The purpose of this study was to investigate the factors influencing the terminal care performance of nurses working in hospice specialized agency unit. Data were collected from 205 hospice nurses working at 45 different hospice specialist institutions using the self-reported questionnaires from July 21 to October 13, 2017. The demographic characteristics, death anxiety, hospice knowledge, terminal care attitude, empathy and terminal care performance were analyzed by the descriptive statistical analysis. The difference of terminal care performance by demographic characteristics was analyzed by the t-test, and ANOVA with Scheffe post hoc, and the correlation between variables was analyzed by Pearson Correlation Coefficient.
The results of this study are as follows.
1. The average death anxiety score of the participants was 2.48 out of 4 points. Sub-factor scores were: self present anxiety (2.68), death anxiety of others (2.63), self death anxiety (2.41) and present anxiety of others(2.28). 2. The average hospice knowledge score of the participants was 12.31 out of 20 points, with range from 7 to 17. 3. The average terminal care attitude score of the participants was 3.13 out of 4 points. 4. The average empathy score of the participants was 3.72 out of 5 points. Sub-factor scores were: emotional empathy (3.81) and cognitive empathy (3.54). 5. The average terminal care performance score of the participants was 3.02 out of 4 points. Sub-factor scores were as following: the psychological nursing performance was the highest at 3.22 and spiritual nursing performance was the lowest at 2.68.
6. There was a significantly positive correlation among terminal care attitude (r=.47, p<.001), empathy (r=.39, p<.001) and terminal care performance.
7. According to hierarchical regression analysis, significant predictors for the terminal care performance were terminal care attitude (β=.29, p=<.001) and empathy (β=.17, p=.012). In addition, among the general characteristics of the participants, age (over 50, β=.20, p=.004) influenced the terminal care performance. These factors explained 31.6% of the variance in terminal care performance of nurses
In conclusion, There was a significantly positive correlation among terminal care attitude, empathy and terminal care performance and nurses' terminal care attitude and empathy were the factors influencing terminal care performance. The results of this study suggest that terminal care performance of nurses can be strengthened by improving terminal care attitude and empathy. Therefore based on this study, we propose a study to develop a program that can improve the terminal care attitude and empathy in order to enhance the terminal care performance.
