 치료를 받고 있는 60세 이상의 노인 환자로 총 210부의 질문지를 배부하여 198부가 회수되었으며 이 중 불완전응답과 의무기록의 미비로 5부가 제외되어 최종 193부가 분석에 사용되었다. 표본수의 산출은 G*Power 3.

성능/효과

그리고 자가간호행위는 자가관리지식과 상관관계를 나타내어 자가간호행위를 증진시키기 위해서는 자가관리지식을 증진시킬 필요가 있겠다. 또한 이러한 자가관리지식은 건강문해력, 특히 독해력과 관련성이 있는 것으로 나타났다. 따라서 혈액 투석 노인의 자가간호행위를 증진시키기 위해서 관련 교육을 제공할 때는 노인의 교육 수준에 대한 고려와 더불어 제공되는 정보의 의미와 내용에 대한 적합한 이해를 도울 수 있는 간호 중재 프로그램이 개발되어야 할 것이다.
본 연구에서는 만성신부전으로 혈액 투석 치료를 받고 있는 혈액투석 노인의 자가간호행위 증진을 모색하기 위해 진행되었다. 연구 결과를 통해 혈액 투석 노인의 자가간호행위는 교육수준, 종교, 배우자 유무, 혈액투석 교육 참여 경험에 따라 차이를 나타낼 수 있음을 확인하였다. 그리고 자가간호행위는 자가관리지식과 상관관계를 나타내어 자가간호행위를 증진시키기 위해서는 자가관리지식을 증진시킬 필요가 있겠다.

후속연구

둘째, 본 연구에서 혈액 투석 환자의 건강문해력 측정 도구와 자가관리지식 측정 도구를 수정 보완하여 사용하였지만 신뢰도가 높지 않게 나타났다. 그 이유는 도구개발의 전체 과정을 따르지 못하고 개발하여 적용하였던 것이 문제점으로 고려될 수 있다.

또한 이러한 자가관리지식은 건강문해력, 특히 독해력과 관련성이 있는 것으로 나타났다. 따라서 혈액 투석 노인의 자가간호행위를 증진시키기 위해서 관련 교육을 제공할 때는 노인의 교육 수준에 대한 고려와 더불어 제공되는 정보의 의미와 내용에 대한 적합한 이해를 도울 수 있는 간호 중재 프로그램이 개발되어야 할 것이다.
첫째, 만성 신부전 환자의 증가에 따른 노인 혈액 투석 환자 수와 평균 수명의 연장을 고려할 때, 신장 이식을 하지 않는 한 평생을 이어가게 될 자가간호행위를증진시키기 위해 특화된 노인 혈액 투석 환자를 대상으로 한 교육 프로그램의 개발이 필요함을 제언한다.
