목 적 : 소아에서 만성 두드러기의 원인과 경과를 알아보았다. 만성 두드러기의 예후에 영향을 주는 인자들을 살펴보았고, 만성 특발성 두드러기에서 자가면역 두드러기의 진단을 위한 ASST의 임상적 의의를 알아보고자 하였다. 방 법 : 6주 이상 두드러기가 지속되었던 만성두드러기 환아 39명의 검사 결과와 임상 양상을 후향적으로 조사하였다. 만성 두드러기의 완해율에 영향을 미치는 임상적인 요인을 고찰하고, ASST의 양성군과 음성군의 임상 양상을 비교하였다. 결과 : 1) 대상 환아 39명 중 남아는 56.4%(22/39명), 여아는 43.6%(17/39명)이었다. 외래 방문시나이는 12개월에서 13년 9개월의 분포를 보였고, 방문 전 증상의 지속 기간은 6주에서 6개월로 평균 9주였다. 2) 발병 원인은 물리 두드러기가 35.9%(14/39명)(한랭 두드러기 17.9%(7/39명), 증상적 피부묘기증 12.8%(5/39명), 콜린성 두드러기 5.1%(2/39명)), 감염성 원인으로 부비동염 5.1%(2/39명), 그리고 원인을 알 수 없었던 경우가 59.5%(23/39명)이었으며, 이 중 34.8%(8/23명)는 자가혈청 피부반응시험(autologous serum skin test, ASST) 양성으로 자가면역 두드러기가 의심되었다. 3) 만성 두드러기의 완해율은 56.4%(22/39명), 비완해율은 43.6%(17/39명)이었다. 원인이 밝혀진 비율은 완해군이 31.8%(7/22명), 비완해군이 52.9%(9/17명)로써 비완해군이 높았으나, 통계학적인 유의성은 없었으며, 성비, 연령도 두 군간에 유의한 차이는 없었다. 4) ASST 양성군과 ASST 음성군을 비교하였을 때 두 군간에 성비, 연령, 치료 기간의 유의한 차이는 없었다. 완해율은 ASST 음성군이 66.7%(10/1s명), ASST 양성군이 50.0%(5/1O명)로써 ASST 음성군의 완해율이 높았으나 통계적인 유의성은 없었다. 결 론 : 결론적으로 소아에서 만성 두드러기의 원인으로는 물리적 인자가 가장 많았으며, 만성특발성 두드러기 중 34.8%에서 ASST가 양성으로 자가면역 두드러기가 의심되었다. 그러나ASST 양성이 두드러기의 경과에 영향을 미치지는 않았으며 이의 의의에 대해서는 향후 재평가 되어야 하겠다.

Results : Male occupied about 56.4% of patients (n=22). Age at first outpatient clinic visit was from 12 month to 13 years 9 month. Duration of symptoms before visit were from 6 weeks to 6 months. Among the etiologic causes, 35.9% (n=14) of patients had physical urticaria, concretely, 17.9% (n=7) of patients had cold urticaria, 12.8% (n=5) of patients had symptomatic dermographism, 5.1% (n=2) of patients had cholinergic urticaria. Sinusitis was associated with CU in 5.1% (n=2) of patients. Unknown causes were 59.9% (n=23) of patients, and among them, 34.8% (n=8) of patients had positive ASST. Disease remission was observed in 56.4% (n=22), but non-remission was observed in 43.6% (n=17). Remission of CU was not associated with age, gender, etiology, and ASST. The remission rate of ASST negative group was higher than the ASST positive group, but it was not significant statistically. Conclusion : Etiologies of CU were variable and physical urticaria should be evaluated first.
