본 연구의 목적은 복합외상 학대피해 아동의 심리사회적 적응을 위한 외상중심 인지행동치료(Trauma Focused–Cognitive Behavioral Therapy: TF-CBT) 기반 집단상담 프로그램을 개발하고 그 효과를 검증하는데 있다. 프로그램 개발은 프로그램 개발 연구모형을 기본으로 하여 계획수립, 프로그램 구성, 프로그램 개발, 전문가 타당도 검증, 프로그램 실시, 프로그램 평가 총 6단계로 진행하였다. 프로그램 계획단계에서 외상(트라우마), 외상 후 스트레스 장애(PTSD), PTSD 감소 프로그램, 인지행동치료(Cognitive Behavioral Therapy: CBT), 외상중심 인지행동치료(TF-CBT), 근거기반 트라우마 심리치료, 외상 후 성장(posttraumatic growth: PTG)과 그 밖의 국내외 문헌을 고찰하여 프로그램 개발의 토대를 구성하였다. 그리고 학대피해 아동들의 개인 면담 자료를 근거로 구성된 프로그램에 대해 전문가 자문과 평가를 통해 수정, 보완하여 최종적으로 외상중심 인지행동치료(TF-CBT) 기반 집단상담 프로그램을 구성하였다. 집단상담 프로그램의 구성요인은 다음과 같다. 첫째, 학대 외상으로 인한 과각성, 재경험, 회피 등 복합외상 후 스트레스 장애(Complex Posttraumatic Stress Disorder: CPTSD) 증상 감소를 위해 트라우마 심리교육, 마음챙김(Mindfulness), 이완 기법, 정서표현 및 정서조절을 활용하여 일상생활 속 각성 조절과 스트레스 관리에 중점을 두는 안정화 작업과 자원강화에 중점을 두었다. 둘째, 복합외상 학대피해 아동들이 갖고 있는 역기능적인 자기 지각 변인들에 대한 자동적 사고를 찾아 대안적 사고로 수정하는 인지적 재구성 요인을 구성하였다. 셋째, 외상 후 성장과 기억처리를 위한 의도적 반추 과정으로 인생선 작업, 인생 이야기, 가족 이야기, 트라우마 내러티브를 활용한 인지적 재구성 요인을 구성하였다. 넷째, 사회적 기능향상을 위해 대인관계 어려움을 보이는 학대 트라우마 아동들의 특성을 고려하여 효과적인 대인관계 기술, 의사소통 기술, 나-전달법 등을 활용한 사회기술 훈련을 구성요인으로 하였다. 구성된 집단상담 프로그램은 효과검증을 위해 실험집단과 통제집단을 나누어 프로그램을 실시하였다. 본 연구의 참여자 선정은 아동학대로 신고되어 특례법과 아동복지법에 의거 아동보호시설에 입소한 초등학교 4학년부터 6학년까지 남자 아동을 대상으로 모집 광고를 통해 20명 모집하였다. 그리고 안전한 연구 진행을 위해 아동보호시설 심리상담사에게 개인 상담을 받고 있는 모집 된 아동 중 내·외적 자원과 안정화 능력이 있는지 검정을 거쳐 16명 선정하였다. 연구에 선별된 아동들은 외상중심 인지행동치료(TF-CBT) 기반 집단상담 프로그램인 실험집단 8명과 통제집단 8명에 무선 배정되었으며 실험집단은 주 1회 혹은 2회, 회기당 60분씩 총 13회기를 실시하였다. 집단상담 프로그램 전후 변화를 측정하기 위해 한국판 복합외상 질문지, 인지적 변인(자기체계 척도, 자아존중감 척도), 정서적 변인(아동용 정서조절 척도), 행동적 변인(대인관계 변화 척도, 공격성 척도), 한국판 외상 후 성장 척도를 사전-사후-추후에 측정하였다. 자료 분석은 SPSS 27.0 통계 프로그램을 사용하였으며 실험집단과 통제집단 각 집단별 인원의 크기가 8명 이하이므로 비모수 검증(Mann-Whitney U test, Wilcoxon 부호순위 검정)을 통해 분석하였다. 그리고 아동의 변화에 대해 더욱 심층적이고 풍부한 해석을 위해 매회기 경험보고서, 회기 내 위크시트, 참여관찰, 만족도 등을 활용한 내용분석의 질적 연구를 병행하였다. 구체적인 연구 결과는 다음과 같다. 첫째, 집단상담 프로그램에 참여한 실험집단은 복합외상 후 스트레스 장애(CPTSD) 증상이 유의미하게 감소하는 것으로 나타났고, 프로그램 종료 4주 후 추후까지 효과성이 유지되며 감소하였다. 다양한 마음챙김 이완 기법과 인지적 지각은 자기통제력 향상을 가져와 일상생활 속의 각성 수준 조절뿐 아니라 재경험, 회피, 위협감, 정서조절 곤란 증상의 완화를 도운 것으로 보인다. 둘째, 실험집단은 자아 체계와 자아존중감이 사후와 추후까지 유의미한 변화를 보이는 것으로 나타났다. 이는 인지적 재구조화로 긍정적인 사고의 변화를 가져오고, 집단 역동을 통한 또래 아동의 공감, 격려, 지지라는 내적 자원의 요인이 효과를 보였을 것으로 보인다. 셋째, 실험집단은 정서조절의 변화에서 사후와 추후가 사전보다 유의미하게 높았으며, 프로그램 종료 4주 후까지 효과성이 유지되며, 정서조절 증진에 효과가 있었던 것으로 나타났다. 통제집단 역시 사후가 사전보다 유의하게 높게 나타났으나 변화의 정도를 두 집단 간에 비교하였을 때 상대적으로 정서조절 변화에서 실험집단이 더욱 유의미한 변화를 보였다. 넷째, 프로그램 참여 실험집단은 공격성과 대인관계 변화 중 공격성이 매우 감소하였다. 대인관계 변화는 실험집단과 통제집단 모두 유의미한 변화를 보였지만 프로그램에 참여한 실험집단이 통제집단과 비교할 때 더 유의미한 변화를 보였다. 이는 집단 역동 경험과 자기 이해와 타인이해를 통해 자기통제력 향상을 가져오고, 타인의 입장과 상황을 조망하며 사려 깊은 대응으로 대인관계의 질이 향상된 것으로 보인다. 다섯째, 외상 후 성장에서 실험집단이 유의미한 변화가 있었다. 이는 자존감 향상으로 새롭게 긍정적으로 자기 지각의 변화와 집단 역동을 통한 대인관계 깊이의 변화 요인이 효과를 보였을 것으로 보인다. 그리고 통제집단 역시 실험집단과 비교할 때 높은 수준은 아니지만, 외상 후 성장이 증진된 것으로 나타났다. 통제집단의 변화 정도는 모든 아동이 아동보호 시설에서 개인 상담을 받고 있어 이로 인한 효과라고 해석할 수 있다. 여섯째, 집단상담 참여자의 변화 양상에 대한 질적 분석을 보면 공격성과 외상 증상이 감소하는 모습이 관찰되었다. 이는 집단 역동을 통해 마음의 상처가 치유되며 심리적으로 안정되자 감정적인 자신을 마주하는 능력이 향상되고 자신이 소중한 사람이라는 것을 인식하면서 효과를 보였을 것으로 보인다. 결론적으로 본 프로그램에 참여한 실험집단이 통제집단에 비해 통계적으로 더욱 유의미한 변화를 보였으며, 특히 실험집단만 공격성 감소와 복합외상 후 스트레스 장애 감소, 자존감 증진에 효과적이었다. 따라서 본 연구에서 개발한 복합외상 학대피해 아동을 위한 TF-CBT 기반 집단상담 프로그램은 복합외상 학대피해 아동의 심리사회적 적응 향상과 유지에 효과가 있는 것으로 나타났다. 본 연구의 의의는 다음과 같다. 첫째, 아동학대로 복합외상 후 스트레스 장애(CPTSD) 증상을 보이는 아동을 위한 집단상담 프로그램을 찾아보기 어려운 여건에서 프로그램을 개발하고 효과성을 검증한 연구라는 점에서 의의가 있다. 이러한 측면에서 본 연구는 아동의 외상 회복과 성장 과정에서 발생할 부정적인 영향을 예방하는 효과를 기대할 수 있을 것이다. 둘째, 본 연구는 언어 기반으로 진행되는 인지행동치료와 마음 챙김의 신체 심리치료를 상호 통합적으로 결합하여 구성하였다. 이러한 통합적 접근은 안정화 작업과 인지적 재구성으로 치료 효율을 높이고 심층적인 변화인 외상 후 성장에 효과가 있음을 밝힐 수 있었다. 셋째, 본 연구에서는 학대피해 아동의 기존 외상 연구에서 중점을 두었던 정서적 측면 뿐만 아니라 복합 PTSD 증상 중에서 자기 지각, 정서조절, 대인관계라는 심리사회적 변인을 포함하여 복합적인 통합 접근을 시도하였는데 의의를 지닌다. 넷째, 복합외상 학대피해 아동들을 대상으로 한 개인 면담과 전문가 자문을 근거로 집단상담 프로그램을 구성함으로써 실제적인 학대 트라우마 아동들의 특성을 파악하였고 구체적인 개입방법을 마련하였는데 의의가 있다. 이는 학대피해 아동의 심층적 변화를 위한 효과적인 집단상담 프로그램을 찾으려는 임상현장의 치료자에게 기초연구와 증거자료를 마련하는데 기여할 것으로 기대한다.

