약물대사에 관여하는 유전자변이가 대사성의약품인 경구용혈당강하제와 혈소판응집억제제의 약동학적 특징에 미치는 영향을 규명하고, 한국인에서 약물대사효소의 유전자변이에 따른 약동학적 파라메타에 대한 정보를 제공하고자 하였다.

경구용혈당강하제인 glipizide는 CYP2C9, gliclazide는 CYP2C9과 CYP2C19에 의해 대사되며, 혈소판응집억제제인 cilostazol은 CYP2C19, CYP2D6와 CYP3A4/5에 의해 대사되는 것으로 알려져 있다. 따라서 본 연구에서는 약물대사효소의 유전적 다형성이 glipizide, gliclazide와 cilostazol의 약동학적 지표에 미치는 영향을 관찰하였다.
한국인 1146명을 대상으로 실시한 CYP2C9의 유전형 분석에서 CYP2C9*2는 확인되지 않았으며, CYP2C9*1, CYP2C9*3와 CYP2C8*13 allele의 빈도가 각각 0.948, 0.048와 0.004로 나타났다. 한국인 1157명을 대상으로 실시한 유전형 분석에서 CYP2C19*1, CYP2C19*2와 CYP2C19*3 allele의 빈도가 각각 0.609, 0.277와 0.114로 나타났다. 한국인 1151명을 대상으로 실시한 유전형 분석에서 CYP2D6*1, CYP2D6*2, CYP2D6*5, CYP2D6*10과 CYP2D6*1$\times$N$(*2{\times}N)$ allele의 빈도가 각각 0.337, 0.141, 0.033, 0.484와 0.050로 나타났다. 한국인 637명을 대상으로 실시한 유전형 분석에서 CYP3A5*6는 확인되지 않았으며, CYP3A5*1과 CYP3A5*3 allele의 빈도가 각각 0.217과 0.783으로 나타났다.
Glipizide의 경우 CYP2C9의 유전형에 따라, gliclazide의 경우 CYP2C9과 CYP2C19의 유전형에 따라, cilostazol의 경우 CYP2C19, CYP2D6와 CYP3A5의 유전형에 따라 각각 24명, 23명, 33명의 피험자로 시험을 수행하였다. Gliclazide의 경우 CYP2C19과 CYP2C9의 유전자 변이에 의하여 반감기, 청소율, AUC등이 유의성 있게 변하였으며, glipizide의 경우 CYP2C9의 유전자 변이에 따라 청소율과 AUC가 유의성 있게 변화하였다. Cilostazol은 CYP2C19, CYP2D6와 CYP3A5등의 유전자 변이에 대하여 유의할 만한 약동학적 파라메타의 변화는 없었다.
본 연구에서 유전자변이가 약물의 생체이용률에 미치는 영향을 확인함으로써, 차후 이러한 약물을 사용하기전 유전자변이 검사를 통하여 약용량을 조절하기 위한 정보를 얻었으며, 이를 통하여 약물사용의 안전성과 유효성을 증대 시킬 수 있을 것이다. 한국인의 유전형 분포에 대한 정보를 제공하고, 외국인과의 차이를 비교분석함으로써, 의약품의 평가에 필요한 자료를 제공하고, 한국인에서 유전적 다형성에 따른 약물반응 예측자료로 활용할 수 있을 것이다.

The efficacy and toxicity of many drugs may be significantly affected by the genetic polymorphisms of drug metabolizing enzymes. Glipizide and gliclazide, a second-generation sulfonylurea, are potent hypoglycemic agent. Sulfonylurea hypoglycemics are mainly metabolized by CYP2C9. Cilostazol is widely used as an antiplatelet vasodilator agent. It undergoes extensive hepatic metabolism. In vitro results suggest the CYP3A, 2C19, 2D6 and 1A2 may be involved in the metabolism of cilostazol.
In first study, we investigated the effects of genetic polymorphisms of CYP2C9 on the PK of glipizide. A 5 mg oral dose of glipizide was given to 24 Korean volunteers with different CYP2C9 genotypes. The plasma concentrations of glipizide were analyzed by HPLC-UV until 15 hr after drug administration. In subjects heterozygous for the CYP2C9*3 and CYP2C9*13 allele, $AUC_{inf}$ of glipizide were greater, the oral clearance were lower than those in homozygous CYP2C9*1 subjects.
In second study, we investigated the effects of genetic polymorphisms of CYP2C9 and CYP2C19 on the PK of gliclazide. A 80 mg oral dose of gliclazide was given to 27 Korean volunteers with different CYP2C9 and CYP2C19 genotypes. The plasma concentrations of gliclazide analyzed by HPLC-UV until 48 hr after drug administration. The pharmacodynamic effects of gliclazide were determined by measuring the amount of glucose and insulin in serum. Pharmacokinetic parameters of gliclazide were significantly different between CYP2C19 EM and PM genotype subjects.
In conclusion, genetic polymorphisms of CYP2C9 markedly affect the pharmacokinetics of glipizide. The pharmacokinetics of gliclazide are mainly affected by CYP2C19 genetic polymorphism, although CYP2C9 may also be affected.
In third study, the effects of major polymorphism of the CYP2C19, CYP3A5 and CYP2D6 on pharmacokinetics of cilostazol were studied in healthy volunteers. A 100 mg oral dose of cilostazol was given to 33 Korean volunteers with different genotype. Cilostazol was analyzed by HPLC-UV in plasma samples collected up to 48 hours after drug intake. The homozygous variant subjects (PM/PM; CYP2C19/CYP3A5) had a lower oral clearance of cilostazol than the homozygous wild-type subjects (EM/EM; CYP2C19/CYP3A5). No significant differences in $t_{max}$, $C_{max}$, $t_{1/2}$, CL/F and $AUC_{0-\infty}$ were observed in different CYP2C19, CYP2D6 and CYP3A5 genotype groups. In conclusion, the CYP2C19, CYP3A5 and CYP2D6 polymorphisms are a minor factor influencing the pharmacokinetics of cilostazol.
