Objectives: The purpose of this study was to investigate the effects of Korean medicine on patients with idiopathic Parkinson’s disease.Methods: The charts of 47 patients diagnosed with idiopathic Parkinson’s disease in the Department of Internal Korean medicine, Stroke and Neurological Disorders Center, Kyung Hee University Hospital, Gangdong between August 2012 and July 2016 were reviewed. The Unified Parkinson’s disease rating scale (UPDRS) was administered before and after treatment with Korean medicine.Results: Thee average UPDRS Ⅱ+Ⅲ, UPDRS Ⅱ, and UPDRS Ⅲ of the 47 patients decreased significantly from 22.26±15.15, 10.19±7.53, and 12.06±8.35, respectively, pretreatment to 16.96±13.63, 7.47±6.44, and 9.49±7.73, respectively, post-treatment. The average postural instability and gait difficulty (PIGD), tremor, and bradykinesia also significantly improved post-treatment.Conclusions: These results provide evidence that Korean medicine can improve the activities of daily living and motor function of patients with idiopathic Parkinson’s disease. In particular, Korean medicine may be effective for the treatment of PIGD, tremor, and bradykinesia.

하지만 한국의 한의의료기관에서는 파킨슨병 치료에 주로 침, 약침, 봉침, 한약 투여 등의 복합치료를 시행하고 있으나, 복합치료의 구체적인 효과에 대한 연구는 부족한 실정이다. 따라서 본 연구에서는 특발성 파킨슨병 환자에게 복합 한의치료가 파킨슨병의 tremor, bradykinesia, PIGD 각각의 세부적 지표에 있어 어떤 효과가 있는지 알아보고자 강동경희대학교병원 중풍뇌질환센터 한방내과의 치료 사례들을 후향적으로 차트 분석했다.
제안 방법
증류수에 1:20000으로 희석한 봉독(한국 유밀농원산)을 1주일에 1회 양측 태충(LR3), 양릉천(GB34), 족삼리(ST36), 곡지(LI11), 풍지(GB20)에 인슐린 주사기로 각각 0.1 cc씩 주입하였다.
주로 억간산, 청간소요산이나 체간 및 망문문절을 통해 사상체질을 변증하여 사상체질 처방을 사용하였다. 탕약의 경우 1일 기준으로 2첩을 3회로 나누어 120 cc씩 매 식후 2시간에 복용하였고, 제제약은 강동경희대한방병원에서 조제한 약을 복용하였다.
25 mm, 길이 30 mm의 규격의 일회용 stainless steel 호침을 사용하여 15분간 유침하였다. 환자의 상태에 따라 자침을 하되, 주로 백회(GV20)와 양측 풍지(GB20), 합곡(LI4), 곡지(LI11), 족삼리(ST36), 양릉천(GB34), 삼음교(SP6), 태충(LR3) 등의 혈위에 자침하였다.
환자의 주소증에 따라 필요 시 약침, 전침, 뜸, 부항 치료 등을 추가적으로 시행하였다.
대상 데이터
2012년 8월 1일부터 2016년 7월 31일까지 강동경희대학교병원 중풍뇌질환센터 한방내과에 내원하여 특발성 파킨슨병으로 치료받은 환자 중 초진 시 UPDRS를 평가를 하고 추후 UPDRS follow up 기록이 남아있는 환자를 선별하여 전자 의무 기록 (electronic medical records, EMR)을 확인하였다.
2012년 8월 1일부터 2016년 7월 31일까지 강동경희대학교병원 중풍뇌질환센터 한방내과에 내원하여 파킨슨 증후군으로 치료받은 환자는 총 431명이 었다. 이 중 초진 시 UPDRS를 평가를 한 환자는 189명이고 추후 UPDRS follow up 기록이 남아있는 환자는 92명이었다.
