제안 방법
대상자의 측정 자세는 의자에 앉은 상태에서 상체를 바로 세우고 견관절은 해부학적 위치를 유지하였으며 주관절은 어깨높이와 같게 한 후 Digital angle gauge(WR300 Wixey™, , 2010, USA)를 사용하여 10° 굴곡부터 90° 굴곡까지 10° 씩 증가시키면서 주관절 각도를 조정하였다.

대상 데이터
연구 대상의 자료 수집은 2011년 10월부터 2012년 4월까지 다음의 선정 조건을 만족하는 뇌졸중 환자 18명을 선정하였다. 선정 조건은 다음과 같다: 발병 후 신경학적 회복이 어느 정도 일어나고 불용기간을 거친 6개월 이상 18개월 이하 인자; 주관절 MAS(Modified Ashworth Scale) 등급은 수동운동이 어려운 4등급을 제외한 1에서 3인자; 주관절의 수동굴곡 운동이 10°∼90° 가능한 자(주관절 완전 신전이 0°); 주관절에 근골격계의 병적 소견이 없는 자; 실험자의 지시를 이해하고 수행할 수 있는 자; 보톨리눔 톡신이나 페놀 또는 알코올 주사의 치료력이 없는 자; 대상자는 연구에 대해 실험자로부터 설명을 들었으며, 자발적으로 동의하였다.
영상 수집은 GE LOGIQ-3 EXPERT(GE Medical systems co., 2003, Korea)를 사용하여 상완이두근(biceps brachii)을 측정하였다. 초음파 영상 측정에 사용된 초음파 변환기는 12 ㎒ 선형탐촉자(linear transducer)이며, 민감도(gain: G72)와 동적범위(dynamic range: C4)는 고정된 값으로 모든 검사에 동일하게 적용하였다.
대상 환자의 환측 상완이두근의 경직평가는 임상경력 5년이 넘는 5명의 물리치료사가 MAS를 이용하여 평가하여 18명의 대상자 중 측정자 간 동일한 등급이 나온 12명의 환자를 다시 선발 하였다[Table 1].

이론/모형
상완이두근의 근 긴장도를 측정하기 위해 Myotonometer® (Neurogenic Technologies, Inc., Missoula, 2000, USA)를 사용하였다.
성능/효과
근긴장은 50°이하 신전 위치에서 전위 값이 낮게 나타나 근긴장이 높아짐을 나타냈다. 이렇듯 이완 시 상완이두근의 근구조 및 탄성은 주관절 각도에 의해 영향을 받는다는 것을 알 수 있었다. 본 결과를 뇌졸중 환자의 근구조 변화 및 임상치료의 연구 기초자료로 사용될 수 있을 것으로 생각된다.
그러나, 지금까지 뇌졸중 발병 후 이완 시에서 관절 각도를 변화시켜 상지근육의 구조적 특징을 초음파로 평가한 연구는 전무하다. 따라서 관절의 움직임을 배제한 근육 구조의 측정보다 관절 각도 변화에 따른 이완 시 근육 구조의 측정은 재활치료 후 근육 기능의 향상을 평가하고 뇌졸중 환자의 근육 구조의 메커니즘을 이해하는데 있어서 많은 도움을 줄 수 있을 것으로 생각된다.
