Purpose: This study was conducted to investigate the correlation among nurses' knowledge, performance, and stress about care for delirium. Methods: The data were collected using the instruments for knowledge about nursing care (50 items), performance (23 items), and stress (20 items). Descriptive statistics and Pearson's correlation coefficient were used to analyze the data of 222 participants. Results: Nursing knowledge were different by clinical experience (F=3.12, p=.016), position at work (t=-2.54, p=.012), experience in caring patients with delirium (t=3.90, p<.001), and conflicts with other disciplines on matters related to delirium (t=4.00, p=.001). The difference in performance were associated with age (F=4.19, p=.001), clinical experience (F=2.67, p=.003), and whether there was a guideline for managing the patients with delirium (t=2.30, p=.022). Stress of nurses was different by whether they had a religion or not (t=-2.41, p=.017). The knowledge of care for delirium had the positive relationship with performance (r=.21, p=.001) and negative relationship with stress (r=-.29, p=.001). Conclusion: It is necessary to develop educational program and guideline for improving the knowledge and performance in care for delirium among nurses working at orthopedic hospitals.

대상 데이터

본 연구의 대상은 P시의 5개 정형외과병원에서 외래와 수술실을 제외한 병동 간호사로 본 연구의 목적을 이해하고 참여에 동의한 간호사를 대상으로 하였다. 연구대상자의 크기는 G*Power analysis program 3.
20일 때, 표본크기를 산출하면 최소표본 크기는 191명이었다. 이에 탈락률 25% 고려하여 총 250부의 설문지를 배부하여, 그 중 12부(4.8%)가 미회수되었고, 16부(6.4%)는 답이 누락되었으며, 본 연구에는 222부(88.8%)가 분석에 사용되었다.

Kim (1989)이 개발한 도구를 Park (2011)이 수정 ‧ 보완한 도구로 측정하였다. 이 도구는 총 20문항으로 구성되어있으며, 스트레스가 없으면 0점, 스트레스가 매우 심할 때 100점으로 하여 그 정도를 표시하도록 하였다.
섬망간호 수행정도는 Seo (2006)의 도구를 Park (2011)이 수정 ‧ 보완한 도구로 측정하였다. 도구는 섬망 위험요인 사정 수행 4문항과 간호중재 19문항으로 총 23문항으로 구성되었다.
섬망간호 지식은 섬망 위험요인, 예후, 사정 및 중재에 대한 지식으로 Seo (2006)의 도구를 Park (2011)이 수정 ‧ 보완한 도구로 본 연구자가 문헌고찰을 통해 골절과 관련된 1문항과 섬망의 원인 1문항, 섬망의 증상 1문항, 섬망의 치료 목적 1문항을 추가하여 정형외과병원 간호사의 섬망간호 지식을 측정하는 도구로 총 50문항으로 구성하였다. 도구는 간호대학 교수 1인, 신경과 전문의 1인, 정형외과 전문의 2인에게 내용 타당도를 확인받았다.

성능/효과

결과적으로 섬망간호 지식과 수행은 비교적 낮은 수준이었고, 섬망간호 지식과 수행은 정적상관관계이며, 섬망간호 지식과 스트레스는 부적상관관계를 보여 정형외과병원 간호사의 섬망간호 지식수준을 높이고 섬망간호 수행을 높일 수 있는 교육 프로그램 개발과 가이드라인 개발이 적극적으로 요구되며, 섬망간호 스트레스를 관리할 수 있는 스트레스 중재 프로그램 개발이 필요한 것으로 나타났다.
대상자의 특성에 따른 섬망간호 스트레스 차이는 종교유무’(t=-2.41, p=.017)에서 차이가 있는 것으로 나타났다. 종교유무는 ‘종교가 없는 군’이 ‘종교가 있는 군’에 비하여 스트레스 정도가 심한 것으로 나타났다(Table 3).
정형외과병원 간호사의 일반적 특성에 따른 섬망간호 지식을 살펴보면, 총 임상경력, 직위, 섬망 환자 전동, 전원 경험 유무, 섬망 환자로 인한 의료진과 타부서와 갈등 경험 유무에서 차이를 보였다. 간호사의 임상경력이 많을수록 지식수준이 높다는 선행연구들(Ham, 2011; Kim, 2011; Yang, 2006)와 유사한 결과를 보였다.

후속연구

둘째, 섬망 환자 간호를 위한 교육 프로그램 개발과 가이드라인을 개발하고 그 효과를 검증하는 연구를 제언한다.
