전 세계적으로 동물실험 금지에 따른 동물대체시험법 개발이 요구됨에 따라 국제표준기구(ISO)에서도 의료기기 생물학적 안전성평가 시 3R 원칙(동물 수 감소, 고통경감, 대체) 등 동물복지를 우선 고려하고 있다.
이에 2018년 OECD 피부감작성 시험가이드라인으로 채택된 국내 개발 유세포 분석을 이용한 국소림프절시험법(LLNA:BrdU-FCM)을 ISO 피부감작성 시험가이드라인(10993-10)에 반영할 수 있도록 의료기기 피부감작성 동물대체시험법 적용을 위한 연구 개발이 필요하다.
따라서 본 과제에서는 의

이에 2018년 OECD 피부감작성 시험가이드라인으로 채택된 국내 개발 유세포 분석을 이용한 국소림프절시험법(LLNA:BrdU-FCM)을 ISO 피부감작성 시험가이드라인(10993-10)에 반영할 수 있도록 의료기기 피부감작성 동물대체시험법 적용을 위한 연구 개발이 필요하다.
따라서 본 과제에서는 의료기기 생물학적 안전성 평가 시 피부감작성 동물대체시험법의 적용 기반마련을 위한 연구를 수행하였다.
의료기기 시험물질(시편) 제작을 위해 OECD 피부감작성 시험법 개발에 사용한 감작물질 42종의 감작물질을 선별하고 그 중 ISO 10993-10에 따라 약, 중감작성 물질 15종을 선별하였으며, Base material (치과용 인상재)과 중합이 용이한 10종의 의료기기 시험물질(시편)을 제작하였다. 시험물질은 가속용출, 시험물질 비율(6 cm2/mL 또는 0.2 g/mL), 용출 온도 및 시간(37 ± 1℃, 72 ± 2 h) 등의 용출조건으로 극성 용매(Saline)와 비극성 용매(Sesame oil)로 각각 용출하였으며, 용출물 평가 결과(pH 및 성상 평가 등)에 따라 최종적으로 4종의 의료기기 시험물질(시편)을 선정하였다.
이 시험물질(용출물)을 대상으로 마우스를 이용한 LLNA:BrdU-FCM을 실험한 결과, 40% Isoeugenol in dental silicone을 비극성 용매에 용출한 시험물질(5,000 ppm 이상)에서 감작이 나타난 반면에 극성 용매에서 용출한 시험물질은 비감작이 나타났다. 또한 비극성 용매에 용출한 10%, 20%, 40%의 Isoeugenol in dental silicone의 피부감작성 시험결과 40% Isoeugenol의 고농도 군에서 감작성이 나타났다.
현재 OECD TG 442B에 제시되어 있는 유세포 분석을 이용한 국소림프절시험법은 ISO 10993-10 시험법으로 제안되어 개정 중(질의단계, 2020)에 있다.
최근 피부자극시험의 중요성이 대두됨에 따라 ISO 피부감작성과 자극 시험가이드라인(10993-10)에서 자극과 관련한 부분이 10993-23 제정(안)으로 제안되고 이를 위한 승인 절차가 진행되고 있다. 이에 따라 OECD TG 439에 시험법 승인을 제안해 진행 중인 국내 개발 인체피부모델(KeraSkin™)을 이용한 피부자극시험을 의료기기 분야에 적용하여 ISO 10993-23의 시험법으로 제안하기 위한 연구개발이 필요하다.
따라서 2차년에는 의료기기 피부자극 평가와 관련해 인체피부모델(KeraSkin™)을 이용한 피부자극 동물대체시험법의 의료기기분야 적용기반 마련을 위한 연구를 수행하였다.
