본 연구는 골반 림프절 절제술 및 대동맥주위 림프절을 시행한 부인암 환자들이 경험하는 하지 림프부종과 삶의 질의 관계를 확인하여 부인암 환자의 삶의 질 향상을 위한 교육 프로그램의 기초자료를 제공하고자 한다. 연구대상은 A 종합병원에서 골반 림프절 절제술 및 대동맥주위 림프절 절제술을 포함한 부인암 수술을 받은 143명의 환자를 대상으로 2015년 3월 19일로부터 2016년 2월 24일까지 서술적 조사연구로 시행되었다. 조사도구는 Lim (2014)의 Korean Version of the Gynecologic CancerLymphedema Questionnaire와 암환자 삶의 질 도구인 EORTC QLQ C30 (1986)을 이용하였다. 수집된 자료는 SPSS 20.0 통계 프로그램을 이용하였다.
연구 결과는 다음과 같다. 1) 골반 림프절 절제술 및 대동맥주위 림프절 절제술을 시행한 부인암 환자의 하지 림프부종은 평균 4.31±3.86점이었고 대상자의 55명(38.5%)이 하지 림프부종이 있었다. 하지 림프부종이 있는 환자 군의 평균은 8.38±2.92점이었고, 하지 림프부종이 없는 환자 군의 평균은 1.76±1.41점이었다. 두 군간의 하지 림프부종은 유의한 차이가 있었다(t=-15.67,p<.001). 2) 일반적 특성 및 임상적 특성에 따른 하지 림프부종을 확인하였을 때 유의한 차이를 보인 변수는 절제된 림프절 개수로 하지 림프부종이 없는 군은 평균 39.47±22.54개, 있는 군은 평균 47.31±23.38개가 절제되었고, 두 군간에는 유의한 차이가 있었다(t=-1.99, p=.049). 3) 하지 림프부종이 있는 환자 군이 없는 환자 군보다 기능척도 삶의 질에서는 모든 기능에서 삶의 질 수준이 낮은 것을 확인하였고, 증상척도 삶의 질에서는 식욕부진, 경제적 어려움 항목을 제외한 나머지 영역에서 모두 낮은 삶의 질을 나타냈다. 전반적인 건강수준 삶의 질 또한 하지 림프부종이 있는 환자 군에서 삶의 질 수준이 낮게 측정되었다(Table 6). 4) 하지 림프부종 유무에 따라 두 군으로 나누어 하지 림프부종 점수와 삶의 질의 상관관계를 확인하였을 때, 하지 림프부종이 있는 군에서는 하지 림프부종 점수와 신체적 기능 (r=-.42, p=.001)은 부적 상관관계, 오심과 구토(r=.32, p=.019), 통증(r=.32, p=.019)과는 정적 상관관계를 확인하였다. 하지 림프부종이 없는 군에서는 하지 림프부종 점수와 신체기능(r=-.36, p=.001), 역할기능(r=-.39, p<.001), 인지 기능(r=-.38, p<.001), 사회적 기능(r=-.22, p=.043)은 부적 상관관계를 나타냈다. 또한 호흡곤란(r=.30, p=.004), 경제적 어려움(r=.24, p=.022)은 정적 상관관계를 나타냈으며 전반적인 건강수준(r=-.34, p=.001)과는 부적 상관관계를 나타냈다. 5) 일반적 특성 및 임상적 특성 중 삶의 질 점수에 유의한 차이를 보이는 변수는 연령, 교육 수준, 경제적 수준, 재발 유무, 림프절 전이유무, 항암치료 유무, 누적된 항암치료 횟수, 방사선 치료 유무, 동반 질환 유무 등이었다.
이상의 결과를 통해, 골반 림프절 절제술 및 대동맥주위 림프절 절제술을 시행 받은 부인암 환자는 높은 하지 림프부종 발생률을 나타냈다. 하지 림프부종이 있는 부인암 환자의 삶의 질은 하지 림프부종이 없는 환자에 비해 낮은 삶의 질을 보고하였다. 하지 림프부종은 지속적인 관리가 필요한 만성질환으로 이를 예방하기 위한 교육 프로그램이 필요하다.

The purpose of this study was to identify the lower limb lymphedema and quality of life in gynecologic cancer patients who have received pelvic lymphadenectomy and/or para-aortic lymphadenectomy. It will provide useful information in developing an education program in daily life for patients with lower limb lymphedema. The study design was a descriptive study. The subjects of the study were 143 patients patients who have received pelvic lymphadenectomy and/or para-aortic lymphadenectomy. The data was collected from March 19th, 2015 to February 24th, 2016. The study was performed using the Korean Version of the Gynecologic Cancer Lymphedema Questionnaire (GCLQ-K) for lower limb lymphedema by Lim (2014). Quality of life scale was European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 (EORCT QLQ-C30) that have been developed by researchers at the international quality of life center (1986). The data were analyzed by using the SPSS Wins 20.0 program.
The results of the study are summarized below. 1. The mean & standard deviation score in all subjects was 4.31±3.86 and 55 (38.5%) of participants was experienced lower limb lymphedema. The mean & standard deviation of the total GCLQ-K score was higher in the LLL (lower limb lymphedema) group than No LLL group (mean ± standard deviation, 8.38±2.92 vs 1.76±1.41; t=-15.67, p<.001). 2. The lower limb lymphedema by general & clinical characteristics was not significantly different between LLL group, and No LLL group except on number of removed lymphnodes. The number of removed nodes was higher in LLL group than No LLL group (mean ± standard deviation, 47.31±23.38 vs 39.47±22.54; t=-1.99, p=.049). 3. All Functional Scale score was lower in the LLL group than No LLL, and all Symptom Scale score except on appetite loss and Financial difficulties was higher in LLL group than No LLL. Global Health status score was lower in LLL group than No LLL group (Table 6). 4. There was a statistically significant correlation between the lower limb lymphedema and physical functioning (r=-.42, p=.001), nausea & vomiting (r=.32, p=.019) and pain (r=.32, p=.019) in LLL group. also, there was a statistically significant correlation between the lower limb lymphedema and physical functioning (r=-.36, p=.001), role functioning (r=-.39, p<.001), cognitive functioning (r=-.38, p<.001), social functioning (r=-.22, p=.043), dyspnea (r=.30, p=.004), financial difficulties (r=.24, p=.022) and global health status (r=-.34, p=.001) in No LLL group. 5. The quality of life scores according to the age, education status, economic status, cancer recurrence, lymphnode metastasis, chemotherapy experience, number of the cumulative chemotherapy, radiotherapy experience, comorbidity were showed significantly different between LLL group and No LLL group(Appendix 1,2).
The above results show that in lower limb lymphedema patient after pelvic & para-aortic lymphadenectomy was a high incidence. Quality of life is lower in LLL group than No LLL group. To prevent lower limb lymphedema as a chronic disease which is needed continuous management, an education program is needed.