The purpose of this study is to develop and verify the effectiveness of TF-CBT(Trauma-Focused Cognitive Behavioral Therapy)-based group counseling program for psychosocial adaptation of abused children with complex trauma. The program development was implemented in the following six phases, grounding on the program development research model: program planning, program organization, program development, expert validation, program implementation, and program evaluation. The program planning stage reviewed the following Korean and non-Korean academic works and the foundation for development were constructed: trauma; post-traumatic stress disorder (PTSD); PTSD reduction programs; cognitive behavioral therapy (CBT); TF-CBT; evidence-based trauma psychotherapy; posttraumatic growth (PTG), etc. In addition, the constructed program based on the personal interview data of abused children was modified and supplemented through expert consultation and evaluation. In this way, the final trauma-focused cognitive behavioral therapy (TF-CBT)-based group counseling program was formed. The following are the constituents of a group counseling program. First, in order to reduce the complex posttraumatic stress disorder (CPTSD) symptoms, like hyperarousal, re-experiencing, and avoidance that abuse trauma arouses, the program focused on stabilization and resource reinforcement using trauma psychoeducation, mindfulness, relaxation techniques, affective expression and regulation with an emphasis on arousal regulation and daily life stress management. Second, cognitive restructuring factors were constructed to find automatic thoughts about the dysfunctional self-perception factors of abused children with complex trauma and modify them into alternative thoughts. Third, as a deliberate rumination process for post-traumatic growth and memory processing, cognitive restructuring factors were constructed through the following: lifeline exercise, life narrative, and trauma narrative. Fourth, considering the characteristics of abused children with trauma, having difficulties in interpersonal relationships, social skills training including effective interpersonal skills, communication skills, and I-message methods was used as a component to improve their social skills. The constructed group counseling program was implemented by first dividing the participants into an experimental group and a control group to verify the effectiveness of the program. The target of this study were male elementary schoolers from 4th to 6th grade who were reported as abused children and who have entered child protection facilities per the Act On Special Cases Concerning The Punishment Of Child Abuse Crimes and Child Welfare Act; 20 were recruited through advertisements. To ensure the safety of the research progress, 16 children were re-selected after examining if they acquire internal and external resources and stabilization capacities among the recruited children receiving individual counseling from a psychological counselor at a child protection facility. The selected children for this study were randomly assigned to an experimental group and a control group, a group counseling program based on TF-CBT and each group includes 8 children. In the experimental group, the program was conducted once or twice a week, 60 minutes per session, for a total of 13 sessions. To measure changes that children have gone through before and after the group counseling program, the following were measured for pre-test, post-test, and follow-up: complex trauma inventory (CTI); cognitive factors (traumatized self-system scale: TSSS, Rosenberg self-esteem scale: RSES); emotional factors (affect regulation scale for children); behavioral factors (aggression questionnaire: AQ, relationship change scale: RCS); Korean posttraumatic growth inventory (K-PTGI). The SPSS 27.0 statistical program was employed for the data analysis; since the size of both groups, the experimental and control group was 8 or less, non-parametric tests (Mann-Whitney U test, Wilcoxon signed-rank test) were employed for analysis. In addition, to interpret children's changes more deeply and abundantly, qualitative research was simultaneously conducted in content analysis, using experience from each session, weeksheet of the sessions, participatory observation, evaluation of satisfaction, and so on. The following are the detailed results of this study. First, in the experimental group, group counseling program participants, the complex post-traumatic stress disorder symptoms were significantly reduced; the effectiveness was maintained and the symptoms