본 연구는 노인의 사회경제적 특징에 따른 활동제한과 건강행태의 차이를 실증분석함으로써 노인의 활동제한 문제를 개선하기 위한 방안 마련에 기초가 되는 정보를 제공하고자 한다. 이를 위해 2005년 서울시 시민보건지표조사 자료를 활용하여 일상생활에서 시력, 청력, 말하기, 활동 등에 1가지 이상 어려움을 갖고 있는 노인을 활동제한 노인으로 정의하고, 사회경제적 수준에 따른 활동제한 인구분포의 차이를 확인하였다. 또한 활동제한 노인을 대상으로 이들의 건강행태가 사회경제계층별로 다른지를 분석하였다. 다중로지스틱회귀분석을 실시한 결과, 가장 높은 가구소득집단에 비해 가장 낮은 가구소득집단의 활동제한 및 중증의 활동제한 오즈비는 각각 2.95와 4.62로, 가장 낮은 교육집단의 활동제한 및 중증의 활동제한 오즈비는 가장 높은 교육집단에 비해 각각 2.30과 4.90으로 나타나, 65세 이상 인구 중 활동제한 노인의 분포는 교육 및 소득집단별로 유의하게 다른 것으로 확인되었다. 또한 활동제한을 경험하고 있는 노인의 건강행태(규칙적 운동, 건강검진, 건강노력)는 연령, 성별, 가족유무, 활동제한의 중증도, 다른 사회경제 요인을 보정하고도 교육수준이 낮은 집단에서 불리한 것으로 나타났다. 노인의 활동제한은 미래의 보건의료 및 장기요양과 같은 사회복지서비스의 수요와 밀접한 관련이 있으므로 노인의 사회경제 계층간 활동제한과 건강행태 차이를 개선하는 노력이 필요하다. 이러한 노력에는 요양서비스 등의 지원 외에도 사회경제적 차이를 효과적으로 줄일 수 있도록 취약한 사회경제집단에 유효할 수 있는 교육 및 중재프로그램을 개발하는 데에 집중이 요구되는 것으로 판단된다.

This research investigated the socioeconomic differentials in activity limitations and health behaviors among the elderly Seoul residents, using the 2005 Seoul Citizens` Health Interview Survey Data. About 10% of study subjects(1,467) reported to have at least one or more troubles in seeing, hearing, speaking or other functions of daily life. Socioeconomic status of individuals were proxi-measured by family income, educational attainments, and job presence. Results from the multivariate logistic regression analyses showed that elderly populations of the lowest family income category were about 3 and 4.6 times more likely to suffer from activity limitations and severe activity limitations, respectively, compared to their high family income counterparts. Disadvantages in activity and severe limitations among the elderly with low educational attainments were also notably high (odds ratios: 2.30 and 4.90, respectively). Further, socioeconomic disadvantage significantly lowered the odds of general health check-ups and physical exercises among the functionally limited elderly Seoul residents. Based on these findings, we suggest that more academic and policy attention should be paid to the socioeconomically disadvantaged elderly urban residents in Korea.