초진 시 UPDRS 중 part Ⅱ와 part Ⅲ 평가를 하였고, 한의치료 시행한 후 평균 3-4개월 후 재진 시 UPDRS 재평가를 하였다. UPDRS를 통해 PIGD, tremor, bradykinesia의 score를 계산하였다. PIGD score는 UPDRS Ⅱ 13, 14, 15번 항목의 합과 UPDRS Ⅲ 29, 30번 합의 합계로 계산하고, tremor score는 UPDRS Ⅱ 16번 항목과 UPDRS Ⅲ 20, 21번 항목의 합으로 계산하고, bradykinesia score는 UPDRS Ⅲ 23, 24, 25, 26번 항목의 합으로 계산하였다.
본 연구의 통계처리는 statistical program for social science(SPSS) v12.0을 이용하였으며, 모든 자료는 mean±standard deviation(SD)으로 나타내었다. 환자들의 한의치료 전후 UPDRS 변화는 paired t-test를 이용하여 분석하였다.
UPDRS는 I에서 Ⅵ 파트까지 있고, 이 중 Ⅱ는 일상생활활동 능력을 평가하고, Ⅲ는 운동기능을 평가하는 항목이다15. 초진 시 UPDRS 중 part Ⅱ와 part Ⅲ 평가를 하였고, 한의치료 시행한 후 평균 3-4개월 후 재진 시 UPDRS 재평가를 하였다. UPDRS를 통해 PIGD, tremor, bradykinesia의 score를 계산하였다.

성능/효과
Bradykinesia score는 본 연구결과, 한의치료 후 3.83±3.18에서 3.02±3.12로 21.15% 감소하여 유의한 차이를 보였다. Wang 등22은 전침치료가 파킨슨병 환자의 bradykinesia 증상을 개선시켰는데, 특히 bradykinesia-rigidity type과 mixed type에서 더 좋은 효과를 보였다고 보고하였다.
PIGD에 한의치료가 유의한 효과를 보이는 것은 파킨슨병의 대표적인 치료 약제인 도파민 제제가 PIGD를 개선시키지 못하므로8,9 더욱 의미가 있다. PIGD 5개 항목 중 falling, freezing, walking, gait 등의 4개 항목에서 모두 유의한 개선 효과를 보였으나, postural stability는 유의한 차이를 보이지 않았다. 하지만 Cho 등13에 따르면 파킨슨병 환자에 침치료와 봉독치료를 통해 Berg balance scale상에서 유의한 개선효과가 있음을 보고하였고, Toosizadeh 등12에 따르면 파킨슨병 환자에서 전침치료 시행 후 stabilogram으로 일반인에 비해 증가된 ratio of medial-lateral(ML) center of gravity(COG) sway to anterior-posterior(AP) sway(COGML/AP)가 감소되어 전침치료가 postural stability에 유효하다는 것을 확인한 바 있고, 이 등20에 따르면 한양방 복합치료를 통해 UPDRS Ⅲ 중 postural stability가 개선됨을 보고한 바 있다.
PIGD score는 본 연구결과, 한의치료 후 4.13±3.65에서 3.00±3.26로 27.36% 감소하여 유의하게 호전되었다. PIGD에 한의치료가 유의한 효과를 보이는 것은 파킨슨병의 대표적인 치료 약제인 도파민 제제가 PIGD를 개선시키지 못하므로8,9 더욱 의미가 있다.
Tremor score는 본 연구결과, 한의치료 후 2.49±1.67에서 1.94±1.63로 22.09% 감소하여 유의한 차이를 보였다. 특히 tremor 정도와 resting tremor를 감소시키는 것을 확인하였다.
5-5점 정도 감소하는 것이 임상적으로 유효한 최소 변동폭(minimal clinically important change)으로 알려져있다16-18. 따라서 본 연구 결과를 통해 파킨슨병 환자의 UPDRS Ⅱ+Ⅲ, Ⅱ와 Ⅲ 개선에 있어 한의치료가 유효하다는 것을 확인할 수 있다.
