소아는 생리학적으로 성인과 차이가 존재하며 발달과정에 따라 변화하는 양상이 다양하므로 약물의 적정 용량·용법 설정에는 많은 요인이 고려되어야 한다. 특히 소아를 대상으로 한 임상시험을 통해 적응증에 대한 유효성과 안전성을 평가하는 것에는 많은 제약이 따른다. 이에 최근 국내외 의약품 규제당국은 의약품 허가자료에 계량약리학적 연구결과를 사용할 수 있도록 허용하고 있으며, 제약사는 IND/NDA 제출자료 중 계량약리학적 연구자료를 근거자료로 활용하고 있다.
소아 임상시험의 한계로 소아에 대한 의약품은 빈번하게 허가외 사용이
소아는 생리학적으로 성인과 차이가 존재하며 발달과정에 따라 변화하는 양상이 다양하므로 약물의 적정 용량·용법 설정에는 많은 요인이 고려되어야 한다. 특히 소아를 대상으로 한 임상시험을 통해 적응증에 대한 유효성과 안전성을 평가하는 것에는 많은 제약이 따른다. 이에 최근 국내외 의약품 규제당국은 의약품 허가자료에 계량약리학적 연구결과를 사용할 수 있도록 허용하고 있으며, 제약사는 IND/NDA 제출자료 중 계량약리학적 연구자료를 근거자료로 활용하고 있다.
소아 임상시험의 한계로 소아에 대한 의약품은 빈번하게 허가외 사용이 이루어지고 있어 의약품 안전사용의 사각지대에 놓여져 있다. 이에 식품의약품안전처에서는 안전한 의약품 사용을 위해 이러한 소아대상 허가초과 의약품 중 적정한 용량·용법 설정이 필요한 약물을 선정하여 임상시험을 실시하고, 그 결과를 바탕으로 계량약리학적 접근을 통해 해당 의약품의 소아에 대한 적정 용량·용법을 제안하였다.
대상 약물 선정은 자체 문헌조사와 실제 임상현장에서의 전문가 요구를 종합하여 선정하였으며, 연구기간 내 모집 가능한 대상자 수, 소요 연구비 등을 고려하였다. 최종적으로 선정된 약물은 「덱스메데토미딘」으로 국내에서는 집중치료 관리하의 진정, 수술 및 시술 시 비삽관 환자의 의식하 진정의 적응증으로 성인에 대해서만 허가되어 있다. 국내와 유사하게 미국 FDA와 유럽 EMA에서도 동 제품은 소아에 대한 허가는 없다. 반면 일본 PMDA는 소아환자에 대해 집중치료 관리하의 인공호흡 및 이탈 후의 진정으로 허가되어있다.
이와 관련하여 식약처에서는 2014년 중환자실에서 기계환기를 받는 소아의 진정을 연구하기 위하여 임상시험을 실시하였으나 연령의 분포가 2-12세로 13세 이상의 소아환자 데이터가 없어 소아 전 연령을 평가하기에는 한계점이 있었고, 해당 적응증에 대하여 추가적인 연구가 필요하다고 판단되어 2014년 기 수행된 임상시험을 토대로 추가적 임상시험을 실시하였다.
임상시험은 서울대학교병원에서 실시하였으며, IRB 승인 후 대상자를 모집하여 일정한 기준에 적합한 7-18세 소아 12명이 참여하였다. 약동학은 덱스메데토미딘 투약 전부터 투약 후 8시간까지 채혈하여 측정하였으며, 약력학은 투약 전부터 투약 후 8시간까지 약동학 측정시점과 동일하게 BIS와 UMSS 점수를 평가하였다. 약동학과 약력학의 분석, 약동-약력학의 상관관계 등을 통해 덱스메데토미딘은 적응증인 전신마취 후 기계환기를 받는 7-15세의 소아에게 진정의 목적으로 투약하기 적합한 약물로 확인되었으며 약물 투약으로 인한 중대한 이상반응은 관찰되지 않았다.
계량약리학적 분석은 2014년 기 수행된 임상시험과 본 연구의 임상시험의 결과를 합하여 총 40명의 소아환자 데이터를 가지고 실시하였다. 집단약동학 기본모델은 결측치를 포함하여 추정하는 M3법을 사용한 3구획 모델로 구축하였으며, 청소율에 대한 연령의 효과를 반영하여 최종모델로 확립하였다.
