Well-dying means comfortable death, good death, happy death, and quality death and its culture has spread as many people have begun to think that quality of death is as important as quality of life. To respect patients' rights to make decisions for themselves, the Life-sustaining Treatment Determination Act (concerning life-sustaining treatment decisions for patients in hospice care or dying process) has been legislated for dying patients to decide whether to stop meaningless life-sustaining treatment. The act will be effective from February 2018 after three months of pilot period. This study was conducted to survey local community seniors' attitude toward and preference of advance directives to check how ready Korean seniors are to accept the advance directives and provide basic resources to systemize and spread the advance directives. The subjects were 200 65-year-old or older seniors using one senior college, one welfare center, and five senior centers in Seoul and they were surveyed from August 4, 2017 through September 4, 2017. Among the 192 subjects who agreed to participate in the survey, 12 quit before completion and 180 completed the survey. For the study tool, the Korean version of the 13 questions developed by Akabayashi, Slingsby & Kai (2003) on the attitude toward and preference of advance directives and translated into Korean by Eun Sook Yoon (2009) was used. The data were processed on the SPSS WIN 21.0 program to find the frequency, percentage, mean, and standard deviation and analyzed using χ² and Fisher's exact test.
The following are the findings:
1. The subjects responded as follows in regards to the advance directives: 39.4% said they have heard of the advance directives and 6.7% have written the advance directives. Also, 86.7% agreed to the advance directives and 68.5% responded that it is because they ‘do not want leave the decision-making a burden for the family.’ Among those who were against it, 63.6% responded that ‘the family will decide what to do in such a case.’ The information to include in the advance directives was mostly about ‘whether to receive life-sustaining treatment at the terminal stage of disease and most of them preferred general directives, such as pain control or rejection to CPR. They wanted the advance directives to be followed as much as possible and 53.2% preferred documentation of the directives. In regards to the consent to life-sustaining treatment, 83.9% subjects said they would want to suppress pain and reject life-sustaining treatment and 80.5% wanted to be informed about the symptoms and prognosis. Also, 70.6% said they want to write the advance directives. 2. Considering the subjects' general demographics, the perception of advance directives was statistically significantly different according to age (χ2=7.814, p=.020), education (χ2= 17.093, p=.002), and monthly income (χ2=10.015, p=.040). 3. Considering the subjects' general demographics, the consent to advance directives was statistically significantly different according to religion (χ2 =8.361, p=.039), marital status (χ2=14.029, p=.003), monthly income (χ2= 9.303 p=.010), co-residence with family (χ2=13.466, p=.004), and final decision-maker (χ2=8.906, p=.031). 4. Considering the subjects' general demographics, the determination to write the advance directives was statistically significantly different according to religion (χ2=14.218, p=.027) and subjective condition of health (χ2=1.014, p=.040).
In sum, the local community seniors' perception of advance directives was low, but their preference and determination to write it were high. The term 'advance directives' is still unfamiliar to both the general public and the medical practitioners in Korea. Therefore, it is necessary to actively train people and promote the concept to spread the use of advance directives. Also, seniors should receive personalized training, individual counseling, and various learning opportunities so they can have accurate knowledge about the advance directives and make decisions about their condition and treatment close to death. The attitude toward and preference of advance directives according to the general demographics were different from the results of preceding studies, but it was because there were not many studies in Korea to compare and the preceding studies of other countries showed different results because of difference in viewpoints and culture. It is necessary to repeat the study to identify the related variables. This study discussed the seniors' attitude toward and preference of advance directives to provide basic resources for systemized and active use of advance directives.
