첫째, 영유아 건강의 9가지 핵심영역을 도출하고 이를 분석틀로 하여 전문가 및 실무자 토론회, 양적인 지표분석 등을 통해 영유아건강요구를 분석하였고, 이와 함께 영유아건강관련 공공사업과 주요 민간사업을 분석하고, 영유아건강관련 기관이나 단체를 파악하고, 관련 인력을 조사함으로써 영유아의 건강역량을 함께 분석하여 우리나라 영유아의 건강요구를 진단안을 제시하였다.영유아 건강역량을 분석하기 위해 1) 영유아 건강 관련 공공사업 및 주요 민간사업을 분석하였고, 2) 영유아 건강 관련 주요 정부부처 조직및 관련 단체 및 기관을 분석하였으며, 3) 아동건강 관련 인력을 조사하였다.영유아건강관련 주요 사업들을 수행한 기관은 보건복지가족부 모자보건과와 건강증진과, 아동권리팀, 질병정책과, 정신건강정책과, 생활위생과, 건강정책과, 아동청소년 보호과, 교육과학기술부 및 산하 교육청, 여성가족부 건강가정지원센터, 질병관리본부, 국민건강보험공단, 인구보건복지협회, 한국보건사회연구원, 건강증진사업지원단, 서울특별시 건강증진담당관, 저출산대책담당관 및 인력운영과, 구청 건강가정지원센터, 한국청소년상담원, 한국지역사회교육협의회, 한국심리상담연구소, 위스타트 운동본부등의 정부부처 기관 및 지자체, 민간기관 등이었다.영유아건강관련 주요 공공 및 민간의 59개 사업이 성장발달검진, 성장발달 촉진, 예방접종, 안전사고예방, 아동학대 예방 및 관리, 보육시설 건강관리, 양육기술 향상 및 양육지원, 미숙아 및 선천성 이상아 관리, 천식 및 아토피, 난치성 질환, 비만, 정신건강, 영아급사증후군, 영양균형, 개인위생, 활동, 장애아 지원 및 관리, 취약가정 아동의 18개 분야로 분류되었다 개 분야는 건강한 . 18 성장발달, 안전한 환경, 건강지원적 사회문화,고위험관리 및 장애예방, 주요 건강문제, 건강한 생활습관, 취약가정, 건강한 출산, 생식건강의 9개 영유아건강의 핵심영역으로 분류되었다. 이와 같은 9개의 핵심영역을 분석틀로 하여 이후 영유아의 건강요구와 건강역량을 조사분석하였으며 영유아건강중재 프로그램의 기틀이 되었다.
아동건강 관련 정부부처 및 지자체에는 교육과학기술부 내 교육복지지원국 내 유아교육지원과와 특수교육지원과, 보건복지가족부 내 건강정책국의 모자보건과와 건강증진과, 아동청소년정책실의 아동청소년복지정책관과 보육정책관, 서울특별시청 내 복지국의 장애인복지과 및 장애인정책팀, 복지정책과 생활보장팀, 식품안전추진단, 여성가족정책과의 건강증진담당관 및 보육담당관, 저출산대책담당관, 보건정책담당관, 안전국의 안전지원과와 예방과, 여성부 내 권익증진국의 인권보호과, 보건의료정책관의 식품정책과 및 공공의료과 등이 포함되었다. 그 외 관련 단체로 한국국공립유치원교원연합회, 한국보육시설연합회, 한국보육교사회, 중앙보육정보센터, YWCA, 대한적십자사, UNICEF 한국위원회, 홀트아동복지회,동방사회복지회, 한국수양부모협회, 한국아동청소년그룹홈협의회, 한국아동단체협의회, 대한사회복지회, 세이프키즈코리아, 부스러기사랑나눔회, 굿네이버스, 육영재단, 한국장애인복지시설협회, 월드비젼, 해바라기아동센터, 대한병원협회, 대한의사협회, 대한소아청소년과개원의사회 러브앤키즈,소아과학회, 대한간호협회, 아동간호학회, 모자보건학회, 건강관리협회,EBS 등이 포함되었다.영유아건강 관련 인력은 공공부문과 민간부문으로 나누어 조사하였다. 공공부문은 보건소, 보건지소 의사와 간호사, 보건진료원이 포함되었고 민간부문은 소아청소년과 전문의와 가정의학과전문의, 신생아실간호사,아동병동이나 관련기관에서 일하는 아동간호사, 보육교사와 아이돌보미등을 영유아건강관련인력으로 조사하였다.영유아건강에 대한 심층적인 요구 사정을 하기 위해 질적인 접근방법을 사용하여 분석하였다. 이를 위해 영유아건강관련 실무자 토론회와 전문가 집단 토론회, 전문가 및 실무자 전화면담 등이 이루어졌다. 9개의핵심영역 중 안전한 성장환경 및 건강지원적 가정과 사회문화, 고위험관리 및 장애예방, 취약가정이 함께 접근될 수 있고 여기에 건강한 성장발달과 자기주도적 건강생활습관이 합쳐진 것이 하나의 영유아건강중재프로그램의 주요 영역으로 통합되었다.
