본 연구과제의 최종연구목표는
1) 도파민 신경세포주와 파킨슨병 동물모델을 이용하여 의약품중 임상에서 다빈도로 사용되는 신경-정신계 의약품이 도파민성 신경퇴행의 악화를 유발하는지 평가하고 2) 도파민 신경세포사를 촉진하는 생화학 및 분자생물학적 기전을 규명함으로써, 의약품의 퇴행성뇌질환에 미치는 독성에 대한 평가 자료를 확보하여 의약품안전성평가 연구에 기여하고 의약품 안전성정보 관리 정책지원을 위한 기초자료로 활용하기 위함.
이를 위해, 사전문헌조사 및 주관부서와의 연구착수 검토과정을 거쳐 선정한 7가지 주요 신경-정신계
본 연구과제의 최종연구목표는
1) 도파민 신경세포주와 파킨슨병 동물모델을 이용하여 의약품중 임상에서 다빈도로 사용되는 신경-정신계 의약품이 도파민성 신경퇴행의 악화를 유발하는지 평가하고 2) 도파민 신경세포사를 촉진하는 생화학 및 분자생물학적 기전을 규명함으로써, 의약품의 퇴행성뇌질환에 미치는 독성에 대한 평가 자료를 확보하여 의약품안전성평가 연구에 기여하고 의약품 안전성정보 관리 정책지원을 위한 기초자료로 활용하기 위함.
이를 위해, 사전문헌조사 및 주관부서와의 연구착수 검토과정을 거쳐 선정한 7가지 주요 신경-정신계 약물(Amitriptyline, Bupropion, Gabapentin, Fluoxetine, Haloperidol, Propofol, Risperidone)이 도파민성 신경세포(SH-SY5Y cell) 생존에 미치는 영향과 분자생물학적 메커니즘을 세포모델융 통해 평가하고(연구내용 1), 파킨슨병 동물모델에 최종 선정된 의약품융 투여하여 도파민 신경세포 퇴행의 악화 및 보호에 대한 영향을 평가하고 기전을 규명하는 연구(연구내용2)를 수행하였음. 대표적인 연구방법론으로는, 세포사멸 측정(MTT assay), 산화적스트레스에 미치는 영향 평가 (DHE를 이용한 FACS analysis 및 nitrotyrosine immunoblot), 세포사 신호전달 및 ER stress에 미치는 영향을 평가 (JNK, PERK, caspase-3 활성, Grp78, CHOP 발현 및 cytochrome c release)하였으며 (연구내용 1), 동물실험에서 세포사를 유발하는 fluoxetine과 haloperidol 단독 투여 또는 낮은 용량의 MPTP와 병행투여 시, 행동영향평가 (Computerized rota-rod test), 흑질에서의 tyrosine hydroxylase 및 ER stress protein (PERK, Grp78 immunoblot) 발현 측정, 선조체에서의 dopamine과 대사체 측정 (LC-MS/MS) 및 GFAP 발현을 측정하였음 (연구내용 2).
평가한 약물 중에서 bupropion (> 100 $\mu$g/mL), fluoxetine (> 1 $\mu$g/mL), haloperidol (>0.1 $\mu$M) 이 세포사를 유발 하였으며, 산화적스트레스에 대한 영향은 3가지 약물 모두 미약하였으나 bupropion의 경우 고농도에서 nitrotyrosine 생성이 다소 증가함. Bupropion은 ER stress response 중에서 PERK 활성 (eIF2$\alpha$ 인산화), JNK 활성을 증가시켰으며 Grp78 발현도 다소 증가함. Haloperid이의 경우에는 JNK 활성을 증가시킴. 세포사와 연관된 p38-MAPK의 활성은 bupropion만이 활성화시켰으며, JNK 억제제에 의해 bupropion에 의한 세포사가 다소 억제됨. Miochondrial apoptotic pathway에서는 세가지 약물 모두 cytochrome c를 유리시켰으며, caspase-3 활성은 bupropion과 haloperid이에 의해 증가함. 동물실험에는 임상적으로 사용되는 용량범위 (농도)에서 세포사를 유발할 수 있을 것으로 최종 판단된 fluoxetine과 haloperid이을 사용함.
동물실험에서는 C57/B16 mice에 fluoxetine과 haloperidol이 함유된 osmotic mini-pump를 삽입하여 3주간 투여 후, 흑질과 선조체를 채취하여 이용함. 약물과 환경적 영향과의 상관성을 보기 위해서는 MPTP를 낮은 용량 (총 20 mg/kg)을 마지막 1주 전에 투여하여 3주째 평가함.
행동약리 실험결과 haloperidol (0.25mg/kg/day)만이 운동능력 저하가 1-2주에 의미있게 나타났으며, fluoxetine(7mg/kg/day)도 다소 감소하는 영향을 보였으나 통계적 의의는 없었음. 3주에서는 두 약물 모두에서 의미있는 약물의 효과가 관찰되지 않음. 이러한 결과는 아마도 약물자체의 약리효과 의한 초기의 운동능력 저하에 의한 영향이라고 판단됨. 이러한 유추는 3주 후 흑질과 선조체에서의 생화학적 검사결과에 의해 뒷받침됨. 즉, 선조체에서의 도파민 및 대사체의 함량이 약물 자체에 의해 의미있는 변화가 없었으며, GFAP의 발현도 큰 차이를 보이지 않음으로써 약물 자체의 퇴행성 독성유발은 없는 것으로 판단됨. 다만, 낮은 용량의 MPTP와 약물이 병용투여 되었을 경우, haloperidol + MPTP 군에서 도파민의 농도가 크게 감소하였으며, 반면 도파민 대사체인 DOPAC과 HVA는 상대적으로 증가하여 dopamine turn-over rate가 크게 증가함. 또한 흑질에서 PERK 활성 및 Grp78발현이 haloperidol 투여군에서 감소하였고 fluoxetine에 의해서는 증가함. 따라서 haloperidol이 ER stress에 대한 세포의 방어 메커니즘(unfolded protein response)을 약화시키는 것으로 판단됨.
