The purpose of this study is to determine the relationship between role ambiguity, job burnout, and organizational effectiveness among nurses working in comprehensive nursing care service units and to investigate the mediating effect of job burnout on the relationship between role ambiguity and organizational effectiveness. The participants were 149 nurses working in 10 general hospitals. Data were collected using a self-report questionnaire and analyzed using IBM SPSS Statistics 25.0 and Model 4 of the PROCESS macro program. The results reveal that organizational effectiveness has a significant negative relationship with role ambiguity and job burnout, with job burnout completely mediating the effect of role ambiguity on organizational effectiveness. These findings suggest that interventions to reduce job burnout should be considered to enhance organizational effectiveness.

제안 방법

본 연구에서는 Rizzo와 Lirtzman(1970)가 개발하고 Kim[29]이 번안한 도구를 수정, 보완하여 사용하였다. 이 도구는 역할모호성 7문항, 역할갈등 8문항의 총 15문항으로 구성되어 있다.

대상 데이터

본 연구는 K대학교 기관생명윤리위원회의 승인(IRB NO: 2020–24)을 받은 후 2020년 9월 1일부터 9월 30일까지 C도와 D광역시에 소재한 10개 종합병원에서 자료를 수집하였다
본 연구의 대상자는 C도와 D광역시에 소재한 10개 종합병원을 임의선정하고, 통합서비스병동에서 최소 1년 이상 근무 중인 간호사를 편의표집하였다. 표본수는 G*power 3.
80, 예측변수가 17개일 때 148명의 최소 표본수가 산출되었다. 탈락률 20% 정도를 고려하여 178명에게 설문조사를 실시하였으며, 이 중 설문 응답이 불충분한 7부, 1년 미만인 간호사 22명의 설문지를 제외한 149부의 자료를 최종 분석에 사용하였다.

데이터처리

1) 일반적 특성 변수, 직무관련 변수 및 연구변수 중 범주형 변수는 빈도와 백분율로, 연속형 변수는 평균, 표준편차로 요약하였다.
2) 역할모호성과 직무소진 및 조직유효성 간의 관련성을 보기 위하여 Pearson 상관분석을 실시하였다.
3) 통제변수를 확인하기 위하여 대상자의 일반적 특성, 직무관련 특성에 따른 조직유효성의 차이는 independent t-test, Pearson 상관분석과 one-way ANOVA, 다중비교의 방법으로는 Scheffé 방법으로 분석하였다
4) 역할모호성과 조직유효성의 관계에서 직무소진의 3개 하위요인(정서적 고갈, 비인격화, 개인적 성취감 저하)을 매개변수로 한 병렬다중매개효과 분석은 PROCESS macro Program의 model 4를 이용하여 검정하였다. 이를 위해 Bootstrapping 과정은 10,000회로, 신뢰구간은 95% 수준으로 지정하였으며, 조직유효성에 영향을 미치는 것으로 나타난 일반적 특성과 직무관련 변수를 공변인(covariate)으로 설정하여 그 영향을 통제하였다.
3) 통제변수를 확인하기 위하여 대상자의 일반적 특성, 직무관련 특성에 따른 조직유효성의 차이는 independent t-test, Pearson 상관분석과 one-way ANOVA, 다중비교의 방법으로는 Scheffé 방법으로 분석하였다. 자료가 정규성을 가정하기 어려운 경우 Mann-Whitney U test, Kruskal-Wallis test를 적용하였다.

이론/모형

본 연구에서는 Maslach와 Jackson(1981)이 개발한 소진척도(Maslach Burnout Inventory,MBI)를 Bae[30]가 번안한 도구를 사용하였다. 이 도구는 정서적 고갈(emotional exhaustion) 9문항, 비인격화(depersonalization) 5문항, 개인적 성취 저하(diminished personal accomplishment) 8문항의 총 22문항으로 구성되어 있다.
본 연구에서는 Price와 Muller(1990), Quinn과 Staines(1979)이 개발한 직무만족 설문과 Mowday, Steers와 Porter(1979)가 개발한 조직몰입 설문을 Shin[31]이 번안한 것을 사용하였다. 이 도구는 직무만족 11문항, 조직몰입 9문항의 총 20문항으로 구성되어 있다.

성능/효과

대상자의 역할모호성과 조직유효성과의 관계에서 직무소진의 매개효과를 검정한 결과, 직무소진의 하위요인인 정서적 고갈, 비인격화, 개인 성취감 저하 모두 완전매개 효과가 있는 것으로 나타났다. 즉 간호사가 업무 역할을 모호하게 인식할수록 소진(정서적 고갈, 비인격화와 개인적 성취감 저하)의 위험을 증가시키게 되고 이는 다시 직무만족과 조직몰입 등 조직유효성의 저하로 이어질 수 있다는 것을 의미한다.
본 연구는 종합병원 통합서비스병동 간호사의 역할모호성과 조직유효성의 관계에서 직무소진의 매개효과를 파악하기 위한 서술적 상관관계 연구이다. 연구 결과, 직무소진이 역할모호성과 조직유효성 간의 관계를 완전매개하는 것으로 나타났다.

그러나 대체인력이 부족한 직장이나 경직된 조직 문화로 인하여 휴가를 사용하기 어려운 경우 오히려 소진 발생의 위험요인으로 작용할 수 있다[16]. 따라서 간호사의 자기관리를 장려하기 위해서는 유연한 근무 일정 제공, 적정 의무 휴식 시간 확보, 간호사의 업무량 관리 및 적정 인력 수준 유지가 선행되고, 더 나아가 자기관리 기법에 대한 교육 프로그램 제공과 정기적인 감독 및 피드백 등 의료기관 차원의 지원 정책을 개발해야 할 것이다.
본 연구는 일부 지역 종합병원의 간호사를 대상으로 편의표본추출을 하였으므로 모든 종별 의료기관의 통합서비스병동 간호사로 일반화하는 데 한계가 있다. 향후 다양한 지역, 의료기관 종별로 연구를 시행해 볼 필요가 있다.
이상 살펴본 바와 같이, 지금까지 간호사의 역할모호성에 관한 연구들은 직무소진, 조직유효성과의 관련성을 탐색하는데 주로 관심을 두고 진행되었다, 그렇다면 어떤 인과경로를 통해 역할모호성이 조직유효성을 낮추는지 추가적인 분석이 필요하다. 본 논문에서는 이를 집중적으로 연구하고자 한다.
본 연구는 일부 지역 종합병원의 간호사를 대상으로 편의표본추출을 하였으므로 모든 종별 의료기관의 통합서비스병동 간호사로 일반화하는 데 한계가 있다. 향후 다양한 지역, 의료기관 종별로 연구를 시행해 볼 필요가 있다.
