Purpose: The purpose of this study was to examine the degree of intensive and critical care nursing competence in senior nursing students. Methods: The research method for this study comprised a descriptive survey study with a convenience sample of 113 senior nursing students from one university. The instrument entitled ICCN-CS-1, which measures intensive and critical care nursing competence, was translated by the authors following WHO guidelines for translation process. The data were collected using ICCN-CS-1. Results: The mean score of intensive and critical care nursing was above the intermediate level. The competence indices of knowledge, skill and attitude/value were also above the intermediate level, which suggests that their competence is good. For two intensive and critical care nursing domains, the nursing students' mean score of clinical competence is higher than that of professional competence. The competence of intensive and critical care is also influenced by nursing students' satisfaction in their major and clinical practice as well as their confidence in clinical practice. Conclusion: The senior nursing students' competence of intensive and critical care nursing is above the intermediate level. A consideration of the influencing factors of satisfaction of nursing major/ICU practice and confidence of ICU practice is needed in intensive and critical care nursing education.

본 연구의 대상자는 D시에 소재한 일개 간호대학의 중환자간호와 관련된 이론과 실습을 모두 이수한 간호대학 4학년학생을 대상으로 하였다. 연구대상자수는 G*Power 3.
3으로 채택하였고, 유의 수준과 검정력의 경우 중환자실 간호사의 지식 및 간호수행도를 관찰한 선행연구(Yang, 2010)를 토대로 산출하였으며 약 10%의 탈락률을 고려하여 120명의 대상자를 선정하였다. 이 중 설문을 거부한 7명을 제외하여 최종 113명의 자료수집을 수행하였다.

데이터처리

또한 중환자 간호 지식, 기술 및 태도/가치 간의 관계는 Pearson’s correlation coefficient로 분석하였다. 대상자의 일반적 특성에 따른 중환자 간호 지식, 기술 및 태도/가치의 차이는independent t-test, one-way ANOVA 또는 Kruskal-Wallis test를 이용하여 분석하였으며, 사후검증은 one-way ANOVA의 경우 bonferroni test, Kruskal-Wallis test의 경우는 1종 오류 발생 가능성을 보정하기 위하여 Mann-Whitney U test with bonferroni correction을 이용하였다. 통계량의 유의 수준은p<.

이론/모형

본 도구는 현재 국내에서 번역되어 사용되지 않았기 때문에 본 연구팀에서는 도구 개발자에게 도구 사용에 대한 허락을 받았다. 도구 번안은 세계보건기구(WHO, World Health Organization)의 가이드라인에 따라 번역-역번역 방법을 이용하여 번역하였다(WHO, n.d.). 본 연구자 3명이 원 도구를 한글로 번역하였고, 이를 2개 국어(영어, 한국어)를 사용하는 중환자 간호 전문가 2명에게 번역의 적합성을 검증 받은 후 연구팀에서 수정하였다.
본 연구에서는 졸업반 간호대학생을 대상으로 중환자 간호역량을 측정하기 위해 Lakanmaa (2012)가 개발한 중환자 간호역량 측정도구(intensive and critical care nursing competence scale, ICCN-CS-1)을 사용하였다. 본 도구는 중환자 간호 역량을 세 가지 측면(dimension); 중환자 간호 지식(36문항), 기술(36문항), 태도/가치(36문항)와 두 가지 영역(domain); 임상적영역(60문항) 및 전문적 영역(48문항)으로 각각 구분하여 측정할 수 있도록 구성되어 있다.

성능/효과

84점 이었고, 하위 측면인 지식, 기술 및 태도/가치 모두 중앙값을 상회하는 수준으로 나타났으며, 지식, 기술 및 태도/가치는 서로 상관관계가 있는 것으로 나타났다. 또한 중환자 간호 역량을 임상적 영역 및 전문적 영역으로 구분하여 분석한 결과 임상적 역량이 전문적 역량에 비해 다소 높은 것으로 나타났다.
본 연구는 일 대학 간호대학생의 중환자 간호 역량 정도를 파악하기 위해 수행하였으며, 본 연구결과 간호대학생이 지각하고 있는 중환자 간호 역량의 총 평균은 3.84점 이었고, 하위 측면인 지식, 기술 및 태도/가치 모두 중앙값을 상회하는 수준으로 나타났으며, 지식, 기술 및 태도/가치는 서로 상관관계가 있는 것으로 나타났다. 또한 중환자 간호 역량을 임상적 영역 및 전문적 영역으로 구분하여 분석한 결과 임상적 역량이 전문적 역량에 비해 다소 높은 것으로 나타났다.
일반적 특성에 따른 중환자 간호 역량을 살펴본 결과 전공만족도, 중환자 간호학 실습 자신감이 높을수록 지식 및 기술측면의 점수가 높은 것으로 나타났고, 중환자 간호학 실습 만족도가 높을수록 기술 측면의 점수가 높은 것으로 나타났다.

후속연구

둘째, 중환자 간호학 실습 시 실습장소별 프리셉터를 배치하여 간호대학생의 임상실습 지도를 강화할 것을 제언한다.
셋째, 연구결과의 일반화를 위해 연구 표본 크기를 확대한 반복연구를 제언한다.
첫째, 자가평가와 객관적 평가 방식을 이용하여 간호대학생의 지각된 중환자 간호 역량과 실제 역량 간 차이를 비교하는 연구가 필요함을 제언한다.
