음악심리치료에서 치료적 관계형성 및 정서경험하기: 자폐스펙트럼장애 사례연구 Formation of Therapeutic Relationship and Experiencing Emotion in Music Psychotherapy: A Case Study of a Person with Autism Spectrum Disorder원문보기

Studies have been conducted using pantheoretical perspectives on the importance of building therapeutic relationships and emotional experiences which are crucial to the treatment result. So far, studies have been mostly focused on verifications of therapeutic intervention. Recently however, studies are increasingly being conducted not only on the intervention of the therapists in the therapeutic process, but also on the determination of the clients’ active roles and their personal experience related to the treatment. Music psychotherapy refers to the psychotherapy in which musical elements are used in conjunction to literal elements of traditional psychotherapy. This study focuses on the application of music psychotherapy which emphasizes therapeutic relationship formating and music experience. Various music exercises including improvisational experiences, re-creative experiences, composing experiences and receptive experiences on the basis of cooperating, and mutual complementary relationship between the therapists and the clients, enables the clients to experience their emotion in the safe environment. This Study focuses on a person with Autism Spectrum Disorder (ASD) who is unable to verbally communicate, and is intended to teach the person with ASD how to build therapeutic relationships by creating shared experiences and emotion. Music psychotherapy was conducted throughout 46 sessions from October 2002 to August 2004. Free improvisation, thematic improvisation, singing, and music imaging were used throughout the whole process. After each session, the client’s intentions were interpreted via the client’s facilitated communication (FC) and E-mails. Next, the following treatment plan was examined and established with the client through social story methods. Since clear communication with the ASD client is difficult to achieve, music therapy assessment and case studies were conducted in both pre/post-sessions. The data analysis included the client’s treatment progress record, therapist's personal journal, songs, E-mails that were exchanged during the process, and other results like photographs, poems, tape recordings, etc. Nordoff-Robins music therapy analysis was used in conjunction with audio tapes of the client and his treatment progress record. Also, indirect experiences were provided through descriptive case analysis and pilot study among other case study recording methods, and an in-depth analysis is meant to clarify inevident factors of the process. Finally, in-depth conclusions were drawn through themed analysis. The results and conclusions of the study are as follows: First, the integrated use of music experience, suggested by music psychotherapy, encouraged formating therapeutic relationships and the experiencing emotions. Concerning the music therapy assessment, the client showed improvement in instrument play, music concept recognition and expression, and music learning skills (excluding singing because of the client’s inability to verbally communicate). In addition to the musical improvement, the client showed improvement in behavioral and emotional expression, sensory and motor, imitation and imagination, communication skills, and social interaction. Second, using language and music throughout the whole process was adequate to form a therapeutic relationship with the client who was incapable of verbal communication. Initially, the treatment was led by the client’s non-verbal music experience. However, additional music experience and co-writing poems gradually extended communication and social interaction. Halfway through the therapy process, by the client expressing and sharing emotion, the goal of bidirectional therapeutic improvement was achieved. That is, music as a form of communication enabled the integration of a therapeutic relationship, music and meaning. Third, the client was able to recognize and express emotion through music which emphasizes on therapeutic relationship formating and music experiences. The therapist as a cooperative supporter witnessed emotions being controlled, which previously had been avoided. The meaning of the client’s emotion, new hope and meaning could be expressed by listening to music and song writing. Fourth, despite the incapability of verbal communication, the client was able to naturally move from a music-centered process to a verbal-centered process. This result was revealed by the content change of the client’s lyrics and the interview with the client on music psychotherapy. This suggests the important point that there is no difference with the emotional experience in verbal psychotherapy. In this study, the researcher used music and language as media in the integral context, not being attached to any specific theory or method of music psychotherapy. For the in-depth analysis, we applied this to the person with ASD who has difficulty communicating. This reason, even if it is a single case study, it is suggesting an important point for following research for the clients who are in need of media using psychotherapy. Still, more on multi-study is required, since ASD clients show various clinical characteristics different from one another. Also, more study in extension is needed to reveal the common factors and the differences between verbal psychotherapy and music psychotherapy.
