This is a quasi-experimental study using non-equivalent control group pretest-posttest design to see the effects of kangaroo care(KC) and voice intervention(VI) on preterm infants and their mothers.
This study was conducted with preterm infants hospitalized in NICU at E University Hospital and their mothers from June 2012 to April 2013. A total of 68 subjects were examined: 22 for KC+VI group to which kangaroo care and voice intervention were applied, 20 for KC group to which kangaroo care was applied, and 26 for the control group. To prevent the spread of the research, data were first collected for control group, followed by KC group and KC+VI group.
In this study, kangaroo care was employed in reference to the Kangaroo care guideline suggested by WHO (2003), and mothers were asked to read the children's book, I Love You Through And Through slowly to infants three times and sing Mozart's lullaby three times. A total of three sessions were conducted every two days (Day 2, 4, and 6), 30 minutes per session, for a week. Infants' physiological reactions (HR, RR, SpO2, SCRIP) and behavioral state were measured on the day before starting the research, at the start of each intervention, 15 minutes after its start, and at the end of each intervention (30 minutes later), and the sleep/activity pattern were recorded for a week from the day of starting intervention on. Mothers were asked to personally report their stress, self-esteem, postpartum depression, anxiety, and maternal attachment before starting the research and on the day when the research was completed.
Data analysis was performed by using an SPSS 20.0 program. χ²-test and ANOVA were used to test homogeneity in the general characteristics and dependent variables of preterm infants and their mothers. Three groups of infants had physiological variables (HR, RR, SpO2, SCRIP) and the behavioral state analyzed by ANCOVA and repeated measures ANCOVA and the sleep/activity pattern by ANCOVA, and mothers had their stress, self-esteem, postpartum depression, anxiety, and maternal attachment analyzed by ANCOVA before and after treatment, followed by Tukey's test as posttest.
The main results of this study are as follows:
1) KC+VI group and KC group had infants' HR (F=3.242, p=.028) decreased significantly after kangaroo care and voice intervention as compared with control group, with no difference in RR or SpO2 and the significant increase in SCRIP (F=5.254, p=.008).
2) KC+VI group and KC group had infants' behavioral state decreased significantly as compared with control group(F=51.128, p<.001).
3) While there was no difference in such aspects of the sleep/activity pattern as length of sleeping and length of crying among three groups, KC group showed differences in length of awakening at night as compared with KC+VI group and control group (F=4.054, p=.022).
4) KC+VI group and KC group had mother's stress decreased significantly as compared with control group (F=3.357, p=.041).
5) KC+VI group and KC group had mother's self-esteem increased significantly as compared with control group (F=3.925, p=.025).
6) KC+VI group had mother's postpartum depression decreased significantly as compared with KC group and control group (F=5.987, p=.004).
7) KC+VI group had mother's anxiety decreased significantly as compared with KC group and control group (F=4.472, p=.015).
8) There was no difference in mother's maternal attachment among three groups.
To put the results together, kangaroo care and voice intervention were effective in giving physiological stabilization to preterm infants and stabilizing their behavioral state and in reducing mother's self-esteem, postpartum depression, and anxiety. As compared with more than 10 sessions over two to three weeks in the existing research, this study could enable nurses to reduce their workload by employing kangaroo care and voice intervention for a shorter period of time―a total of three sessions over a week―and demonstrated that it was effective nursing intervention for mothers and infants alike since it could also be applied easily at home by mothers after leaving hospital.
