This study was performed to investigate prevalence of salt sensitivity in Koreans, and the characteristics of salt sensitivity based on changes in blood pressure levels, anthropometric data, biochemical data, and urinary minerals after transition from a low salt to a high salt diet. The age range of the subjects(n=101) who volunteered to participate in this study was 18-65years. The participants of this study were divided into 2 groups. The hypertensive group(HG) had 31 hypertensive patients(18 men and 13 women) and the control group had 70 normotensive subjects(normotensive group(NG) 33 men and 37 women). The subjects of the HG and NG were fed diets containing 100mmol Na(low-Na diet) and 300mmol Na(high-Na diet) for 7days, respectively, during the 15-days hospital period. Chapter 1. Characteristics of salt-sensitivity and risk factors for salt-sensitivity in Koreans The mean age of patients in the HG was 57.7 years, and that of subjects in the NG was 40.8 years. The patients of HG were obese with high BMI and waist circumference. The subjects were classified as salt sensitive or salt resistant according to the changes in their blood pressure levels after transition from a low-salt to high-salt diet. The prevalence of salt sensitivity was 52% in the HG and 17% in the NG. The mean age of salt sensitive subjects(54.4 yeats) were older than that of salt resistant subjects(42.8years). On transition from a low-salt to a high-salt diet, the levels of SBP, DBP, and MAP increased significantly in the HG. However, no significant changes in blood pressure were observed in the NG. The salt sensitive subjects with normotension showed significantly increased levels of SBP, DBP, and MAP compared with salt resistant subjects at baseline and after high salt diet. Salt sensitivite subjects with hypertension resulted in a significant increase in mean PP and PPnight. Salt-sensitive individuals with hypertension had significantly decreased levels of hemoglobin, hematocrit, and fasting blood sugar, and reduced renin activity after transition from a low-salt to a high-salt diet. These results might be ascribed to increase of blood volume due to high-salt diet. Compared to salt-resistant subjects, salt-sensitive subjects had low renin activity on consumption of both low-salt and high-salt diets. Salt sensitivity with hypertension resulted in a significant increase in the 24-hour urine volume and the urinary levels of creatinine, Na, K, Na/K ratio, Cl, and Ca; salt-resistant subjects with hypertension had significantly increased urinary levels of Na, Na/K ratio, Cl, and Ca. After consuming the DASH diet, salt-sensitive individuals with hypertension showed a significant decrease in the levels of SBP, DBP, PP, and MAP, whereas salt-resistant individuals with hypertension showed no significant decrease in blood pressure levels. Salt sensitivity in normotensive subjects also resulted in a significant decrease in the levels of SBP, DBP, mean PP, and MAP. The HG showed a significant increase in the levels of total cholesterol and AI after the DASH diet. The NG showed a significant decrease in the levels of TG and HDL cholesterol, but the level of AI was found to be increased. Compared with the usual Korean diet, the DASH diet has more protein and fat and lower carbohydrate content. Therefore, it is expected that TG level would decrease and other lipids levels would increase by the DASH diet. After the DASH diet, hypertensive individuals showed a significant decrease in the 24-hour urine volume and the urinary levels of Na, Cl, and Ca. Thus we assumed that the decrease in Ca excretion would be achieved by reduction of sodium intake. Normotensive individuals with salt sensitivity showed a significant decrease in the 24-hour urine volume and in the urinary levels of Na, and Cl, along with a significant increase in the urinary levels of creatinine and K. This is partly ascribed to the altered nutritional composition, such as high levels of protein and K of the DASH diet. The level of lipoprotein (a) was increased in normotensive people with high LDL cholesterol after consuming a high-salt diet; this may increase the risk of cardiovascular disease. In conclusion, restriction of salt intake should be prescribed to people at a high risk of salt-sensitivity, irrespective of whether they are hypertensive or normotensive.