국내 개발 인체피부모델(KeraSkin™)을 이용한 피부자극시험법 개발을 수행하기 위해 우선 숙련도 물질(10종)을 이용하여 시험자의 숙련도를 평가하고, 기 수행된 Round robin study와 동일한 의료기기 피부자극 시험물질(시편)을 대상으로 시험방법의 검증을 요구하는 ISO 10993-23(발간단계, 2020년)에 따라 5종의 의료기기 피부자극 시험물질(시편)을 극성 용매(Saline)와 비극성 용매(Sesame oil)로 각각 용출해 이를 이용하여 국내 개발 인체피부모델(KeraSkin™)을 이용한 피부자극시험법 개발 연구를 수행하였다.
시험물질 적용시간(18h), 시험물질 적용량(100 ㎕), MTT 농도(1 mg/mL) 등 확립된 시험 조건으로 반복실험(3회)한 결과 자극물질 SDS를 포함한 의료기기 시험물질(시편) 15% SDS in silicone의 극성 용매 용출물은 자극, 비극성 용매 용출물은 비자극을 나타내고, 자극물질 Genapol X-80을 포함한 5.8% Genapol X-80 in PVC(Y-4)는 극성 및 비극성 용매 용출물은 모두 자극을 나타내며, 4% Genapol X-100 in PVC 또한 극성 및 비극성 용매 용출물에서 모두 자극을 나타냄을 확인하였다. 또한 각 자극 물질(SDS, Genapol X-80, Genapol X-100)을 포함한 의료기기 시험물질(시편)의 base material로 사용된 Silicone 및 PVC는 극성 및 비극성 용매 용출물에서 모두 비자극을 나타냄을 확인하였다. 이는 ISO 10993-23의 승인된 인체피부모델(EpiDerm™, SkinEthic™)을 이용한 피부자극시험과 동일한 결과였다.
자극물질 3종을 대상으로 자극물질 함유량을 분석한 결과(3회) 극성 용매 용출물에서 15% SDS in silicone은 3653 ± 688 ppm, 5.8% Genapol X-80 in PVC 1039 ± 93 ppm, 4% Genapol X-100 in PVC는 검출이 불가하였으며 비극성 용매 용출물에서 3종 모두 검출이 불가하였다.
의료기기 피부자극 시험물질(시편) 전체(6종)를 대상으로 실험실 내 시험자 간 반복시험(5회)을 수행한 결과 재현성 및 시험자 간 일치도가 100%로 나타났고 국내 개발 인체피부모델(KeraSkin™)을 이용한 피부자극시험에서 높은 정확성을 나타내었다. 또한 ISO 10993-23에 승인된 선행 피부모델의 결과와 동일한 결과를 보이는 것으로 보아 의료기기 분야의 피부자극평가에 적합하다는 것을 확인하였다.
따라서 본 과제를 통해 국내 개발 인체피부모델(KeraSkin™)을 이용한 의료기기의 피부자극 대체시험법을 ISO 10993 표준 시험법으로 제안하기 위한 과학적 근거자료를 확보하였다.

As the prohibition on animal testing for cosmetics increases the need to develop alternatives to animal testing around the world, the International organization of standardization (ISO) considers animal welfare such as the 3Rs Principles (reduction, refinement and replacement) a priority in biological safety assessment of medical devices.
In this context, further research and development in the application of alternative test methods for prediction of skin sensitizing potential of medical devices is required in order to have the LLNA:BrdU-FCM developed in Korea be approved as the ISO Test Guideline for skin sensitization (10993-10).
Therefore, this study aimed to lay the foundation for the application of alternative test methods for skin sensitization to the biological safety assessment of medical devices.
Among the materials used to develop the OECD test methods for skin sensitization, 42 sensitizers were selected based on the OECD GD 34. In addition, out of the 42 chemicals, 15 chemicals with weak and moderate skin sensitizers were selected in accordance with the ISO 10993-10. Then, 10 skin sensitizing materials that are able to be polymerized with the base materials (Dental silicone, Alginate) were used to produce the test materials of medical devices. The test materials were extracted both in polar solvent (saline) and in non-polar solvent (sesame oil) at the ratio of 6 cm2/mL or 0.2 g/mL using accelerated extraction at 37 ± 1℃, 72 ± 2 hr. Finally, based on the assessment of the extracts (i.e. physical characteristics, pH and the concentrations of the extracts), 4 test materials were selected.
