The purpose of this study was to investigate the effects of upper thoracic auto-mobilization training on forward head posture by measuring and analyzing cervical spine alignment, cervical range of motion, and muscle activity. This study selected 33 people suffering from forward head posture. The subjects were divided into three equal groups: the thoracic-cervical auto-mobilization group (EGⅠ), cervical auto-mobilization group (EGⅡ), and control group (CG). The eleven members of EGⅠ engaged in auto-mobilization training of the thoracic and cervical spine. The participants in EGⅡ took part in auto-mobilization training of the cervical spine only, while the members of the CG were prescribed strengthening exercise of the neck and shoulder muscles. In order to examine the effects of the intervention, absolute rotation angle (ARA); anterior weight bearing (AWB); range of flexion, extension, and rotation motion; and muscle activity (EMG) tests were conducted to compare pre and post intervention scores. The ARA and AWB were used to measure the effects of the training through radiodiagnosis, cervical range of motion (CROM) was employed to assess range of motion of the neck, and EMG MP35 system was used to measure muscle activity. The results revealed that both the experimental groups and the control group had significant within-group changes in cervical spine alignment, range of motion, and muscle activity of the neck and shoulder muscles. EGⅠ demonstrated a more substantial effect on cervical spine alignment compared to EGⅡ and the CG. In the case of flexion angle, EGⅠ and EGⅡ had a noteworthy between-group increase compared to the CG. For the extension angle, EGⅡ showed a significant between-group increase compared to EGⅠ and the CG. As for the change of muscle activity in the muscles around the neck and shoulders, EGⅠ showed a significant between-group difference in the upper trapezius, splenius capitis, and serratus anterior compared to EGⅡ and the CG. No substantial between-group difference was observed in the sternocleidomastoid muscle and lower trapezius. In summary, self-mobilization training of the spine is effective in improving forward head posture by changing cervical spine alignment and muscle activity around the neck and shoulders. Also, indirect, rather than direct, treatment on the cervical spine can be more helpful in such cases. Therefore, we suggest self-mobilization training of the spine as an effective therapy to ameliorate the posture of subjects with forward head posture.
