Long COVID는 단순한 증상뿐만 아니라 심장 질환, 당뇨, 신장질환 등 여러 만성 질환으로도 나타날 수 있음이 알려져 중요성이 더욱 부각되고 있으며 경증 코로나19 환자들도 만성 후유증을 호소하는 경우가 많은 것으로 알려져 있다. 앞으로도 코로나19는 지속적으로 발생할 것으로 예상되기에 장기 후유증에 관한 연구는 매우 중요하다고 할 수 있다. 이에 따라 본 연구는 체계화된 코로나19 코호트 구축을 통해 코로나19의 임상, 역학적 특성을 이해하고 중장기 합병증 발생 및 후유증에 대해 평가하고자 하였다. 또한, 코로나19 회복 환자들의 세포매개면역 반응의 장기 변화와 중증도에 따른 면역 반응을 비교하여 그 임상적 의미를 분석하고자 하였다.
다기관 코로나19 코호트를 형성하여 2년 동안의 증상, 혈액 검사, 폐기능검사, 영상학적 검사 등을 추적 관찰하여 시간에 따른 변화와 중증도에 따른 차이를 분석했고, 이들의 혈액 검체를 이용하여 세포매개면역 반응을 분석하였다. Long COVID는 3개 이상의 증상을 호소하는 경우로 정의하였다. 또한 국민건강보험공단 자료를 이용하여 코로나19 환자들의 심뇌혈관 질환, 호흡기질환, 신장질환의 발생 추이를 조사하였다.
총 290명의 코호트 환자들이 등록되었다. 이 중 절반 이상인 182명(62.8%)의 환자가 추적관찰 중 Long COVID를 호소하였다. 특히 코로나19 감염 18개월 이후에도 30% 이상의 환자들이 Long COVID에 해당할 정도로 상당수의 환자가 코로나19 후 후유증을 장기간 호소하는 것으로 나타났다. 피로, 기억력 감퇴, 집중력 저하 등이 제일 자주 호소한 증상이었고 코로나19의 중증도와 Long COVID는 연관이 없었다.
외상 후 스트레스 장애 및 범 불안 장애 선별 평가에서도 30%가량의 환자들이 관련 증상을 호소하는 것으로 확인되었다. 코로나19 중증도가 높을수록 호흡기 관련 후유증을 더 많이 호소하는 양상이었고, 폐기능 검사 결과도 중등증/중증 군에서 좀 더 낮게 나오는 양상이었다. 코로나19 회복 환자의 세포매개면역 분석상 SARS-CoV-2 specific T세포, 특히 SARS-CoV-2 specific CD4+ T세포가 감염 후 10개월까지도 잘 유지됨을 확인하였고, SARS-CoV-2 specific TSCM 및 polyfunctiaonality 도 지속되는 것을 확인하였다. 국민건강보험공단 자료를 이용한 코로나19 합병증 분석상 코로나19 환자에서 심뇌혈관 질환, 급성 신부전, 폐렴이 미감염자 대비 발생 위험이 큰 것으로 나타났다.
결론적으로, 코로나19 회복 환자들은 중증도와 관계없이 장기 코로나 후유증을 자주 동반하며, 여러 합병증 발생의 위험도 증가함을 확인하였다. 또한 코로나19 회복 환자들 에서의 SARS-CoV-2 specific T세포의 지속성을 확인하였다. 본 연구 결과는 앞으로의 코로나19가 국민 건강에 지속적으로 끼칠 영향과 코로나19 백신 접종 등의 국민 건강 관리 측면에서 유용한 자료로 활용될 것으로 기대된다.

Since 2020, a vast number of COVID-19 patients have occurred, and it is known that some patients continue to experience fatigue and other symptoms after COVID-19, also known as Long COVID. Long COVID is gaining importance as various chronic diseases such as heart disease, diabetes, and kidney disease are reported to be prevalent in patients after COVID-19. Moreover, patients with mild COVID-19 also often complain of chronic symptoms. As it is expected that COVID-19 will continue, research on long-term sequelae has become more important. Accordingly, this study aimed to understand clinical and epidemiological characteristics and evaluate the occurrence of chronic sequelae by establishing a multi-center COVID-19 cohort. In addition, we measured the long-term changes in cell-mediated immunity among patients recovering from COVID-19.
For two years, we established a multi-center COVID-19 cohort and performed surveys for symptoms, blood tests, pulmonary function tests, and radiologic studies. Cell-mediated immunity was analyzed using blood samples. Long COVID was defined as complaints of 3 or more symptoms. In addition, using data from the National Health Insurance Service database, we investigated the incidence of cardiovascular disease, pneumonia, and kidney disease in patients after COVID-19.
A total of 290 patients were enrolled in the cohort. 182 (62.8%) patients complained of Long COVID during follow-up. In particular, more than 30% of the patients with COVID-19 correspond to Long COVID even after 18 months of infection. Fatigue, memory loss, and loss of concentration were the most frequent symptoms, and there was no association between the severity of COVID-19 and Long COVID. Screening tools for post-traumatic stress disorder and generalized anxiety disorder identified approximately 30% of patients as likely to have a psychiatric disorder. The higher the severity of COVID-19, the more respiratory-related sequelae complained. The pulmonary function test results also tended to be lower in the moderate/severe group. In the analysis of cell-mediated immunity of patients recovering from COVID-19, SARS-CoV-2 specific T cells, especially SARS-CoV-2 specific CD4+ T cells, were sustained up to 10 months after COVID-19, and SARS-CoV-2 specific TSCM and polyfunctionality were also confirmed to persist. In the analysis of COVID-19 complications using data from the National Health Insurance Service database, the risk of cardiovascular disease, acute kidney injury, and pneumonia was higher in the COVID-19 group compared to the non-infected group.
In conclusion, patients after COVID-19 often have long-term sequelae, regardless of the severity, and increase the risk of various complications. In addition, we confirmed the persistence of SARS-CoV-2 specific T cells in the patients after COVID-19. We expect the results of this study to be used as valuable data regarding the continuous impact of COVID-19 on public health and public health strategy such as COVID-19 vaccination.
