연구의 목적 및 내용
한국인 결핵환자에서 항결핵제 약물치료의 안정성 및 유효성 향상을 위해, 약물반응 영향요인들을 종합 분석하여 과학적 및 임상적 근거를 확보하고, 약물유전체 기반 용량결정 및 종합적 환자모니터링 방법을 포함한 임상진료지침 개발 및 적용 모델을 제시하므로써 한국인 결핵환자에서 항결핵제 약물치료의 안전성 및 유효성 향상에 기여하고자 함.

연구의 목적 및 내용
한국인 결핵환자에서 항결핵제 약물치료의 안정성 및 유효성 향상을 위해, 약물반응 영향요인들을 종합 분석하여 과학적 및 임상적 근거를 확보하고, 약물유전체 기반 용량결정 및 종합적 환자모니터링 방법을 포함한 임상진료지침 개발 및 적용 모델을 제시하므로써 한국인 결핵환자에서 항결핵제 약물치료의 안전성 및 유효성 향상에 기여하고자 함.
연구개발성과
1. 진료 환경 내 맞춤진료 임상시험 프로토콜 구축 및 임상시험 수행
- 결핵환자 및 대조군 대상 후향적 및 전향적 임상시험 표준 프로토콜을 개발하여 임상시험을 성공적으로 수행함. (결핵환자 총 500예, 대조군 350예 완료)
2. 각종 임상 지표 및 시료 수집과 검사자료 확보, 통합 DB 구축
- 기본 인구역학 정보, 투약력 및 질환관련정보, 기본혈액화학검사, 유전형 및 항결핵제 약물농도, 영양 및 면역상태 평가지표 등을 포함한 직접 측정 및 수집하고, 통합 DB 개발 구축 운영함. (혈청 및 혈장, DNA를 포함한 임상검체 총 1,000개 이상 확보 완료)
3. 용량결정모델 개발 및 임상 검증
- 결핵진단 후 표준용량치료 시작한 환자 대상으로 NAT2 유전형 및 약동학 기반 집단약동학 모델링을 수행하여 적정용량 예측모델을 개발하였고, 이를 적용한 임상시험을 통해 용량예측모델 적용 환자군에서의 약물치료 유효성 향상 및 부작용 감소 효과 확인함.
4. 임상환자검체 대상 다양한 약물반응 관련 지표 분석
- 결핵환자 및 대조군 검체를 대상으로 NAT2 유전형 및 항결핵제 4종 약물농도측정, 미량원소 8종 및 비타민 8종, 사이토카인 9종을 포함한 영양 및 면역상태 평가지표 측정 수행하고 임상적 유용성 및 상관성을 평가함.(각 항목별 156~605예의 임상검체를 대상으로 함, 총 10,000건 이상의 분석 수행, 논문)
5. 약물유전체기반 맞춤용량결정 및 모니터링 기법을 포함한 항결핵제적정결핵치료지침 개발

Response to treatment of tuberculosis could be affected by multiple genetic and non-genetic factors. These factors must be taken into consideration for effective tuberculosis control. The aim of this study was to evaluate the potential clinical usefulness of using pharmacogenetic data to guide the management of anti-tuberculosis therapy and to characterize the effects of genetic and non-genetic factors that might contribute to the variability in drug response in Korean patients with tuberculosis, with the view to developing an individualized dosing regimen.
Results
1. Establishment of a standardized clinical trial protocol and conducting retrospective and prospective clinical studies for the personalized anti-tuberculosis treatment in a clinical setting. (500tuberculosis, 350 controls)
2. Collection of clinical information, laboratory parameters, and clinical samples, with a comprehensive database (>1,000 clinical specimens)
3. Developoment and clinical validation of NAT2 genotype-based individualized dosing in Korean patients with tuberculosis

Expected Contribution
Pharmacogenetics-based individualized dosing guidelines and therapeutic drug monitoring during treatment of anti-tuberculosis drugs show promise for optimizing tuberculosis treatment. Also, comprehensive assessment and management of comorbid conditions, including nutritional and immunological status, can improve safety and efficacy of anti-tuberculosis therapy in Korean patients with tuberculosis.
