Purpose: This study is a descriptive survey research that aims to provide basic data for efficient human resource management that can contribute to reducing turnover rate among nurses, by identifying the impact of sleep disorder and job stress on the turnover intention of shift working nurses and understanding the key factors influencing their turnover intention.
Method: The subjects of this research were 188 nurses working 8-hour shifts in J-city, G-city and N-city (all in J-province), of which 13 respondents who produced unreliable answers were excluded from the analysis, making the total number of valid questionnaire 175. Upon gaining the IRB approval, data was collected between Sep. 1 and 15, 2015 using a structured questionnaire, and the participants were asked to sign a consent form prior to completing the survey. The research tool contained 62 questions, of which 10 were about general characteristics, 15 about sleep disorder, 27 about job stress, and the remaining 10 the nurse’s intent to turnover. Collected data were analyzed with a SPSS 12.0 program using descriptive statistics, ANOVA, correlation and stepwise multiple regression.
Results: The results can be summarized as follows:
1. Female accounted for 96% of the respondents, while the largest age group was those aged 30 or under at 38.9%. By category, about 65% said they had less than 5 year experience in clinical career, 23.4% were working in division of internal medicine, and 49.1% were college graduates. Monthly payroll was between 2-2.5 million KRW for 55.4%, more than half were married at 53.1%, and on skill level, 42.9% answered they were “familiar with” their work. Majority(54.9%) of the subjects were sleeping 7-8 hours per day, but 2.9% reported they were “rarely sleeping.” The lion’s share of subjects worked night shifts, amounting to 72.6%.
2. Total sleep disorder among the subjects was 31.83±7.66. Total job stress was 66.68±8.36. Under the domain of job stress, the highest mean score was 12.17±3.12 for job demand, while the lowest was 4.78±1.27 for job insecurity. Total turnover intention was 37.35±6.64.
3. Analysis of sleep disorder according to general characteristics revealed that a significant difference was found in the group with 5 to 10 years of clinical experience (F=3,124, p=.016) and in the group reporting they “rarely sleep” (F=4.313, p=.002). In job stress analysis, significant difference was indicated in the group of nurses working in ICU, ER (F=4.054 p=.004), who had the highest level of stress. No significant difference existed in turnover intention.
4. As for correlation between domains, sleep disorder and turnover intention had positive correlation (r=.365, p〈.001). Between job stress and turnover intention, the total job stress was positively correlated with turnover intention (r=.501, p〈.001), and in the subdomain, physical environment (r=.320,p〈.001), job demand (r=.243,p〈.05), relationship conflict (r=.178, p〈.05), job insecurity (r=.374, p〈.001), organizational structure (r=.383, p〈.001), inadequate compensation (r=.352, p〈.001) and organizational culture (r=.443, p〈.001) had positive correlation.
As for correlation between sleep disorder and job stress, total job stress had positive correlation with sleep disorder (r=.403, p〈.001). In the subdomain, physical environment (r=.313,p〈.001), job demand (r=.205, p〈.001), lack of job autonomy (r=.151,p〈.05), organizational structure (r=.289, p〈.001), inadequate compensation (r=.507, p〈.001) and organizational culture (r=.221, p〈.001) had positive correlation.
5. Variables influencing turnover intention among the subjects were; the more clinically experienced (β=.304), and in regards to job stress, the less adapted to workplace climate (β=.243), the lower the level of payment (β=-.237) and the more serious the sleep disorder (β=.211).
Conclusion: In order to reduce turnover rate among shift-working nurses, it is recommended that improvements be made to work patterns to better cater to clinical experience of individual nurses and an adequate compensation plan developed to reward low-paid new nurses based on job performance, while a consistent training program should be provided to create an environment where nurses can take pride and stay positively and creatively motivated. Furthermore, shiftwork conditions and organizational climate should be improved to reduce job stress factors and help nurses easily adapt to workplace culture. Additionally, introducing short naps should be considered to help overcome the effects of sleep disruption caused by 8-hour shift, while allowing flexibility to choose shift hours and maintain a consistent shift schedule will increase job satisfaction and contribute to improving the quality of sleep by creating a more stable working condition. The findings of this research is expected to provide basic insight for efficient human resource management to hospitals, which can be achieved by improving the quality of sleep, reducing job stress and thereby minimizing turnover intention among the shift-working nurse population.
