목적: 속립성 폐결핵 환자에서 급성호흡부전을 동반한 군과 동반하지 않는 두 군간의 단순흉부사진 소견의 차이를 알아보고자 하였다. 대상과 방법: 속립성 폐결핵으로 진단된 32명의 환자를 급성호흡부전을 동반한 군과 (n=10),동반하지 않은 군 (n=22)으로 분류하여 단순흉부사진을 대상으로 결절, 폐경결, 간유리 음영, 흉막 삼출액, 작은 석회화 결절과 불규칙한 선상의 증가된 음영의 유무를 분석하였다. 폐야를 4등분한 후 진폐증의 분류에 쓰이는 ILO 분류법을 이용하여 각 폐야의 결절의 크기와 풍부성을 분석하였고 폐경결과 간유리 음영의 분포범위는 점수화하여 합산한 후 각 방사선 소견에 대해 두 군간의 차이를 비교하였다. 추적검사 중 급성호흡부전이 발생한 4명의 환자에서는 연속적인 단순흉부사진을 분석하였다. 결과: 급성호흡부전을 동반한 속립성 폐결핵 환자군에서 급성호흡부전을 동반하지 않는 군에 비해 간유리 음영, 폐경결, 그리고 흉막 삼출액이 통계학적으로 유의하게 높게 나타났다. 또한 급성호흡부전을 동반한 군에서 폐경결과 간유리 음영의 분포 범위가 유의하게 컸으나 결절의 크기와 풍부성은 통계학적으로 유의한 차이를 보이지 않았다. 연속적인 방사선사진을 분석한 4예에서 급성호흡부전이 발생한 후 간유리 음영의 분포범위가 증가하였다. 결론: 속립성 결핵환자에서 급성호흡부전을 동반한 경우에는 보다 광범위한 폐경결과 간유리 음영을 보인다. 단순흉부사진에서 초기에 광범위하게 나타나는 간유리 음영은 급성호흡부전으로의 진행을 시사하는 소견이다.

We compared the radiologic findings between the two groups. Results: Ground-glass opacity, consolidation, and pleural effusion were seen more frequently in miliary tuberculosis patients with ARF than in those without ARF. Although the size and profusion of nodules were similar in both groups (p>0.05), consolidation and ground-glass opacity in cases of miliary tuberculosis with ARF were significantly more extensive than in those without ARF (p<0.005). Conclusion: GGO and consolidation were more extensive in miliary tuberculosis patients with ARF. A finding of ground-glass opacity in miliary tuberculosis patients might be an early indication of developing ARF.