stayed improved even 4 weeks later of the program termination and even for a follow-up test. A myriad of mindful relaxation techniques and cognitive perception seems to have improved self-control, assisting to alleviate symptoms of re-experiencing, avoidance, sense of threat, affect dysregulation as well as regulating daily life degree of arousal. Second, the experimental group showed significant changes in self-system and self-esteem for post-test as well as follow-up. Such an outcome can be explained as follows: cognitive restructuring brought about positive changes in thoughts and internal resources factors, namely, empathy, encouragement, and support from same-age children led by group dynamics would have helped. Third, the experimental group showed significantly higher changes in affect regulation in the post-test and follow-up compared to the pre-test; the effectiveness was maintained until 4 weeks later of the program termination; it was effective in improvement regarding affective regulation. In the control group, the post-test score was also significantly than the pre-test score; comparing the degree of change between the experiment and control groups, the experimental group showed a more significant change regarding affective regulation change. Fourth, the aggression in the experimental group participating in the program was greatly reduced, among the changes in aggression and interpersonal relationships. Both the experimental and control groups showed significant changes in interpersonal relationships; The experimental group, the group participating in the program, showed more significant changes. This seems to be that group dynamic experience, self-understanding, and understanding of others lead to improvement in self-control, and the quality of interpersonal relationships would have been improved for being able to view the position and situation of others, and considerate responses. Fifth, the experimental group showed a significant change in posttraumatic growth; this seems to be because improved self-esteem brings novel and positive changes in regard to self-perception, and it would also have changed the depth of interpersonal relationships through group dynamics. The control group showed improved post-traumatic growth as well, although lower than the experimental group. The change shown by the control group can be explained as follows: the effect that all children participants get individual counseling in the facilities. Sixth, the qualitative analysis regarding aspects of change of the group counseling participants shows the reduction of aggression and trauma symptoms. This is because as group dynamics healed their wounded heart and they get psychologically stabilized, their capacity of facing themselves emotionally improves and they could recognize that they are valuable. In conclusion, the experimental group, or program participants, showed more statistically significant changes than the control group; in particular, only the experimental group showed a statistically significant reduction in aggression and post-traumatic stress disorder and improvement in self-esteem. Therefore, the TF-CBT-based group counseling program for abused children with complex trauma that this study developed was found to be effective in the improvement and the maintenance of the psychosocial adjustment of abused children with complex trauma. The following describes the significance of this study. First, this study holds its significance because it developed a program and verified its effectiveness while there are few group counseling programs for abused children who show symptoms of complex PTSD. In this vein, this study is expected to prevent the probable negative effects stemming from children's recovery from trauma and the growth process. Second, this study was constructed by combining language-based cognitive-behavioral therapy and mindfulness somatic psychotherapy through mutual integration. Such an integrated approach increased the treatment efficiency of the subjects through stabilization and cognitive restructuring and was shown to be effective in post-traumatic growth, which is a profound change. Third, this study is significant for attempting a complex integration approach