배경 및 목적: 염증성장질환에서 면역억제제로 분류되는 azathioprine (AZA)/6-mercaptopurine (6-MP)이 관해 유도와 유지에 효과적임이 알려진 이후 그 사용 빈도가 늘어나고 있다. 그러나 AZA/6-MP의 가장 중요한 이상반응인 골수기능억제의 빈도는 외국에 비하여 우리나라에서는 매우 높다. 한편, thiopurine S-methyltransferase (TPMT)과 inosine triphosphatase (ITPA)의 유전자 다형성과 TPMT 효소활성도가 AZA/6-MP의 대사에 관여하여 골수기능억제와 연관되어 있다는 보고가 있다. 따라서, 본 연구에서는 한국인 염증성장질환 환자에서 AZA/6-MP 사용에 따른 골수기능억제와 TPMT/ITPA 유전자 다형성 및 TPMT 효소활성도 사이에 상관관계가 있는지를 규명하여 안전한 약물 사용 방안을 제시고자 하였다. 대상 및 방법. 본 연구는 AZA/6-MP 치료후 6개월 이상 경과 관찰한 염증성장질환 환자 286명을 대상으로 한 후향적 연구이다. AZA/6-MP 치료후 백혈구수 $3,000/mm^3$ 이하의 골수기능억제가 나타난 환자를 이상반응군으로 그리고 이상반응이 나타나지 않은 환자를 대조환자군으로 정의하였다. PCR과 denatured high performance liquid chromatography로 TPMT 유전자 돌연변이 검색과 TPMT 전체 염기서열분석을 하였고 ITPA 유전자 돌연변이 검색도 시행하였다. Liquid chromatography with tandem mass spectrometry로 TPMT 효소활성도를 측정하였다. 결과: 총 286예의 환자 중 크론병 228예(79.7%), 궤양성대장염 34예(11.9%), 및 베체트장염 21예(8.4%)가 있었고, 성별은 남자 187명(65.4%), 여자 99명(34.6%)이었다. 평균 나이는 $25.7{\pm}9.3$세 (범위 9-63)이었고 AZA와 6-MP의 사용빈도는 각각 278예(97.2%)와 8예(2.8%)였다. 이상반응군은 총 118예(41.3%)였다. 이상반응군과 대조환자군 간에 나이, 성별, 염증성장질환 가족력, 염증성장질환의 종류, Azathioprine/6-MP 적응증, 수술력에서는 유의한 차이가 없었고, 체중이 적고, 신장이 적을수록 골수 기능 억제의 빈도가 유의하게 높았다. TPMT*1/3C는 7예(2.4%)에서 관찰되었고, ITPA 94 C>A는 66예(23.1%)가 있었는데 이형접합체 63예(22.0%)와 동형접합체 3예(1.0%)가 있었다. TPMT 효소활성도는 평균 $13.4{\pm}12.1U/mL$ (범위 2.1-76.2)이었다. TPMT 유전자 다형성 및 ITPA 유전자 다형성과 골수기능억제와는 상관관계가 없었고, TPMT*1/3C가 나타난 환자의 TPMT 효소활성도는 야생형의 효소활성도와 유의한 차이가 없었다. 이상반응군과 대조환자군 사이에 TPMT 효소활성도는 유의한 차이가 없었다. 각종 임상적인 변수들 중 통계적으로 유의한 변수들만 모아서 시행한 다변량 분석에서 AZA/6-MP 사용기간이 길수록 그리고 최대관용용량에 이르는 기간이 짧을수록 골수억제의 빈도가 높았다(p=0.01). 그러나, TPMT 유전자 돌연변이 및 효소활성도와 AZA/6-MP에 의한 골수기능억제 사이에는 유의한 상관관계가 없어 TPMT 유전자 돌연변이 및 효소활성도에 근거하여 AZA/6-MP 이상반응을 예측할 수는 없었다. 결론: 한국인 염증성장질환 환자에서 AZA/6-MP 치료에 따른 골수기능억제 빈도는 매우 높은 반면 TPMT와 ITPA 돌연변이의 빈도는 동북아시아의 보고와 유사하였으며, TPMT/ITPA 유전자 다형성과 TPMT 효소활성도 및 골수억제 사이에는 상관관계가 없었고 골수억제를 예견할 수 있는 임상적 위험인자도 찾을 수 없었다. 그러므로, Az4/6-MP 치료시에는 골수기능억제에 유의하면서 규칙적인 혈액검사와 함께 적은 용량부터 조심스럽게 증량하면서 사용하는 것을 권한다.

