PURPOSE : Influenza pandemic, a world wide epidemic due to the emergence of novel influenza virus, can lead to high levels of illness, death and economic loss, since the general population have no immunity for the new virus. While currently WHO strongly advises each country to set up its own national preparedness plan, studies on past pandemic was rarely done in Korea except some historical studies. Also, modeling or forecasting studies has not been studied on next pandemic. The purposes of this study are to analyze the impact of past pandemic in 1918 in Korea and, in order to prepare for a future outbreak, to estimate the impact of next pandemic through mathematical modelling, which results could be used as a basic material for setting up a national preparedness plan in Korea. MATERIALS and METHODS: In order to estimate the impact of 1918 pandemic in Korea, the excess mortality, epidemic curve, and doctor visiting rate of patients with influenza symptoms were analyzed, based on the data in statistical almanac of governor-general of Chosun from 1915 to 1923. Also, in order to estimate the next pandemic impact, we used 'Influsim', a deterministic influenza modelling program, and calculated the estimated number of death, inpatients, and outpatients. We analyzed the effect of various preventive measures with no medication involved, including patients isolation, school closure, cancelling of mass gathering events and also the effect of a preventive measure with using anti-viral medication as well. RESULTS: The death toll of the pandemic influenza that swept across the world in 1918 is estimated 0.92%~1.25% of the Korean population. High occurrence of excess mortality was observed among the old age group. The pandemic influenza lasted from October, 1918 to January 1919, and reached its peak in november of 1918. Upon analyses of the data of 1918, the number of people who died from communicable diseases, influenza and respiratory illnesses showed a significant increase during the pandemic period, leading to a conclusion that the cause of the excess mortality was largely attributable to the influenza pandemic. The influenza pandemic in Korea in 1918 is assumed to have started from Cheollanam-Do, Chungcheongnam-Do and Kyeonggi-Do area, spreading out very fast and reaching its peak in November of the same year in every province. Upon analysis of the 'most likely' modeling which assumes the re-occurrence of a new pandemic influenza in Korea, the number of out-patient is estimated to be 12,113,634, the number of in-patient to be 1,237,525 person-day, and the number of deaths to reach 46,252 people. Further analysis is performed by assuming a new influenza pandemic which is as fatal as that in 1918 ('1918-like). In this analysis, the number of out-patients is estimated be 12,133,176, the number of in-patient to be 10,511,556 person-day and the number of deaths to be 263,268 people. In case of 'most-likely' modeling, which was selected for national preparedness, the outbreak is estimated to reach its peak on 79th day from the first exposure and on the peak day, the number of new patients is estimated to be 691,951. The surge, referring to the point when the number of patients hikes up explosively, is likely to begin on the 60th day from the first exposure. Assuming that everybody is susceptible because of the novel influenza virus, the susceptible-immune equilibrium is estimated to be reached the 100th day and the pandemic influenza is estimated to end after 75% of the population acquired immunity. However, if a new pandemic influenza is as virulent as the one in 1918 which was the extremely severe case, the number of deaths may grow to 260,000 and, therefore, we should seek measures in preparation of such outbreak. After reviewing various preventive measures for mitigation and blocking-off of pandemic surge, among the preventive measures excluding the use of anti-viral medication, the most effective method was analyzed to be isolation of patients. If 10% of moderate patients, 30% of severe patients and 90% of extremely severe patients can be isolated to block-off further transmission of the disease, it will have the effect of delaying the epidemic by 25 days, reducing 2,150,000 patients and 10,000 death tolls. While restriction of mass gathering or temporary school closure is not considered to be highly effective as an independent measure, they can be more effective when combined with other preventive measures. Use of anti-viral medication can be significantly effective in reducing the number of deaths but has only limited effectiveness in mitigating the pandemic itself. The most reasonable preventive measure is the combination of using anti-viral medication and other measures. CONCLUSION: In this study, we investigated the pandemic characteristics in 1918 in Korea and calculated the excess mortality caused by the influenza pandemic in 1918 to be between 143,787 and 203,170. Upon analysis of next pandemic modeling, the number of deaths is estimated to be more than 46,252 people in 3 month of flu-surge period. It is also estimated that practical mitigation of pandemic transmission can be achieved through the combination of using anti-viral medication and other non-medication related measures as well.