덱스메데토미딘의 적정 용량·용법을 위한 시뮬레이션은 일본 PMDA 허가사항의 소아 임상시험 방법을 참고하였으며, 투여종료 시 목표 혈중농도 이내인 환자의 비율을 평가하여 전체 대상자의 약 75%와 유사할 때의 용량·용법을 최적으로 판단하였다. 그 결과 6세 이상에 비해 6세 미만의 소아에서 더 큰 용량을 투여할 필요가 있었다. 연령범위에 따라 최적이라고 판단된 용량은 본 연구의 임상시험 투여량보다 더 크지만 국외에서 허가외로 사용하는 용량의 수준을 벗어나지 않는 정도임을 확인하였다. 집단약동-약력학 모델은 UMSS를 가지고 구축했으며, UMSS가 비연속형 데이터이므로 약물의 효과가 나타날 확률로 표현할 수 있는 모델을 사용하였다. 집단약동-약력학 모델 기반 시뮬레이션은 앞서 수행된 시뮬레이션과 동일한 시나리오로 구성하였으며, PMDA 허가사항의 소아 용량과 비교를 위해 7개의 시나리오를 추가하여 수행하였다. 투여종료 후 3시간째에 UMSS가 목표 진정범위(1~3점)에 있을 확률을 평가하였다. 결과적으로 용량이 증량됨에 따라 증가 정도는 크지 않았지만, 확률이 증가하는 경향을 보였다. 계량약리학적 분석 결과, 덱스메데토미딘의 부하용량을 연령에 따라 증량할 필요는 없을 것으로 판단하였고, 집단약동-약력학 시뮬레이션 결과를 고려하여 유지용량을 결정하였다. 본 연구에서 제안한 적정 용량이 소아 전 연령대에서 목표 진정범위 내에 있을 확률이 70% 이상이므로 4시간 이내의 기계 환기를 위한 진정제로서의 효과가 유효함을 확인할 수 있었다.
본 연구에서는 이번 임상시험 결과와 2014년 기 수행 임상시험 결과를 통합하여 수술 후 중환자실에서 기계 환기를 받는 전 연령대의 소아를 대상으로 덱스메데토미딘의 약동학, 약력학, 약동-약력학 관계 등을 계량약리학적으로 분석하여 적정 용량·용법을 도출하였다.

Therefore, many factors shoulde be considered when setting the appropriate dosage regimen for drugs. In pediatric clinical trials, general clinical trials have many limitations in assessing efficacy and safety for indications. Recently, the pharmacometric approach is reflected in foreign and domestic regulatory science.
The IND/NDA data is submitted with the result of pharmacometric approach as the base data.
The use of pediatric drugs is frequently used in off-label, because of the limitation of pediatric clinical trials. So, MFDS selected drugs requiring dosage regimens from off-label in pediatrics and conducted clinical trials. Based on the results of clinical trials obtained, we proposed a proper dosage regimen of the drug in pediatric through the pharmacometirc approach.
The drug for this research was selected by combining their own literature survey and the demands of experts in the actual clinical field, and the number of subjects that could be recruited during the study period and required research expenses were considered. Dexmedetomidine(DEX) was selected, and in Korea, it is approved only for adults as an indication for sedation under intensive care and consciousness sedation of non-intubated patients during surgery and procedures. Similar to MFDS, FDA and EMA do not have permission for this product for children. On the other hand, PMDA is truly licensed for pediatric patients after intensive ventilation and evacuation.
In this regard, MFDS conducted a clinical trial to study sedation of children receiving mechanical ventilation in the intensive care unit in 2014, but the distribution of age is 2-12 years old, and there is no data on children over 13 years old, so it is difficult to evaluate the entire age of children.
It was determined that additional research was necessary for the indication, and additional clinical trials were conducted based on the clinical trials conducted in 2014.
The clinical trial was conducted at Seoul National University Hospital, and after IRB approval, 12 children aged 7-18 who met certain criteria participated. Blood samples were measured by collecting blood from before administration to 8 hours after administration of DEX, and PD was evaluated by BIS and UMSS scores in the same manner as at the time of PK measurement from before administration to 8 hours after administration. As a result of the analysis of PK and PD, through the correlation between PK and PD, DEX was identified as a drug suitable for sedation to children aged 7-15 years receiving mechanical ventilation after general anesthesia, an indication. No serious adverse reactions were observed.
The pharmacometric analysis was conducted with the data of a total of 40 pediatric patients.
The base model for PK model was constructed as a three-compartment model using the M3 method to estimate including missing values, and was established as a final model by reflecting the effect of age on the clearance. The simulation for the proper dosage and administration of DEX was based on the pediatric clinical trial method of the Japanese PMDA label. At the end of the administration, the proportion of patients within the target blood concentration was evaluated, and the dosage regimen was optimally determined when it was similar to about 75% of the total subjects. As a result, it was necessary to administer a larger dose in children under 6 years of age compared to children over 6 years of age. The dose determined to be optimal according to the age was larger than the dose of the clinical trial in this study, but it was confirmed that it was within the level of the dose used by other countries as off-label. The population PK-PD model was developed with UMSS, and since UMSS is a non-continuous data, a model expressing the probaility of occurrence of drug effeccts was used. The model-based simulation was performed in the same scenario as the previously performed simulation and seven scenarios were added to compare with PMDA pediatrc dose of DEX, The probability of UMSS in the target sedation range was evaluated 3 hours after the end of administration. Although the degree of increase was not significant, the probability increased as the dose was increased. As a result of pharmacometrics analysis, it is considered that it is not necessary to increase the loading dose of DEX and the maintenance dose was determined according to population PK-PD simulation. When the clinical trial dose of this study is administered, the probability of being within the target sedation range in all age groups of pediatric is more than 70%. It was confirmed that the effect as a sedative for mechanical ventilation within 4 hours was effective.
In this study, the PK, PD and PK-PD relationship of DEX were analyzed pharmacometrically in pediatric of all ages receiving mechanical ventilation in the ICU after surgery by integrating the results of this clinical trial with the results of a clinical trial conducted in 2014 to derive an appropriate dosage regimen.
