Consumers are not allowed to choose ethical medicine and drugs of their own. In reality, however, there are some medical supplies available to consumers in bottles or other containers as they come out of the factory. Moreover, many ethical drugs have the following labeling problems: (i) too small print size, (ii) use of difficult-to-understand terms, (iii) expert-oriented label text, (iv) unnecessary, too long or repeated expressions, (v) plain visual layout, and (vi) little use of Braille. Therefore, medicine/drug labeling needs to be improved to contain more relevant and accurate information and use easy-to-understand terms for the safe use of such medicines or drugs. We conducted a survey of 250 consumers in which their degree of understanding (or readability) of 348 difficult medical terms which are used in medicine or drug labeling was measured on a four-point scale. In this survey, their average score was 1.5 points per term. We sought advice from experts on 147 terms lower in readability and developed their alternative terms. The same 250 respondents were surveyed again, and the difficult-to-understand terms were compared to their alternatives in terms of the degree to which they can read them. For those alternatives, the respondents scored an average of 3.1 points per term on a four-point scale, reporting an improvement of 2.6 points from the previous average of 1.5 points. Thirty six terms showed differences of two or more points from their alternatives in readability. This suggests that the alternation of terms can contribute to enhancing consumer readability. In an investigation of consumer satisfaction with medicine or drug labeling information, 52.8 percent (241 respondents) of surveyed consumers answered "dissatisfied" or "very dissatisfied." When those 241 respondents were asked to provide reasons for dissatisfaction, 44.4 percent answered that it was difficult to understand technical words/terms or Chinese characters, while 35 percent of the respondents answered "Too much information makes it hard to know what is important." In conclusion, the improvement of terms used for medicine or drug labeling is more important for increasing consumer satisfaction and ensuring the safe use of medicines and drugs based on accurate information. However, for the effective use of alternative terms, it is necessary to standardize such terms and develop a list of standard terms by collecting data from individual institutes. In addition, since the attempt to improve terminology aims to enhance consumer readability and prevent medicine/drug misuse or abuse, a consumer readability test has to be conducted in parallel with such an attempt.
