연구배경: 최근에는 우리나라에서도 비만한 제2형 당뇨병환자가 증가하고 있고 환자에 따라 인슐린 분비 장애보다 인슐린저항성이 제2형 당뇨병 발생에 있어 중요한 병리 기전이 되고 있다. 본 연구의 목적은 혈당 조절이 되지 않는 우리나라 제2형 당뇨병환자를 대상으로 고정 용량의 로지글리타존/메트포르민 복합 제제를 투여하여 혈당 강하 유효성과 약제의 안정성을 평가하고자 하였다. 방법: 본 연구는 당화혈색소 > 11%, 혹은 공복혈당 > 15 mmol/L (270 mg/dL)인 제2형 당뇨병환자 12명 (남성 6명, 여성 6명)으로 하였다. 대상 환자에게 2주간의 선별 기간을 거쳐 24주간 고정 용량의 로지글리타존/메트포르민 복합 제제를 투여하였다. 0주에 로지글리타존/메트포르민 4 mg/1,000 mg 초기 용량으로 시작하여 내약성 문제가 발생하지 않는다면 4주에 6 mg/1,500 mg으로, 8주에 최대 용량인 8mg/2,000 mg으로 증량하였고 24주까지 용량을 유지하였다. 본 연구의 1차 평가 목표로 약제 치료 24주 후 기준 시점으로부터 당화혈색소의 변화를 비교하고자 하였다. 결과: 대상자의 연령은 48.9 ± 10.6세, 체질량지수는 25.0 ± 3.5 kg/m2, 당화혈색소는 12.0 ± 1.0%, 공복혈장혈당은 16.3 ± 3.1 mmol/L이었다. 약제 치료 24주 후 당화혈색소는 7.54 ± 1.45%로, 공복혈장혈당은 7.96 ± 2.38 mmol/L로 감소하였다. 24주 후 당화혈색소가 기저치보다 0.7% 이상 감소하거나 7% 미만으로 된 대상자는 12명 중 11명이었다 (91.7%). 24주 후 대상자 중 5명에서 (41%) 당화혈색소가 7% 미만으로 되었고, 9명에서 (75%) 8% 미만으로 되었다. 6.5% 이하로 된 대상자는 3명 (33.7%), 7% 미만은 5명 (41.7%), 8% 미만은 9명 (75%)이었다. 결론: 로지글리타존과 메트포르민 복합 제제는 혈당 조절이 되지 않는 우리나라 제2형 당뇨병환자에서 혈당 조절에 효과적이었다.
