It is a crucial time to arm adolescents with accurate and reliable health information so they can develop lifelong healthy behavior. But no practical method for measuring heath literacy of adolescent exists in Korea. This study aimed to identify the components and to develop a valid and reliable scale of Korean Adolescent Health Literacy Scale (KHLS-Teen). This Instrument is an objective health literacy instrument for adolescents. Method : Scale development consisted of two phases, generation of an item pool, and validation of scale which were carried out in 2016. In the phase of generating an item pool, It were performed a literature review, analysing health textbook and national educational curriculum, and focus group interview to extract the health literacy components of korean adolescents. Through these processes, initial items were generated. After consultation with experts panel(n=6) and assessing face validity with adolescents(n=20) to evaluate content validity carried out, a trial version of the test was administered to 200 adolescents in school. In the phase of validating scale, 735 adolescents aged 12∼18 participated the study. Result: It is defined that Adolescent health literacy is 'the competencies that adolescents understand and apply the health information to make a decision related to health promotion and disease prevention in everyday life'. The assessment was designed in relation to 3 dimensions: health demand, competency, skill. Health demand contains health promotion and disease prevention, competency involves understanding and applying of the health informations. Skill dimension includes prose, document, numeracy. An item pool made up of 38 items was generated at the developing scale stage. After content validity was considered, this decreased to 23 items. 200 adolescents of K7∼K12 grade participated in preliminary test. Reliability has been approved by KR-20 = 0.77. Based on the psychometric properties, the item pool were reduced to 16 items by removing items that performed less well. Specifically, items were candidates for removal if they had low item-total correlations(≦.20) and high Cronbach alpha if item deleted. In the validating scale stage, adolescents participated the study with a mean age of 15.9 years. As the response were dichotomous data, Item analysis by item response theory (IRT) was performed. Rasch analysis using jMetrik program showed item fit statistics(infit 0.83∼1.16), Item characteristic curve and Item information function were appropriate. Concurrent validation as criterion validation was approved through correlation with NVS(r=.610, p<.001). Internal consistency reliability, KR-20 was 0.75. Receiver Operating Characteristics was used to classify adolescents as having adequate or inadequate health literacy by cut off score 10.5. The final questionnaire was approved with 16 items required 6.33± 3.78 minutes. Conclusion: Korean Adolescent Health Literacy Assessment (KHLS-Teen) is a valid and reliable tool for the measurement of the health literacy of adolescents and can be used to evaluate different levels in adolescents in Korea and identify adolescents with inadequate health literacy.