핵심영역별 진단과정을 통해 우리나라 영유아 건강요구와 관련된 주요 문제는 아동건강 1) 자원에 대한 통합적인 정보제공이 되고 있지 않다는 것과 2) 영유아 건강평가 및 제공서비스가 아직 활성화되어 효과적으로 제공되고 있지 못하다는 것이다. 상당수의 영유아 건강요구가 통합적인 아동건강 자원에 대한 정보가 원활하게 공유되고, 포괄적인 영유아건강평가 및 상담이 활성화됨으로써 해소될 수 있는 내용으로 파악되었다.제시된 진단을 개념적 기틀로 하여 지역사회 중심의 영유아건강중재프로그램 “함께 키우는 건강한 아이”가 개발되었다. 이 프로그램의 대상은 대한민국에 거주하는 만 6세 이전까지의 모든 영유아와 그 가족을 대상으로 하고 있다. 이 프로그램의 주요 목적은 영유아의 건강한 성장발달을 증진하고 양육능력강화이다. 프로그램은 1) 영유아건강서비스 정보 통합프로그램인 아동건강네비게이터, 2) 아동건강평가 및 상담전문가 발굴안, 3) 아동건강상담전화, 4) 아동건강가정방문안, 5) 영유아건강감시체계안으로 구성되었다.아동건강네비게이터는 영유아의 건강요구별 건강서비스에 대해 서비스 개요 및 목적, 내용, 이용방법, 연락처, 시행되는 지역 등에 대한 정보를 담은 데이터베이스로 영유아의 연령 및 건강요구영역, 사회경제적 상황 및 장애유무 등의 조건에 따라 유용가능한 건강서비스를 추출해서 세부 사항을 확인할 수 있는 데이터베이스이다.영유아 건강평가 전문가란 실무에서 일차적으로 영유아의 성장발달 및 양육을 평가하는 전문인력을 말한다. 영유아 건강평가 전문가는 지정된 검진기관 및 지역사회 검진센터 등에서 의사 및 전문간호사와 연계하여 기관 내에서 또는 가정방문을 통해 영유아 및 그 부모에게 서비스를 제공하게 된다.영유아건강 적시 상담전화는 영유아건강 평가 및 상담전문가에 의해 24시간 365일 전화접촉이 가능한 핫라인으로서 영유아의 성장발달과 양육문제, 아동의 발달에 대한 염려 등에 대해 상담을 제공하고 이러한 상담은 아동건강관련 상담 Q&A나 상담매뉴얼을 바탕으로 이루어진다.
영유아건강 가정방문안은 기존의 맞춤형 방문건강관리의 내용에 양적, 질적인 개선을 제안한 것으로 기존 맞춤형 방문건강관리에서 아동관련서비스는 신생아와 18개월 이하 영유아의 건강평가와 모아상호작용을 확인하는 것으로 서비스가 이루어졌는데 이를 아동 성장발달 및 육아를 증진시키기 위한 포괄적인 서비스로 확대하였다. 특히 영유아 검진 중 사전지도 및 육아상담 등을 적극적으로 도입하는 것을 포함하고, 자율성과주도성의 발달로 훈육의 어려움과 바른 생활습관 형성 및 성장발달 촉진을 위한 조기개입이 강조되는 18개월 이상의 유아를 둔 가정의 방문서비스로 확대할 것을 제시하였다.마지막으로 영유아건강감시체계에 대한 기본안으로 영유아건강 주요지표를 추출하기 위해 영유아건강관련 주요 지표를 인구학적 상태지표,건강상태 지표, 결과 지표의 세 가지로 분류하였다. 다음으로 영유아 건강요구를 반영한 9개의 핵심영역별로 국내외 지표를 분석하여 핵심영역별로 아동건강 주요 지표를 추출하였다.

The new WHO standards prove that differences in children's growth to age five are more influenced by nutrition, feeding practices, environment, and healthcare than genetics. From this perspective, current child health is more focused on early health intervention rather than just treating health problems after it occurs.The purpose of this study are to access the health care needs in young children, and to develop a health care program for those children, 0 to 6years of age.The inductive method was applied to extract the core domains in child health needs. For this purpose, a total of 59 public programs which had been initiated by governmental department or major NGOs were identified and categorized nine core domains. Using the list of core domains as a guide, the capacity of child health care was analyzed. The capacity of child health care includes public health care services for children, the type and major roles of institutions and staffs.The quantitative and qualitative needs assessment was done.
For the quantitative needs assessment, indicators related to child health care were identified. A total of three focus group meetings including experts'brainstorming, practitioners' discussion session, and telephone interview were conducted as a qualitative approach. A summarized outline from each meeting and interviews was generated after further review of the meetings and interviews writing notes and recorded. Issues and problems raised and discussed during the focus group meetings and highlighted in the focus group meeting outlines. The "Community based healthy child project" was developed based on the results of these assessment process.The two core issues in the needs assessment include 1) the lack of integrated health care service information system, and 2) the low quantity and quality of well-child check-up.The goals of "Community based healthy child project" are to improve the well child care (primary care) for young children and to reduce parenting difficulties.
Five core components were developed to improve program efficiency, quality, dissemination, accessibility, and continuity. They are 1)integrated child health care information system (child health nevigator), 2)child health specialists, 3) on-site child health hotline, 4) home visiting, and 5) child health monitoring system.The child health nevigator is a database showing step by step information on the introduction, objectives, service items, contact information of a child health service to parents or health care providers. Child health specialist is a person who can provide growth and development evaluation and counseling regarding child development, parenting, and other child rearing issues. On-site child health hotline is a user-friendly telephone line parents or health care providers can use when questions arise about their child's behavior and development. The information through this telephone line is provided by the child health specialist rather than trained lay persons.
Home visits are also offered in terms of outreach. Home visits for young children more than 18 month old which is added to the current tailored home visit service due to the fact that these children and their parents are likely to have hard time to raise their young children, in particular if they have some vulnerable factors, such as low socioeconomic status, children with disability,and multicultural families.Through the findings of this needs assessment and the developed program, we expect children born in Korea can be given the optimum start in life and have the potential to develop their growth and development, and their opportunity to achieve full potential as adults.