Background and Aim: Azathioprine (AZA)/6-mercaptopurine (6-MP) is an Immunosuppresive drug which is effective in the induction and the maintenance of remission in inflammatory bowel disease. However, If has been reported that genetic polymorphism of thiopurine 5-methyltransferase (TPMT) and inosine triphosphatase (ITPA) and/or TPMT activity are associated with bone marrow suppression. In particular, the frequency of bone marrow suppression which is one of the most important adverse effects of the AZA/6-MP treatment is much higher in Korean population than that of Western population. The aim of this study was to investigate the association between TPMT/ITPA polymorphism and/or TPMT activity and bone marrow suppression during AZA/6-MP treatment in patients with inflammatory bowel disease in Korea and then to suggest a guideline for safe drug usage, Patients and Methods: A retrospective review of the medical records for patients with inflammatory bowel disease who were treated with AZA/6-MP was conducted for at least 6 months. The Adverse reaction group was defined as the patients with leukopenia less than $3,000/mm^3$ after AZA/6-MP treatment and the control group was defined as the patients without leukopenia after AZA/6-MP treatment. Through polymerase chain reaction and denaturing high performance liquid chromatography, TPMT genetic polymorphism (*1, *2, *3A, *3B and *3C) was detectedand TPMT full sequencing was performed. Moreover, ITPA genetic polymophism (94 C>A and IVS2+21 A>C) was investigated. TPMT activity was measured using liquid chromatography with mass spectrometry/mass spectrometry. Results: A total of 286 patients were studied, including 228 cases of Crohn's disease (79.7%), 34 cases of ulcerative colitis (11.9%), and 21 cases of intestinal Behcet's disease (8.4%). Male to female ratio was 1.9:1. Mean age of the patients was $25.7{\pm}9.3$ year-old (range 9-63). Two hundred seventy eight patients (97.2%) were treated with AZA and 8 patients (2.8%) were treated with 6-MP. Adverse reaction occurred in 118 cases (41.3%). TPMT*1/3C was detected in 7 cases (2.4%), ITPA 94 C>A in 66 cases (23.1%) including heterozygote 63 cases (22.0%), and homozygote 3 cases (1.0%). Mean TPMT activity was measured at $13.4{\pm}12.1U/mL$ (range 2.1-76.2). There was no association between TPMT polymorphism/ITPA polymorphism and bone marrow suppression. The TPMT activity with TPM*1/3C was not decreased more than that of wild type. There was no statistically significant difference between adverse reaction group and control group in terms of TPMT activity. Multivariate analysis showed a longer use of AZA/6-MP and short time to maximumtolerated doses were associated with high frequency of bone marrow suppression (p=0.01). However, Bone marrow suppression induced by AZA/6-MP was not significantly associated with TPMT genetic polymorphism and TPMT activity. Conclusions: The frequency of bone marrow suppression during AZA/6-MP treatment in Korean patients with inflammatory bowel disease was much higher than that of Westerners whereas the frequency of the TPMT and ITPA genetic polymorphism was similar to that of far East Asia. There was no association between TPMT/ITPA genetic polymorphism and/or TPMT activity and bone marrow suppression, and we could not find any clinical risk factors to predict the bone marrow suppression. Moreover, adverse reaction of AZA/6-MP was not predictable based on TPMT genetic polymorphism and TPMT activity. In conclusion, we recommend that AZA/6-MP should be slowly increased from the low dose to the maintenance dose with the regular blood test to scrutinize bone marrow suppression.
