○ 혈액수요량 추계에 가장 중요한 영향요인은 암 질환과 노령화 인구 증가였다. 그러나 인구변화 감소는 큰 영향을 주지 못하였다.
○ 적혈구제제 (전혈포함) 기준 대한적십자사의 공급량은 2003년 162만건에서 2007년 163만 건으로 2003년 대비 0.6%가 증가한 반면, 건강 보험심사평가원 적혈구제제 청구량 기준으로 2003년 125만 건에서 2007년 153만 건으로 2003년 대비 22.0% 증가하였다.
○ 적혈구제제의 사용량은 60-69세 연령 구간에서 24.2%로 가장 많이 사용하였으며, 70-79세 연령 구간 (19.3%), 50-59세 연령 구간 (17.0%), 40-49세 연령 구간 (15.0%) 순이었다. 2007년도에는 60세 이상에서 전체 적혈구제제의 54%를 사용하였다.
○ 2007년 의료기관 종별 병상수 대비 적혈구제제 (전혈포함) 사용량은 2003년과 비교하여 종합전문병원은 거의 일정하게 유지되어 병상수 대비 19.2 단위를 사용하였으나, 종합병원과 병원에서의 병상수 대비 혈액사용량은 5.7 단위에서 7.0 단위, 병원은 0.8 단위에서 1.7 단위로 증가하였다.
○ 2030년까지 전혈 및 적혈구제제 사용량 추계는 2010년대 1.5% 전후, 2017년 2.0%, 2027년에 3% 이상의 꾸준한 수요증가가 예상되었으며, 2010년 174만 단위, 2020년 272만 단위, 2030년 421만 단위의 적혈구 제제의 수요량이 있을 것으로 추산되었다.
○ 2030년까지 혈소판제제의 예상 수요량은 2010년 168만 단위, 2020년 221만 단위, 2030년 278만 단위로 추산되었다.
○ 2007년 국내 헌혈자 수는 2,087,762명이었으며 인구대비 헌혈율은 4.3%였다. 이는 2002년도 헌혈율 5.3%에서 조금씩 떨어지기 시작하여 2006년도 4.7%에서 0.3% 더 떨어진 수치이다.

○ 2007년도 미국의 헌혈율은 5.4%, 영국은 2.4%, 일본은 3.9%로 조사되었다. 각 국가의 사회적 요인이 있겠으나 우리나라의 헌혈율을 높이는 노력도 필요하겠으나 지금의 4% 대를 유지하는 노력이 더 필요할수 있다.
○ 외국의 각 나라도 혈액수요량은 매년 1-3%의 수요증가를 예측하고 있다. 그러나 우리나라의 수혈 수요의 증가는 의료 선진국 진입, 노령 인구 증가 등을 고려할 때 Global Strategic Analysts 사가 2008년에 발간한 ‘Blood Banking and Blood Products, A Global Strategic Business Report’에서 보고한 아시아 지역의 혈액은행 시장의 성장률 5.37%보다는 낮지만 유럽, 미국, 일본 등의 우리가 비교하는 선진국의 수치보다는 조금 높은 수치이다.
○ 인구 천 명당 적혈구제제 사용량은 한국 31.69단위, 영국 30.77단위, 미국 47.90단위, 캐나다 24.32단위, 일본 27.47단위, 호주 37.51단위로 조사되었다. 우리나라에서의 적혈구제제 사용량은 외국에 비해 높은편은 아니었으나 일본이나 캐나다에 비해서는 그 사용량을 더 줄이기 위한 노력이 필요할 수 있다.
이상의 결과로 현재의 사회적 변화가 지속되는 한 혈액수요량은 매년 3-4% 꾸준히 증가할 것이고 헌혈율은 3%대 수준으로 감소할 것이 예상된다. 따라서 국가는 더 이상 안정적 혈액확보 정책을 미룰 수 없으며, 국가의 혈액자급원칙에 대한 강력한 의지를 갖고 지금부터 다음과 같은 정책개발과 함께 실제로 이것이 시행될 수 있도록 강력한 법적 뒷받침과 지속적인 지원의 자세가 필요하다.

In our country, the growth in blood products availability is higher than many of the foreign countries, though not as high as the 5.37% growth rate reported for Asian countries as cited by the Global Strategic Analysis in 2008's "Blood Banking and Blood Products, A global Strategic Business Report."
○ For each 1000 persons, the red blood cell utilization is 31.69 units in Korea, 30.77 units in England, 47.9 units in the United States, 24.32 units in Canada, 27.47 units in Japan, 37.51 units in Australia. Compared to the other listed countries, the use of red blood cells did not exceed most. But the utilization rate does exceed Canada and Japan, pointing to the fact that utilization could be further limited with increased efforts.
From these results, the demand for blood will steadily increase by 3-4% and the number of donors will decrease by 3%. Therefore, the nation must not continue to enforce some of our current policies and must make a concerted effort to encourage direct measures to overcome the exigent blood shortage crisis by the following: ① a policy for guaranteeing donor safety ② institution of appropriate use of blood supplies in the medical setting ③ measures to decrease the rate of discarded blood products and maximizing the procurement of utilizable blood ④ efforts to assure the public of the donor and recipient safety and to relay the importance of blood donation ⑤ encouragement of research and development of blood transfusion related medications and supplies.