including psychosocial factors such as self-perception, affective regulation, and interpersonal relationships among complex PTSD symptoms, as well as the emotional aspects, which previous trauma studies regarding abused children focused on. Fourth, this study is significant for constructing a group counseling program based on individual interviews and expert advice targeting abused children with complex trauma, the characteristics of actual abused trauma children were identified and specific intervention methods were prepared. This study is expected to contribute to providing basic research and evidence to therapists in the clinical field who are trying to find an effective group counseling program for in-depth changes in abused children.
The purpose of this study is to develop and verify the effectiveness of TF-CBT(Trauma-Focused Cognitive Behavioral Therapy)-based group counseling program for psychosocial adaptation of abused children with complex trauma. The program development was implemented in the following six phases, grounding on the program development research model: program planning, program organization, program development, expert validation, program implementation, and program evaluation. The program planning stage reviewed the following Korean and non-Korean academic works and the foundation for development were constructed: trauma; post-traumatic stress disorder (PTSD); PTSD reduction programs; cognitive behavioral therapy (CBT); TF-CBT; evidence-based trauma psychotherapy; posttraumatic growth (PTG), etc. In addition, the constructed program based on the personal interview data of abused children was modified and supplemented through expert consultation and evaluation. In this way, the final trauma-focused cognitive behavioral therapy (TF-CBT)-based group counseling program was formed. The following are the constituents of a group counseling program. First, in order to reduce the complex posttraumatic stress disorder (CPTSD) symptoms, like hyperarousal, re-experiencing, and avoidance that abuse trauma arouses, the program focused on stabilization and resource reinforcement using trauma psychoeducation, mindfulness, relaxation techniques, affective expression and regulation with an emphasis on arousal regulation and daily life stress management. Second, cognitive restructuring factors were constructed to find automatic thoughts about the dysfunctional self-perception factors of abused children with complex trauma and modify them into alternative thoughts. Third, as a deliberate rumination process for post-traumatic growth and memory processing, cognitive restructuring factors were constructed through the following: lifeline exercise, life narrative, and trauma narrative. Fourth, considering the characteristics of abused children with trauma, having difficulties in interpersonal relationships, social skills training including effective interpersonal skills, communication skills, and I-message methods was used as a component to improve their social skills. The constructed group counseling program was implemented by first dividing the participants into an experimental group and a control group to verify the effectiveness of the program. The target of this study were male elementary schoolers from 4th to 6th grade who were reported as abused children and who have entered child protection facilities per the Act On Special Cases Concerning The Punishment Of Child Abuse Crimes and Child Welfare Act; 20 were recruited through advertisements. To ensure the safety of the research progress, 16 children were re-selected after examining if they acquire internal and external resources and stabilization capacities among the recruited children receiving individual counseling from a psychological counselor at a child protection facility. The selected children for this study were randomly assigned to an experimental group and a control group, a group counseling program based on TF-CBT and each group includes 8 children. In the experimental group, the program was conducted once or twice a week, 60 minutes per session, for a total of 13 sessions. To measure changes that children have gone through before and after the group counseling program, the following were measured for