Purpose: The purpose of this study was to evaluate the effects of client-centered art and craft activities on depression and quality of life in stroke clients. Through this, we aim to find a way to reduce depression and improve the quality of life in stroke clients. Methods: Clients diagnosed with stroke were selected as participants for the study. Participants in the experimental group (n=13) and control group (n=14) received general occupational therapy. Clients in the experimental group participated in client-centered art and craft activities, whereas clients in the control group participated in general art and craft activities for 8 weeks. Beck Depression Inventory(BDI) and Stroke Specific Quality of Life(SS-QOL) were used to evaluate the depression and quality of life of the clients before and after the intervention. Results: The experimental group and control group presented significant statistical difference in depression before and after intervention (p<.01; p<.05). The experimental group showed a greater decrease in depression (p<.05) than the control group. Additionally, the experimental and control group displayed significant statistical difference in quality of life (p<.01) before and after intervention. The experimental group showed a more statistically significant improvement in quality of life (p<.01) than the control group. Conclusion: These results demonstrate the significance of client-centered art and craft activities in reducing depression and improving quality of life in clients with stroke. Therefore, it is expected to be useful in clinical settings. Occupational therapy should be provided based on the decision of the clients.

제안 방법

대조군은 실험군과 동일하게 일반적인 작업치료 중재와 추가로 구조화된 수공예 활동을 1일 30분씩 주 3회, 총 8주간 실시하였다(Table 2). 실험군과 대조군 모두 중재가 끝난 8주 이후 벡 우울 척도(BDI)와 뇌졸중 특이 삶의 질 척도(SS-QOL)로 재평가를 하였다. 모든 평가는 일관성을 위하여 연구자 1인이 단독으로 진행하였다.
대상자 선정기준에 부합하는 30명을 선발하였다. 이후 초기 평가로 우울과 삶의 질을 각각 벡 우울 척도(BDI) 와 뇌졸중 특이 삶의 질 척도(SS-QOL)로 평가하여, 평가점수가 비슷한 대상자에게 짝수(0), 홀수(1)의 무작위 제비뽑기를 통하여 각각 실험군 15명, 대조군 15명 두 그룹으로 나누었다. 그리고 중재 전 우울과 삶의 질에 대한 두 집단 간 동질성을 확인하였다.
중재는 일반적인 작업치료와 추가로 클라이언트 중심 수공예 활동을 중재한 실험군 15명, 일반적인 작업치료와 추가로 구조화된 수공예 활동을 중재한 대조군 15명으로 나누어 실시하였다. 중재기간 동안 중재거부 실험군 1명, 퇴원 실험군 1명과 대조군 1명 등 총 3명이 중도탈락되었으며, 이에 사전 평가부터 재평가까지 실험군 13 명과 대조군 14명이 모든 과정을 완료하였다.

대상 데이터

본 연구는 경상남도에 소재한 A병원에서 컴퓨터 단층촬영(CT) 또는 자기공명촬영(MRI) 상 뇌졸중으로 진단을 받고 해당 기관에 입원하여 작업치료를 받고 있는 환자 중 대상자 선정기준에 부합하는 자를 대상으로 실시하였다. 구체적인 대상자 선정 기준으로 뇌졸중 진단을 받은 자로 유병기간이 6개월 이상인 자, 재활의학과 의사로부터 인지기능이 정상으로 판명받은 자, 상지기능이 Brunnstrom Recovery Stage(BRS) 4단계 이상으로 해당 중재를 독립적으로 수행 가능한 자, 연구의 목적을 이해하고 자발적으로 연구에 참여한 자이다.
대상자 수를 결정하기 위하여 뇌졸중 환자를 대상으로 진행한 수공예 활동 선행 연구들을 검토한 결과, 집단구성원의 인원은 15명 이하가 가장 많았다. 이에 본 연구의 대상자를 집단 간 15명인 실험군 15명, 대조군 15명 총 30 명으로 계획하여 연구를 진행하였다. 하지만 중재 기간 동안 중재 거부, 퇴원 등으로 총 3명이 중도 탈락되었으며, 이에 사전 평가부터 재평가까지 모든 과정을 완료한 실험군 13명과 대조군 14명, 총 27명이 최종적으로 본연구의 대상자로 포함되었다.
이에 본 연구의 대상자를 집단 간 15명인 실험군 15명, 대조군 15명 총 30 명으로 계획하여 연구를 진행하였다. 하지만 중재 기간 동안 중재 거부, 퇴원 등으로 총 3명이 중도 탈락되었으며, 이에 사전 평가부터 재평가까지 모든 과정을 완료한 실험군 13명과 대조군 14명, 총 27명이 최종적으로 본연구의 대상자로 포함되었다.