연구결과를 요약하면, 본 연구에서 평가한 약물 중에서 haloperid이은 도파민 신경세포에 JNK 및 ER stress 경로 등을 통해 악영향을 미칠 수 있으며, 외부의 해로운 환경적 요인과 상승작용을 나타낼 수 있는 약물로 판단되었음. Fluoxetine의 경우에는 세포실험에서는 세포사를 유발하였으나, 실제 생체에서는 다양한 보호작용 메커니즘이 존재하여 이를 극복할 수 있는 것으로 판단됨. Bupropion의 경우에는 세포사 유발과 ER stress의 유발, caspase-3 활성화 등 다양한 메커니즘에 의해 세포사가 유도되었으나, 비교적 임상적 농도보다 높은 농도에서 유발되었으므로 추가적인 연구가 필요하다고 판단됨. 본 연구에서 사용한 모델을 추후에 독성유전체 연구 등에 이용하여 약물의 신경독성에 대한 유용한 정보를 얻을 수 있으리라 사료됨.
결론적으로, haloperid이 또는 bupropion과 같은 도파민 수용체 및 도파민 대사, 취입에 영향을 주는 약리 메커니즘을 갖는 약물의 경우 도파민 신경세포에 대한 영향평가에 대한 전임상 및 임상 자료에 대한 면밀한 검토가 의약품평가과정에서 이루어져야 함.

The aims of the study are 1) evaluating the effects of neuropsychiatric drugs on dopaminergic neurodegeneration using dopaminergic cell line and animal models, and 2) investigating the biochemical or molecular biologic mechanisms of the dopaminergic neurotoxicities. Finally, we hope that this study can contribute to conduct safety evaluation of drugs affecting dopminergic system and to progression of related research area.
We finally selected 7 major neuropsychiatric drugs (amitriptyline, bupropion, gabapentin, fluoxetine, haloperidol, propofol and risperidone). Using SH-SYSY catecholaminergic cell line, we evaluated the effects of these drugs on cell viability and toxic mechanisms (study 1), and evaluated the dopaminergic neurodegenerative effects of bupropion, fluoxetine and haloperidol and toxic mechanisms in animal model (study 2). We evaluated cell viability (MIT assay) and investigated effects of the drugs on oxidative stress (DHE fluoresence and nitrotyrosine expression), apoptotic signals and ER stress responses in cellular experiments. In vivo, we treated CS71B16 mice with fluoxetine and haloperidol for 3 weeks with or without low dose of MPTP (20 mglkg) using osmotic mini-pump. We evaluated the pharmacobehavior effects of drugs every week by computerized rota-rod test. After 3 weeks, we measured nigral expression of tyrosine hydroxylase and ER stress-related proteins, striatal dopamine, DOPAC and HVA, and striatal GFAP expression (study 2).
Our results showed that bupropion (>100 $\mu$g/mL), fluoxetine (>1 $\mu$g/mL) and haloperidol (>0.1 $\mu$M) decreased cell viability in SH-SYSY cells. The expression of nitrotyrosine, ER stress response proteins (p-JNK, Grp78, p-eIF2$\alpha$), phosphorylated p-38 MAPK, cleaved caspase-3 were increased by high concentration of bupropion. Haloperidol increased JNK activity, cleaved caspase-3 and cytochrome c release. Fluoxetine slightly increased cytochrome c release.
In computerized rota-rod test, haloperidol (O.25mg/kg/day), but not fluoxetine (7mg/kg/day) significantly decreased motor activity in first and second week. However, animals treated with these drugs had no significant impairment of motor function in last week. These results suggested that the early impairment of motor function may be rather caused by the pharmacologic effects of the drugs than dopaminergic degeneration. After 3 weeks, we collected substantia nigra and striatum. The expression level of nigral TH and striatal GF AP were not significantly different between control group and drug-treated group. The striatal levels of dopamine and its metabolites also were not significantly different between control and drug-treated group.
Interestingly, when animals treated with haloperidol and low dose of MPTP, striatal dopamine was significantly decreased, and turn-over rate of dopamine ([DOPAC+HVA]/[DAD]) was dramatically increased. In addition, haloperidol decreased the compensatory expression of phosphorylated eIF2$\alpha$. in substantia nigra. These results suggested that haloperidol decreased protective power against external noxious stimuli in substantia nigra. In summary, haloperidol may have a harmful effects on dopaminergic cells through the JNK activation and ER stress pathway. Particularly, when haloperidol is combined with external noxious stimuli, these deleterious effects may be obvious. Fluoxetine showed the decrease of cell viability, however, in vivo compensatory mechanism may protect against the cytotoxicity. Interestingly, bupropion showed cytotoxicity via activation of ER stress, p38-MAPK, cytochrome c release, and caspase activation, although these effects were observed at relatively high concentration. The effects of bupropion with clinical concentration should be further evaluated in near future. Under models used in this study, future toxicogenomic study, for example, may give further valuable informations about neurotoxicity of drugs.
In conclusion, preclinical and clinical data of drugs affecting on the dopamine receptor, metabolism and re-uptake, such as haloperidol or bupropion should be closely investigated in drug safety evaluation process.