본 연구를 통하여 특발성 파킨슨병 환자에게 평균 약 3-4개월 동안 약 20회 정도의 한의치료 시 UPDRS Ⅱ와 UPDRS Ⅲ에서 유의한 효과가 있는 것을 확인할 수 있었다. 본 연구결과, 한의치료 후 UPDRS Ⅱ+Ⅲ, Ⅱ와 Ⅲ는 각각 5.30점, 2.72점, 2.57점 만큼 유의하게 호전되었다. UPDRS Ⅱ+Ⅲ는 5-8점, UPDRS Ⅲ는 2.

9로 감소하였다고 보고하였는데, 본 연구와 피험자 baseline severity에서 차이가 많이 나며, 본 연구는 전침을 선택적으로 시행하여 주로 자침만을 시행한 경우가 많았고, 치료기간 및 횟수도 본 연구에서 더 적은 등에서 차이가 있었다. 따라서 전침치료를 시행하고, 치료를 조금 더 높은 빈도로 시행한다면 더 좋은 효과가 있을 것으로 기대되며, 향후 파킨슨병의 type과 severity 등에 따른 치료 효과의 차이에 대한 추가적인 연구가 필요할 것으로 사료된다.
향후 파킨슨병에 대한 사상체질 이론을 포함한 한의학적 변증증형과 처방 표준안을 확립하고 이에 대한 연구가 필요할 것으로 사료된다. 또한 파킨슨병의 비운동성 증상인 인지장애, 우울, 불안 등의 기분장애, 수면장애, 피로감, 자율신경장애 등의 증상에 대한 한의치료의 효과에 대한 연구도 필요할 것으로 사료된다.
이를 통해 본 연구에서 사용된 침치료, 전침치료가 tremor 증상을 완화시키는데 기여했을 것으로 추정된다. 또한 향후 본 연구에서 사용된 약침, 봉침이 침치료와 어떤 차이가 있는지에 대한 효과 및 기전 연구가 필요할 것으로 사료된다.
Kim 등23에 따르면 기존의 파킨슨병 연구는 다양한 처방 및 약재를 사용하여 한약치료에 따른 파킨슨병 치료효과에 대한 review를 하기 어려우며, 대조군과 비교하였을 때 한약치료에 따른 특이적인 효과를 명확하게 발견하기 어려웠다고 보고하였다. 억간산24, 열다한소탕25,26, 청심열다탕27, 향사양위탕28, 소합향원29, 소속명탕30 등 다양한 복합처방과 여러 단일 한약재 추출물이나 한약재 성분 추출물에서의 신경보호 작용에 대한 연구는 이미 보고된 바 있는데, 한의의료기관에서는 단일 한약재보다는 한약 복합처방을 주로 사용하므로 향후 복합처방에 대한 연구가 더 많이 필요할 것으로 사료된다.
본 연구를 통하여 파킨슨병의 tremor, bradykinesia, PIGD 증상 각각 대한 한의치료의 효과를 확인할 수 있었다. 향후 위에서 제시한 각 증상 및 치료법별로 추가적인 연구를 위한 참고가 될 수 있도록 본 연구결과를 보고하는 바이다.
또한 이 등19은 파킨슨병환자의 사상체질에 따른 사상체질처방 투여가 포함된 한양방복합치료를 시행한 치험례나 열다한소탕32, 지황백호탕33을 투여한 치험례도 보고된 바있다. 향후 파킨슨병에 대한 사상체질 이론을 포함한 한의학적 변증증형과 처방 표준안을 확립하고 이에 대한 연구가 필요할 것으로 사료된다. 또한 파킨슨병의 비운동성 증상인 인지장애, 우울, 불안 등의 기분장애, 수면장애, 피로감, 자율신경장애 등의 증상에 대한 한의치료의 효과에 대한 연구도 필요할 것으로 사료된다.