05). 연구대상자의 일반적 특성은 빈도와 백분율로 분석을 하였으며, 일반적 특성에 따른 동질성 검정을 chi-squared test를 통하여 실시하였다.

둘째, 클라이언트 중심 수공예 활동을 중재한 실험군과 구조화된 수공예 활동을 중재한 대조군의 중재 전‧ 후 삶의 질을 비교한 결과, 실험군과 대조군 모두 삶의 질이 향상하였으며, 실험군이 대조군에 비해 유의하게 삶의 질이 향상되었다.
본 연구를 통하여 클라이언트 중심 수공예 활동이 뇌졸중 환자의 우울과 삶의 질 향상에 효과적임을 확인할 수 있었다. 향후 클라이언트 중심 수공예 활동이 임상에서 유용하게 적용되리라 기대하며, 추후에는 언급한 제한점을 보완한 후속 연구들이 필요할 것이다.
본 연구의 결과로 클라이언트 중심 수공예 활동을 중재한 실험군과 구조화된 수공예 활동을 중재한 대조군의 중재 전‧ 후 우울을 비교한 결과, 실험군과 대조군 모두 유의하게 우울이 감소하였으며, 중재 후 실험군이 대조군에 비해 유의하게 우울이 감소하였다.

후속연구

현재 병원 및 기관에서 개인별로 클라이언트 중심 수공예 활동을 수행하기에는 시간, 비용, 인력 등의 다양한 어려움이 따르는 것은 사실이다. 그렇지만 이러한 제한점으로 인하여 개인별로 고려되지 않는 활동을 시행하는 것보다는 개인의 기능과 가치, 흥미 등을 고려하여 적합한 수공예 활동을 중재한다면 더 효과적인 재활이 될 수 있을 것이라 확신한다.
이는 시기상 대상자 모집과 연구 참여에 제한점이 있었다. 둘째, 중재기간 이후 후속 평가가 이루어지지 못하여, 효과의 지속성에 대해서는 알지 못한다. 셋째, 클라이언트 중심 접근법을 시행한 다수의 연구들은 캐나다작업 수행평가(COPM)를 통하여 작업수행의 질을 비교하였지만, 본 연구에서는 우울과 삶의 질만을 다루었기에 캐나다작업 수행평가(COPM)를 실시하지 않았다.
마지막으로 우울과 삶의 질 평가를 위하여 자기설문형 형식의 평가만 시행하였기에, 좀 더 객관적인 평가가 요구된다. 따라서 추후 연구에서는 이러한 단점들을 보완한 연구가 필요하다.
이에 대상자의 작업수행에 대한 질적인 변화를 확인할 수 없으며, 완전한 클라이언트 중심 접근이라고 하기에는 제한이 있다. 마지막으로 우울과 삶의 질 평가를 위하여 자기설문형 형식의 평가만 시행하였기에, 좀 더 객관적인 평가가 요구된다. 따라서 추후 연구에서는 이러한 단점들을 보완한 연구가 필요하다.
이러한 의의에도 본 연구는 몇 가지 제한점이 따른다. 먼저, 연구대상자가 적어 본 연구의 결과를 일반화하기에는 제한이 있다. 이는 시기상 대상자 모집과 연구 참여에 제한점이 있었다.
이 결과들을 통하여 클라이언트 중심 수공예 활동은 뇌졸중 환자의 우울 감소와 삶의 질 향상을 위한 효과적인 중재로 기대할 수 있을 것이다. 작업치료에서 치료적으로 중재하는 수공예 활동은 창조적이고 생산적인 과정으로 신체적, 정신적으로 다양한 장점을 나타낸다.
본 연구를 통하여 클라이언트 중심 수공예 활동이 뇌졸중 환자의 우울과 삶의 질 향상에 효과적임을 확인할 수 있었다. 향후 클라이언트 중심 수공예 활동이 임상에서 유용하게 적용되리라 기대하며, 추후에는 언급한 제한점을 보완한 후속 연구들이 필요할 것이다.
