The objective of this study was to evaluate the benefit of glucosamine for OA using meta-analysis combined with systemic quality assessment. Method : Placebo-controlled, double-blinded, randomized human clinical trials were searched in Medline(1966 to 1999) and other sources for unpublished study using the terms of osteoarthritis, clinical trial and glucosamine, and the qualities of studies were scored systematically with quality assessment form. Effect size(ES) of individual study was computed from the intergroup difference in mean outcome values at trial end, divided by the SD(standard deviation) of the outcome value in the placebo group and effect sizes were pooled with a random effects models. Results : Seven studies were included in the analysis. Quality scores ranged from 55.5 to 91.6 with median of 77.7. ES of studies ranged from -0.16 to 2.11 and the pooled effect size was 0.60(95% confidence Interval, CI : 0.13 ∼ 1.07) showing intermediate efficacy. ES(effect size) for oral and parenteral administration were 0.68 and 0.44, respectively. ES of inpatient was 2.11, however in case of outpatient, ES was reduced to 0.41. The difference of ES according to period, administration route and patient status was not significant. Compared with functional outcomes(WOMAC, Lequesne and VAS), pain outcomes was significantly effective(p=0.05). The ES of pain outcomes was 1.35(95% CI : 0.31 ∼ 2.40) and that of functional outcomes was 0.16(95% CI : -0.04 ∼ 0.38). ES was negatively correlated with the number of patient(r=-0.57, p=0.18) and quality score(r=-0/03, p=0.002). Funnel plots showed asymmetry compatible with publication bias. Fail-safe number were 6 for the expected ES of 0.2 and 14 for the expected ES of 0.5. Conclusion : The moderate effect of the glucosamine for treatment of OA was demonstrated.