As a result of experiment with the LLNA:BrdU-FCM on the test materials, the materials that were extracted in the polar solvent were indentified as skin sensitizers while the materials extracted in the polar solvent (over 5000 ppm) were identified as non-sensitizers. In addition, 10, 20 and 40% Isoeugenol in dental silicone that were extracted in the non-polar solvent were tested respectively, and the 40% Isoeugenol was identified as a skin sensitizer.
Local lymph node assay presented in the OECD TG 442B has been proposed as a test methods of the ISO 10993-10 and is being revised (Enquiry Stage, 2020).
As the importance of skin irritation testing has come to the fore, the parts related to irritation in the ISO Test Guideline for skin sensitization and irritation (ISO 10993-10) have been proposed as the draft revision of the ISO 10993-23 and the processes for its approval is underway. In this respect, in order to apply the KeraSkinTM SIT, a method developed in Korea and proposed as a “me-too” test method in the OECD TG 439, to medical devices and propose the method as a test method of the ISO 10993-23, further research and development is required.
Therefore, in the second year, this study aimed to lay the foundation for the application of KeraSkinTM SIT to medical devices.
In order to develop a skin irritation test method using KeraSkinTM model, 10 proficiency chemicals were tested to evaluate the proficiency of experimenters. Then, a study on the development of a skin irritation test method using a KeraSkin™ was performed according to ISO 10993-23(Publication stage, 2020), which suggested that the validation study for a new RhE model listed in the OECD 439 for in vitro skin irritation must demonstrate equal performances in terms of predictive capacity to those of the Round robin study(De Jong et al, 2018). So, we used polar(saline) and non-polar(sesame oil) vehicle extracts of 5 medical devices, the same materials that were used in Round robin study(De Jong et al, 2018).
As a result of 3 runs under the established conditions (i.e. application time (18hr), test material volume (100 ㎕), MTT concentration (1 mg/mL)), the extracts of 15% SDS in silicone were identified as irritants in polar solvent and as non-irritant in non-polar solvent. In addition, the extracts of both 5.8% Genapol X-80 in PVC(Y-4) and 4% Genapol X-100 in PVC were identified as irritants both in polar and non-polar solvent. Silicone and PVC used as the base materials of the test materials that contain irritants (SDS, Genapol X-80, and Genapol X-100) were identified as non-irritant in both polar and non-polar solvents. All of the tests produced the results that are identical to those of the tests using RhE (EpiDerm™, SkinEthic™) that have been adopted as the ISO 10993-23.
As the results of the quantitative analysis of 3 irritants in the extracts, the polar extracts of 15% SDS in silicone and 5.8% Genapol X-80 in PVC contained 3653 ± 688 ppm and 1039 ± 93 ppm of the irritants (SDS and Genapol X-80), respectively. But, these irritants (SDS and Genapol X-80) were not detected in the non-polar extracts. Meanwhile, the irritant (Genapol X-100) could not be analyzed in both polar and non-polar extracts of 4% Genapol X-100 in PVC.
In the within experiments for KeraSkin™using 6 medical devices(2 experimenters, 5 runs), reproducibility was 100%, and KeraSkin™ showed high accuracy in the skin irritation test. In addition, it was confirmed that it was suitable for skin irritation evaluation in the field of medical devices, as it showed the same results as those of the preceding skin model approved in ISO 10993-23.
As a result of conducting repeated tests (5 times) between testers in the laboratory for all of medical device, the reproducibility and agreement between testers were 100%, and a domestically developed KeraSkin™ showed high reproducibility in the skin irritation test. In addition, it was confirmed that it was suitable for skin irritation evaluation in the field of medical devices, as it showed the same results as those of the preceding skin model approved in ISO 10993-23.
Therefore, through this project, scientific basis data was obtained to propose an alternative test method for skin irritation of medical devices using the domestically developed KeraSkin™ RhE as the ISO 10993 standard test method.
