연구배경 및 목적: 대부분의 국가에서 여성은 동일한 수준의 사회경제적 위치에 속해있는 남성보다 오래 사는 반면, 자가 보고된 건강상태, 만성 질환이나 활동장애가 더 높고, 대부분 질환에서 여성의 이환율이 더 높은 것으로 지속적으로 보고되어 왔음. 우리나라의 경우에도 여성이 남성보다 수명이 길지만, 건강수명에 있어서는 차이가 미미함. 따라서 본 연구는 성별 특이적인 삶과 사회적 환경이 건강수명에 미치는 영향을 분석하여 연구하고자 함. 또한 분석 결과에 기초하여 여성건강수명 연장을 위한 정책방향을 제시하고 중장기적인 여성 건강수명 연장

연구방법: 여성건강에 영향을 미치는 사회환경적 요인을 분석하기 위하여 사회환경적 요인들을 포함하는 다양한 건강결정요인 모형을 비교 검토하여 성별, 연령, 유전적 요인 등 개인 고유의 특성을 근간으로 건강에 영향을 미치는 4단계의 사회환경적 요인(건강행태, 사회적 네트워크, 교육, 직업, 고용 등 사회구조적 요인 및 일반적 사회경제문화적 환경조건)을 구분함. 건강의 사회적 모형을 바탕으로 국가적으로 대표성 있는 표본을 포함한 이차자료인 종단적 분석이 가능한 패널 자료와 횡단적 분석이 가능한 조사 자료를 활용하여 분석함. 구체적으로 건강수명을 HALE(Health-adjusted Life Year)로 사회환경적 요인 수준별로 산출하고, 전반적으로 건강수명 산출에 기반이 되는 건강관련 삶의 질(Health-related Quality of Life; HRQoL) 및 주관적 삶의 질 에 영향을 미치는 유의한 사회환경적 요인을 산출함과 함께, 여성에게 상대적으로 큰 부담이 되는 주요 질환별 사회환경적 위험요인을 분석함. 분석한 결과를 기초로 취약계층을 도출하고, 이들을 위한 정책 및 연구과제를 제안함
연구결과: 건강관련 삶의 질 지표를 조사하는 18세 이상 성인 한국의료패널(2012) 11,028명과 국민건강영양조사 제5기(2010-2012) 17,263명의 자료를 이용하여 성별차이를 분석한 결과, 남성보다는 여성에서 유의한 사회환경적 요인이 더 많았고, 두 성별 모두 유의한 영향요인에서도 성별 크기 및 방향에 있어서 차이가 있는 것으로 나타남. 특히 남성에 비하여 여성에서 연령군별로 매우 다른 양상을 보임. 또한, 주요 질환으로 비만, 당뇨병, 고혈압, 골관절염, 골다공증 등을 구체적으로 파악한 결과, 건강위험요인에서 성별차이는 매우 크게 나타났으며, 사회환경적 요인에서의 차이도 질환별로 매우 다른 양상을 보임. 또한 자료에 따라서 유의한 영향요인 및 영향력의 크기도 다르게 나타남
결론: 연령이 높아질수록 고령자 성별 인구 격차가 커져서 급격한 여성 고령인구의 증가로 인하여 여성의 건강수명이 정책으로 이슈가 되고 있으나 여성 건강수명에 영향을 미치는 요인에 대한 관련 근거가 부재한 현실에서, 본 연구는 남성에 비하여 여성에서 교육, 경제수준, 직업, 가족관계 등 사회환경적 요인이 상대적으로 매우 중요함을 객관적 자료를 통하여 도출함. 또한, 주요 질환별로 분석한 경우에도 사회환경적 요인은 질환별로 성별로 서로 다른 요인들이 건강위험요인으로 영향을 미치고 있음. 따라서 여성건강수명 제고를 위한 정책방향 및 연구과제 제안에 있어서 전반적인 사회구조 개선과 함께 건강취약계층 대상으로 세분화된 접근이 필요함을 강조하고 예시를 제안함. 단, 자료원에 따라 유의한 영향요인의 차이가 나타난 문제점은 복수의 다양한 이차자료 사용의 한계로 향후 연구결과가 누적되면서 결과를 수렴해갈 수 있을 것으로 기대함

A variety of biological and behavioral explanations were suggested for this apparent contradiction, but we still do not fully understand the reasons for the paradox or its mechanisms. Although biological factors such as genetics, prenatal hormone exposure and natural hormonal exposure as adults may contribute to differences in men's and women's health, a wide range of social processes can create, maintain or exacerbate underlying biological health differences. Therefore, this study focused socio-environmental factors affecting healthy life years in Korea, especially for women.
Methods: This study analyzed the data from Korea Health Panel (KHP) and Korea National Health and Nutrition Examination Survey (KNHANES), which are large and publicly available data sets collected annually in Korea. For adults aged over 18, HALE(Health-adjusted Life Year) was estimated for levels of socio-environmental factors. The demographic, family network, socioeconomic status, psycho-social status, health behavior, and health status factors were also considered to explain health-related quality of life (HRQoL) by gender. For each gender, more detailed analyses were conducted by age groups: young (18-44), middle (45-64), and old (65 or older). In addition, major diseases such as obesity, diabetes, hypertension, osteoarthritis, osteoporosis were analyzed for socio-environmental risk factors.
Results: Social determinants played a crucial role in the health of women compared to men. Education level, employment, income, family and social support, and psycho-social status were all significant components of social and economic determinants of health. Even after controlling for health behavior and health status, many social determinants were significant. These significances were different among age groups, and between objective and subjective HRQoL assessments. In addition, the socio-environmental factors affected differently risk of major diseases. However, some of the results were not consistent over the data between KHP and KNHANES.
Conclusions: Because social and biological models describe different aspects of men's and women's health differences, it is crucial that we begin to combine studies of social and biological health risks if we wish to fully understand the determinants of men's and women's health and illness. Science and health care advance more slowly and less efficiently than might be achieved under more ambitious and better integrated approaches. Strategic research on social and biological factors affecting men's and women's health could reduce disparities by improving research, prevention, diagnosis and treatment. More interdisciplinary research is necessary in order to adequately examine issues of gender, race/ethnicity, and class in studies of health behaviors, morbidity and mortality.
