FFQ의 개발과 타당성 검증 우리나라 사람들이 많이 섭취하면서 1인 분량당 나트륨 함량이 높은 음식을 112가지 선정하였으며, 섭취빈도는 하루 3회, 하루 2회, 하루 1회, 1주일에 5-6회, 1주일에 3-4회, 1주일에 1-2회, 1달에 2-3회, 1달에 1회, 먹지 않음으로 구성했다. 각 음식별 영양소 섭취량은 1회 분량과 1일 섭취빈도 점수(하루 3회 3점, 하루 2회 2점, 하루 1회 1점, 1주일에 5-6회 0.78점, 1주일에 3-4회 0.5점, 1주일에 1-2회 0.21점, 1달에 2-3회 0.08점, 1달에 1회 0.03점, 먹지 않음 0점)을 곱해 계산하였으며, 이렇게 계산된 각 음식의 영양소 섭취량의 합을 하루 영양소 섭취량으로 했다. 24시간 소변으로부터 추정한 나트륨 섭취량과 FFQ로부터 섭취한 나트륨 섭취량은 약 65%가 일치하였으며, Kappa 지수는 0.533이었다. FFQ의 나트륨 섭취량은 24시간 식이섭취조사법 및 수축기 혈압과 양의 상관관계(r=0.14)를 나타내었다. 2. FFQ의 섭취빈도 및 소금 섭취량 멸치볶음, 물김치, 조개된장국의 섭취량은 고혈압군이 정상군에 비해 유의적으로 높았으며, 이들은 수축기 및 이완기 혈압과도 양의 상관관계를 나타내면서 고혈압의 발병 위험을 높이는 요인(멸치 볶음의 섭취량이 하루 30g 이상일 때 비차비 2.802(1.546-5.079), 조개된장국의 하루 섭취량이24g 이상일 때 비차비 2.934(1.170-5.033))이 되었다. 고혈압군에서 FFQ에 의한 나트륨 섭취량은 6412.9mg으로 정상군에 비해 약 700mg을 더 섭취하면서 혈압과 양의 상관관계를 가지고 있었으며 하루 섭취량이 2400mg 이상일 때 비차비 4.406(2.341-8.291)로 고혈압의 위험요인으로 작용했다. 배추김치, 물김치류, 된장찌개, 멸치볶음, 깍두기, 김치찌개, 자반고등어조림, 시금치나물, 국수장국은 고혈압군의 평균 나트륨 섭취량에서 50%를 차지하는 음식들이었다. 배추김치, 된장찌개, 물김치류, 깍두기, 김치찌개, 멸치볶음, 라면, 미역국, 국수장국, 기타김치류, 시금치나물, 생선구이, 쌈과 쌈장은 정상군의 평균 나트륨 섭취량에서 50%를 차지하는 음식들이었다. 특히 김치는 전체 조사 대상자들의 나트륨 섭취량 중 37.2%를 차지하고 있었다. 자반고등어조림, 멸치볶음, 오이소박이, 불고기, 김치찌개, 생선조림, 생선국, 물김치류, 생선구이, 라면, 만두, 생선젓갈류는 고혈압군과 정상군의 혈압차이를 84% 설명해 준 변이음식들이었다. 3. Na index 개발과 타당성 검증 FFQ에 사용한 각 음식에 대해 섭취량을 고려한 후 나트륨 함량에 따라 4, 3, 2, 1점의 가산점을 부여하고 이 가산점과 섭취빈도 점수를 곱하는 과정으로 Na index를 개발하였다. Na index112는 FFQ에 이용한 전체 음식목록으로 구성되었으며, Na index 55는 나트륨 함량이 높은 50%로, Na index25는 나트륨 함량이 높으면서 전체 조사대상자의 55% 이상이 섭취하는 음식목록으로, Na index14는 전체 대상자의 섭취횟수에 따라 만들어졌다. FFQ의 나트륨 섭취량과 Na index는 39.3∼50.3%가 일치하고 있었으며, 특히 Na index14는 FFQ의 나트륨 섭취량과 약 50% 정도 일치하였다. 