pre-test, post-test, and follow-up: complex trauma inventory (CTI); cognitive factors (traumatized self-system scale: TSSS, Rosenberg self-esteem scale: RSES); emotional factors (affect regulation scale for children); behavioral factors (aggression questionnaire: AQ, relationship change scale: RCS); Korean posttraumatic growth inventory (K-PTGI). The SPSS 27.0 statistical program was employed for the data analysis; since the size of both groups, the experimental and control group was 8 or less, non-parametric tests (Mann-Whitney U test, Wilcoxon signed-rank test) were employed for analysis. In addition, to interpret children's changes more deeply and abundantly, qualitative research was simultaneously conducted in content analysis, using experience from each session, weeksheet of the sessions, participatory observation, evaluation of satisfaction, and so on. The following are the detailed results of this study. First, in the experimental group, group counseling program participants, the complex post-traumatic stress disorder symptoms were significantly reduced; the effectiveness was maintained and the symptoms stayed improved even 4 weeks later of the program termination and even for a follow-up test. A myriad of mindful relaxation techniques and cognitive perception seems to have improved self-control, assisting to alleviate symptoms of re-experiencing, avoidance, sense of threat, affect dysregulation as well as regulating daily life degree of arousal. Second, the experimental group showed significant changes in self-system and self-esteem for post-test as well as follow-up. Such an outcome can be explained as follows: cognitive restructuring brought about positive changes in thoughts and internal resources factors, namely, empathy, encouragement, and support from same-age children led by group dynamics would have helped. Third, the experimental group showed significantly higher changes in affect regulation in the post-test and follow-up compared to the pre-test; the effectiveness was maintained until 4 weeks later of the program termination; it was effective in improvement regarding affective regulation. In the control group, the post-test score was also significantly than the pre-test score; comparing the degree of change between the experiment and control groups, the experimental group showed a more significant change regarding affective regulation change. Fourth, the aggression in the experimental group participating in the program was greatly reduced, among the changes in aggression and interpersonal relationships. Both the experimental and control groups showed significant changes in interpersonal relationships; The experimental group, the group participating in the program, showed more significant changes. This seems to be that group dynamic experience, self-understanding, and understanding of others lead to improvement in self-control, and the quality of interpersonal relationships would have been improved for being able to view the position and situation of others, and considerate responses. Fifth, the experimental group showed a significant change in posttraumatic growth; this seems to be because improved self-esteem brings novel and positive changes in regard to self-perception, and it would also have changed the depth of interpersonal relationships through group dynamics. The control group showed improved post-traumatic growth as well, although lower than the experimental group. The change shown by the control group can be explained as follows: the effect that all children participants get individual counseling in the facilities. Sixth, the qualitative analysis regarding aspects of change of the group counseling participants shows the reduction of aggression and trauma symptoms. This is because as group dynamics healed their wounded heart and they get psychologically stabilized, their capacity of facing themselves emotionally improves and they could recognize that they are valuable. In conclusion, the experimental group, or program participants, showed more statistically significant changes than the control group; in particular, only the experimental group showed a statistically significant reduction in aggression and post-traumatic stress disorder and improvement in self-esteem.
