본 연구는 정신사회재활모델을 바탕으로 정신장애인에게 필요한 정신사회재활 서비스를 사정하는데 사용할 수 있으면서, 사정된 결과를 바탕으로 정신장애인에게 제공된 재활서비스의 장·단기적 성과를 평가하는데도 일괄적으로 적용하여 활용할 수 있는 표준화된 척도를 개발하는데 그 목적이 있다. 이러한 목적을 달성하기 위하여 총 8단계의 과정을 통해 문항의 개발 및 요인구조에 대한 분석, 신뢰도 및 타당도의 검토를 실시하였다. 구체적인 각 단계별 내용과 결과는 다음과 같다. 1단계와 2단계에서는 정신사회재활모델에 근거한 이론적 검토와 전문가를 대상으로 한 조사의 결과를 통합하여 본 연구의 연구모형이 되는 총 4개의 하위 요인(임상관리, 기능, 역할수행, 자기옹호)과 12개의 세부구성개념(병식, 약물관리, 증상관리, 일상생활, 대인관계, 문제해결, 스트레스 대처, 직업, 사회적 역할, 가족내 역할, 낙인 및 차별대처, 임파워먼트)이 포함되는 척도의 이론적 구조모형을 설정하였다. 3단계에서는 이론적 구조모형을 바탕으로 150개의 예비문항을 개발하였고, 개발된 예비문항의 세부구성개념 반영도를 전문가와 정신장애인을 대상으로 조사하였다. 4단계에서는 3단계의 조사 자료를 바탕으로 6명의 전문가들과 함께 예비문항을 수정 및 삭제하여 최종예비문항으로 110문항이 선정하였다. 이상의 2~4단계의 과정을 통해 척도의 요인 구조와 문항의 내용타당도를 검증하였다. 5단계에서는 최종예비문항에 대하여 44명의 정신장애인을 대상으로 예비조사를 실시하여 설문지에 대한 의견을 확인하였다. 6단계에서는 연구의 목적을 이해하고 참여에 동의한 전국 55개 정신보건기관을 이용하는 정신장애인 1,033명의 자료를 수집하였다. 7단계에서는 수집된 자료를 무선 분할하여 탐색적 요인분석에 표본 758부와 확인적 요인분석에 표본 275부를 각각 분석함으로써 교차타당도를 검증하였다. 각 영역별로 문항분석 및 탐색적 요인분석과 확인적 요인분석을 각각 실시하여 문항과 요인을 구성하여 최종적으로 4개의 하위 영역과 10개의 세부구성개념의 구조를 가진 50문항의 척도가 개발되었다. 마지막 8단계에서는 개발된 척도에 대한 신뢰도와 타당도를 확인하였다. 먼저 척도의 신뢰도와 관련하여 내적 일관성에 대한 분석 결과, Cronbach α값이 .936으로 나타났고, 2주 간격의 검사-재검사의 상관계수가 .840(p<.001)으로 나타나 척도의 내적 일관성과 시간적 안정성이 우수한 것으로 볼 수 있다. 다음으로 수렴타당도의 경우에는 본 척도와 약물순응도, 병 대처, 기능평가, 취업동기, 가족관계, 임파워먼트와의 상관을 통해 검토하였는데, 척도의 각 하위영역과 준거변인간의 상관계수가 .516(p < .001) ~ .775(p < .001)와 같이 대체로 높은 것으로 나타나 수렴타당도가 확보되었음을 알 수 있다. 그리고 정신장애인과 일반인의 평균 점수를 비교한 결과, 전체 총합점수가 두 집단 간에 통계적으로 유의한 차이(t = -14.276, p < .001)가 있다는 결과를 보여주어 판별타당도가 확보되었다. 결론적으로 본 정신사회재활 서비스 사정 및 성과평가 척도는 정신장애인에게 필요한 통합정신사회재활 서비스를 각 하위 영역별로 신속하게 사정할 수 있도록 하고, 동시에 제공된 서비스의 단·장기적 성과를 포괄적으로 평가할 수 있도록 하는 신뢰도와 타당도가 검증된 표준화된 도구라고 볼 수 있다.

The purpose of this study was to develop a standardized scale that could not only be used for the assessment but also be used for evaluating short-term & long-term rehabilitation outcome for persons with psychiatric disabilities. In order to achieve this purpose, 8-step processes including item development, factor analysis, reliability and validity analysis and so on were systematically carried out. Step-by-step details for each stage and the results were summarized as follows. In step 1 and 2, this study developed a total of 4 domains (clinical management, function, role performance, self-efficacy) and 12 sub-factors (insight, medication management, symptom management, daily living, interpersonal relationship, problem solving, stress coping, job, social role, in family role, stigma and discrimination coping, empowerment) by integrating the theoretical reviews derived from the psychosocial rehabilitation model and the results of survey from experts. In step 3, 150 preliminary items were developed based on the theoretical structure model and the developed items were administered to the experts and persons with psychiatric disabilities in order to check the appropriateness of each item. In step 4, based on survey results of step 3, the 110 preliminary items were chosen through item deletion and modification with thorough consultation from six experts in mental health field. Through these processes from step 2-4, content validity the scale was examined and verified. In step 5, a pilot survey was conducted with 44 mentally ill persons and a final questionnaire was developed by incorporating comments from the subjects. In step 6, the data were collected from 1,033 people with psychiatric disabilities who understood the purpose of study and agreed to participate in the survey. The subjects were receiving psychiatric services from 55 mental health agencies across the country. In step 7, after data collection, the subjects were randomly divided into two groups. One group consisting of 758 subjects was utilized for exploratory factor analysis and the other group consisting of 275 subjects was used for confirmatory factor analysis. The final scale of 50-items with a structure of 4 domains and 10 sub-factors was developed by item analysis and exploratory and confirmatory factor analysis. In the last step, the developed scale was tested for its reliability and validity. First, the scale’s internal consistency (checked using Cronbach’s alpha) was .936 and the correlation coefficient of 2-weeks interval test-retest reliability was .840. Therefore it was reasonable to assert that the scale had an excellent internal consistency and reasonable timing stability. Second, for the convergent validity, the correlation analysis was performed with the theoretically related variables such as medication compliance, symptom coping, functional assessment, job motivation, family relationship, and empowerment. The results showed positive and significant relationship(r = .516 ~ .775, p < .001) with each variable examined. Finally, in order to test the discriminant validity, the average score between person with psychiatric disabilities group and normal group was compared and the result showed that the total score was significantly different (t = -14.276, p < .001) between the two groups. All of these analyses were taken as evidences of the scale’s construct validity. In conclusion, the developed scale demonstrated outstanding reliability and validity. The scale could be used as a quick and easy assessment tool for people with psychiatric disability. At the same time, the developed measure can also be utilized to comprehensively evaluate short and long-term outcomes of the provided services.