연구 3. 표준레시피의 나트륨 분석과 영양교육 프로그램의 개발 1. FFQ에 의한 나트륨 섭취 group별 음식의 섭취빈도 및 섭취량 나트륨 섭취 상위군은 중위군 및 하위군에 비해 수제비, 곰탕, 알탕, 해물탕, 콩나물국, 김치국, 생선구이, 생선튀김, 자반고등어조림, 배추김치, 깍두기, 물김치류, 오이소박이, 불고기, 편육, 멸치볶음과 같은 밑반찬류, 오징어젓, 김구이의 섭취빈도가 높았다. 조사 대상자들은 매일 김치로부터 31.2%, 국이나 찌개로부터 20.3%, 생선으로부터 12%, 젓갈류로부터 3.2%의 소금을 섭취하고 있었다. 미리 소금간을 하고 구운 생선과 간을 하지 않고 구운 후 간장+와사비+레몬즙으로 만들어진 소스를 제공한 생선에 대한 관능검사 결과 맛, 씹는 느낌, 비린내, 전체적인 맛에서 유의차가 없었으며, 이들로부터의 소금 섭취량을 비교했을 때 전자에서는 2.5g, 후자에는 0.8g의 소금을 섭취하는 것으로 조사되었다. 나트륨 섭취 상위군은 중위군 및 하위군에 비해 식탁에 젓갈류가 없을 때 섭섭하다고 느낀다거나, 채소를 충분히 섭취하지 못한다고 느끼는 식행동을 보여주었다. 2. 표준레시피로부터의 나트류 함량 분석 국의 1인 1회 분량에는 1.4-0.5g의 소금이 함유되어 있었으며, 찌개류에는 1.5-4.4g의 소금이 들어 있었다. 국 중에서는 콩나물국의 소금함량이 3.6g으로 가장 높았으며, 애호박 된장국(1.9g), 쇠고기무우장국(1.4g)의 소금함량이 가장 낮았다. 찌개류 중에서는 순두부찌개, 김치찌개의 소금 함량이 4.4g으로 가장 높았던 반면, 동태찌개의 소금함량은 1.5g으로 가장 낮았다. 김치류의 1인 1회 분량 당 소금함량은 0.6-1.5g이었는데 나박김치의 소금함량이 1.5g으로 가장 높은 반면, 갓김치의 소금 함량은 0.6g으로 가장 낮았다. 3. 영양교육 프로그램의 개발 1) 국이나 찌개 · 국물보다 건더기 위주로 섭취할 것 · 국의 섭취량을 감소시키기 위해 그릇의 크기를 작은 것으로 바꿀 것 · 하루 1번 정도는 국을 뜨거운 차나 숭늉으로 대체하거나 식사에서 국을 제외할 것 2) 김치류 · 하루 1번은 김치를 신선한 샐러드 등으로 대체할 것 · 물김치류의 섭취를 최소화 할 것 · 김치를 먹을 때는 무청김치, 갓김치 등을 중심으로 할 것 3) 생선류 · 자반고등어의 섭취를 피할 것 · 미리 소금간을 한 후 생선을 굽기보다 간을 하지 않고 구운 후 간장+와사비+레몬즙과 같은 소스에 찍어먹을 것 · 젓갈류의 섭취를 피할 것 4) 채소류 · 나물류를 조리할 때 애호박, 풋고추, 양파 등과 같이 향이 있는 채소류를 이용할 것.

