Objectives : The objective of this study is to develop the strategies of the psoriasis clinical trials guideline on Korean medicine by comparison between Clinical guidelines and domestic and foreign clinical trials papers of psoriasis on Korean medicine. Methods : This study is based on analysis of papers on Clinical trials guidelines, Clinical practice guideline and clinical trials about Korean medicine. The papers were searched from Pubmed, Medline, Oasis(Oriental Medicine Advanced Searching Integrated System), Korean Traditional Knowledge Portal and Google portal database. Results : A total 8 Clinical practice guidelines and 2 Clinical trials guidelines were found. Moreover, there were 15 foreign papers about clinical trials and 29 internal articles about case studies. They suggested the diagnostic strategy, classification, effective outcome measure, severity measure, precaution of combination therapy, precaution and treatment period of clinical trials, safety evaluation, patterns of Korean Medicine, clinical specific features on psoriasis.Conclusions : The criteria of every item to provide the clinical trials guideline using Korean medicine on psoriasis were developed by apply the results. If we accumulate the more clinical articles on Korean medicine, it will be great help to develop the reliable standard of that guideline.

제안 방법

국내외 건선과 관련된 임상시험가이드라인 및 임상진료지침의 각 항목 분석을 위하여 임상가이드라인을 검색 및 분류하였다. 구체적인 방법으로 Google portal에서 ‘Psoriasis Guideline', 'Psoriasis trial guideline' 'Psoriasis investigation guideline'이라는 검색식을 이용하였다.
국내외에서 건선을 대상으로 한약제제 관련 임상연구를 진행한 논문과 증례 연구를 고찰하였다. 국외 논문 검색은 pubmed와 medline에서 시행하였으며 실험논문, 한약제제 대상이 아닌 것은 제외하였다.

대상 데이터

1차로 본 연구진이 국외 임상 진료 가이드라인을 선행 조사한 내용을 바탕으로 수차례 회의를 거쳤으며 이를 통해 건선 임상시험 가이드라인 개발을 위해 반드시 필요한 총 10가지의 주제를 선정하였다. 주제는 진단 기준, 건선 분류, 유효성 평가 변수, 중등도 평가 기준, 탐색적 임상시험에서 고려해야 할 사항, 확증적 임상시험 시 적절한 치료 기간, 안전성 평가 항목, 한의학적 유형(변증) 분류, 건선 질환 특이적 고려사항이 포함된다.
medline에도 pubmed에서 검색한 검색식을 풀어 하나씩 검색해 보았으나 선정기준에 부합하는 논문은 없었다. 합하여 총 15가지의 국외 임상연구 논문을 대상으로 연구하였다.
오아시스 포탈에서는 총 21개를 찾을 수 있었으며 같은 방식을 이용하고 앞서 찾은 논문을 제외하여 총 11개를 추가할 수 있었다. 합하여 총 29개의 증례보고 논문을 대상으로 연구를 진행하였다.

이론/모형

국내외 건선과 관련된 임상시험가이드라인 및 임상진료지침의 각 항목 분석을 위하여 임상가이드라인을 검색 및 분류하였다. 구체적인 방법으로 Google portal에서 ‘Psoriasis Guideline', 'Psoriasis trial guideline' 'Psoriasis investigation guideline'이라는 검색식을 이용하였다.
(German Society of Dermatology;Germany), Management of psoriasis vulgaris(Dermatological Society of Malaysia;Malaysia), Diagnosis and management of psoriasis and psoriatic arthritis in adults(Scottish Intercollegiate Guidelines Network;Scotland), Canadian guidelines for the management of plaque psoriasis(Canadian Dermatology Association;Canada)가 있었다. 이 논문에 사용된 건선 임상시험가이드라인으로는 Guideline on clinical Investigation of medicinal products indicated for the treatment of psoriasis(European Medicines Agency;Europe), 中药新药治疗白疕的临床硏究指导原则(中国医药科技出版社)이 있었다.

후속연구

EADV 가이드라인에 제시된 바에 따르면 1차적으로 PASI를 통하여 치료의 성과를 파악하고 PASI reduction rate가 50%에서 75%의 사이일 경우 DLQI를 지표로 삼도록 제시되어 있다47). 또한 유럽 임상시험가이드라인에서 DLQI를 2차성 유효성 평가 변수로 제시하고 있는 것54)을 고려하였을 때, 1차 유효성 평가변수인 PASI를 사용한 후 2차적으로 삶의 질 지표로 DLQI를 우선적으로 사용하는 것이 바람직할 것으로 사료된다. 삶의 질 평가도구의 경우 정신 상태 및 일상생활, 질병 특이적 도구들이 다양하게 존재하므로 임상 시험 목적에 맞는 특이적인 삶의 질 평가 지표를 추가적으로 사용할 것을 고려할 수 있다.
그 중 특히 많이 사용되는 Methotrexate의 경우 간독성, 빈혈, 백혈구, 적혈구, 혈소판 감소 등 부작용이 있으므로 주의해야 하며 Ciclosporin의 경우 신독성, 고혈압 등을, 레티노이드제제 병용 투여 시간, 신장질환, 고지혈증, 심한 음주자 등은 금기사항에 속하니 유의해야 할 필요가 있다47). 앞으로 병용요법의 비중이 높아질 것으로 사료되며, 이에 따른 문제점, 주의점을 알 수 있는 연구가 시급히 필요하리라 사료된다.
이상에서 언급한 진단 기준, 건선 분류, 유효성 평가 변수, 중등도 평가 기준, 탐색적 임상시험에서 고려해야 할 사항, 확증적 임상시험 시 적절한 치료기간, 안전성 평가 항목, 한의학적 유형(변증) 분류, 건선 질환 특이적 고려사항 등을 건선의 한약제제 임상시험가이드라인을 만들 때 반드시 고려해야 할 것으로 생각된다.
건선 임상시험 시 건선 질환 특징 중 하나가 건선의 발생 위치가 환자의 QoL에 상당히 영향을 주며, 이 부분이 건선 질병의 심각도를 좌우할 수 있다는 점이다. 한약제제를 이용한 임상시험가이드라인에서 이러한 부분을 명확하게 추가하여야 제대로 된 검증이 가능할 것이다. 또한, 건선은 동반질환이 많은 질환으로 건선의 임상연구 시 동반질환을 가지고 있는 환자를 배제하기 위한 기준을 만들 때 이러한 부분을 같이 고려해야할 필요가 있다.