The hypertensive group(HG) were composed of 112 hypertensive patients (male:53, female:59) who first visited the hypertension clinic and diagnosed as having primary hypertension(SBP≥140mmHg or DBP≥90mmHg). The regular visitors or the subjects on diet or medical therapy were excluded. The control group(CG) was consisted of 95 subjects (male 41, female 54) matched with age and socioeconomic levels. HG showed the mean age of 48.2years and mean blood pressures(BP) of 159.8/103.3mmHg. CG showed the mean age of 46.6 years and the mean BPs of 119.3/75.8mmHg. The following hypertension risks were measured : dietary intake, dietary habit or behavior related to sodium intake, drinking and smoking habits, anthropometric parameters and biochemical parameters like plasma ennin, aldosterone and serum lipid levels. Anthropometric parameters HG showed significantly higher levels in the mean weight, BMI, triceps skinfold thickness and body fat percentage. Dietary intakes Dietary intakes were estimated by asking subjects to recall meals, snacks and beverage consumption over the previous 24 hours. Trained dietitians conducted these interviews. The NARs of protein, iron and vitamin A were significantly higher in HG when adjusted with age, sex and BMI. However sodium intakes estimated with 24hr recall method did not show any difference between HG and CG.. HG showed significantly increased intakes of soybeans, vegetables and fish and shellfish compared to those of CG.. When the vegetable intakes was classified into Kimchi ( pickled Korean vegetables seasoned with salt, hot red peppers and garlic), fresh vegetables and Namul (cooked vegetables seasoned with salt and some vegetable oil), the HG was observed as having higher intakes of Kimchi and Namul. The intakes of grains(≥311g/day)(odds ratio:2.393, CI:1.186-4.832 ), meats(≥106g/day) (odds ratio:2.210, CI:1.225-3.987) and fish and shellfish(≥102g/day) (odds ratio:2.756, CI:1.486-5.109) were also significantly associated with the higher relative risks of hypertension. 3. Biochemical parameters We observed significantly lower serum Ca, Mg and plasma rennin activity, whereas significantly elevated plasma aldosterone levels in HG. The proportion of the HG subjects with rennin activity less than 1ng/ml/hr was 42%, which means considerable proportion of them were salts sensitive. We observed significant inverse association between the serum Mg(<1.8mg/dl )and the risk of hypertension (odds ratio:1.904, CI:1.047-3.406 ) and positive association for serum TG(≥ 250mg/ml) (odds ratio:2.613, CI: 1.177-5.801 ) and for serum aldosterone level(≥16.0pg/ml)(odds ratio:3.488, CI:2.048-5.940) The HG showed significantly higher levels in serum TG, atherogenic index, sGOT and sGPT. Sodium intake with Kimchi contributed 37.2% of the sodium intake of the all subjects. Eighty-four percent of the variance in the blood pressure difference between HG and CG was explained with 12 dishes like the consumption of boiled salted mackerel, stirred fried anchovy, stuffed cucumber Kimchi, Bulgogi (seasoned and broiled meat), Kimchi stew, boiled seasoned fish, fish soup, Kimchi dipped in salted water, broiled fish, Ramyeon, dumpling, fermented salted fish. 3. Development of the Na index (Na index_(112), Na index_(55), Na index_(25), Na index_(14)) and evaluation of the validity. Na index was developed by giving the weighed score 4, 3, 2 or 1 to the each dish item according to the sodium content of the one serving size of it. Each score was multiplied by the frequency score mentioned in number 1. The score was summed for 112 dish items for Na index_(112). For Na index55 , 55 dish items were selected based on the extent of contribution (eg, content of sodium , eating frequency) to the sodium intake and the index score was calculated in the same way. Na index scores were also obtained in the same way with 25 dish items (Na index_(25)) or 14 dish items (Na index_(14)) The proportion of the subjects included in the same percentile of the sodium intake estimated with FFQ112 and Na index scores was 39.3∼50.3%. Fifty percent of the subjects included in the same percentile based on the distribution of sodium intake estimated with FFQ_(112) and Na index_(14). Characteristics of the dish consumption frequencies of the subjects included in the upper 25^(th) percentiles of the sodium intake(High Sodium Group: HSG), lower 25^(th) percentiles-upper 25^(th) percentiles (Medium Sodium Group: MSG) and lower 25^(th) percentiles(Low Sodium Intake: LSG) HSG showed higher frequencies in soups or thick soups like in Sujebi(hand made small flour dough soup), Gomtang(thick soup of meat and bone), fish egg soup, fish and shell fish soup, bean- sprout soup, Kimchi soup, fishes like broiled fish, fried fish, boiled salted mackerel, Kimchis like Korean cabbage Kimchi, cubed radish Kimchi, Kimchi dipped in salted water, stuffed cucumber Kimchi, meats like Bulgogi(seasoned and broiled beef), sliced boiled pork, and stored side dishes like stirred fried anchovy, fermented salted squid, roasted laver compared to LSG.. HSG also showed higher consumptions or high sodium intakes from the dishes mentioned above. Subjects were taking 31.2% of the daily sodium intakes with Kimchi, 20.3% with soup or stew, 12% with fish and 3.2% from fermented salted fish. When sensory test was done for the broiled salted fish or broiled unsalted fish dipped with the small amount of soybean sauce with some mustard, the preference scores for the taste, texture, and flavor were not significantly different. While the salt intake was 2.5g for the former fish and 0.8g for the latter fish.
