Purpose: The purpose of this study was to describe the status of infection control nurses (ICNs) and their activities, and to identify the factors affecting the level of infection control activities. Methods: Data were collected from 199 hospitals from June 24 to July 26, 2016. The structured questionnaires included status of infection control nurses, type and level of infection control activities. Results: Most participating hospital were advanced general hospital (20.1%) and general hospital (67.8%). Among the hospitals, 86.4% had an infection control department (ICD). The average hospital work experience of ICNs was 14.62 years, and their average infection control career was for 4.94 years. Among the ICNs, 85.6% worked in full time and the average number of beds per ICN was 311.21. There were significant differences in the existence of ICD, infection control activities including surveillance, outbreak investigation, negative pressure room, hand hygiene monitoring, disinfection, and sterilization according to hospital size. The level of infection control activities was higher with more number of ICNs, ICN employment as full time, and healthcare institution accreditation status. The explanatory power was 37.5%. Conclusion: These results of this study which reflect infection control status of healthcare facilities with more than 150 beds in 2016 will provide baseline data to establish infection control system in small to medium sized hospitals after the Middle East Respiratory Syndrome outbreak in 2015.

따라서 본 연구는 국내 MERS 유행발생 이후 의료기관에 근무하는 감염관리간호사의 현황과 감염관리 활동 실태 및 감염관리간호사가 인식하는 의료기관의 감염관리 활동 수준을 파악하고 의료기관 감염관리 활동 수준에 영향을 미치는 요인을 파악함으로써 국가방역체계 제도 시행을 대비하기 위한 기초자료를 제공하고자 한다.
제안 방법
첫째는 대한감염관리간호사회 회원 중 의료기관의 감염관리간호사 대상으로 전자우편을 통해 연구의 목적과 방법,기대 효과, 비밀 유지, 철회 가능성 등을 설명한 후 참여 의지를 밝힌 대상자에게 설문조사를 시행하였다. 둘째, 병원간호사회 협조를 구한 후 간호부서장 대상으로 전자우편을 통해 감염관리간호사 배치 유무를 조사하였다. 감염관리간호사를 배치하였다고 회신한 의료기관의 감염관리간호사에게 우편으로 설명문과 동의서, 설문지를 배포하여 참여 의지를 밝힌 대상자에게 설문조사를 시행하였다.
본 연구의 도구는 Kang [10]의 구조화된 설문지 도구를 바탕으로 Jeong등[4]이 수정 ․ 보완한 도구와 대한감염관리간호사회에서 제시한 감염예방 및 관리 프로그램[1]을 반영하여 연구자들이 개발하였다. 간호학과 교수를 포함한 감염관리전문간호사 5인에게 내용타당도 검증을 받아 수정 ․ 보완하여 최종 개발하였다.
본 연구 시작 전, 연구자 소속 대학병원 임상시험위원회(Institutional Review Board, IRB)의 승인을 받았으며(승인번호: KC16QIMI0420) 대상자 표집을 위하여 2016년 6월 24일부터 7월 26일까지 다음의 2가지 방법으로 설문조사를 실시하였다. 첫째는 대한감염관리간호사회 회원 중 의료기관의 감염관리간호사 대상으로 전자우편을 통해 연구의 목적과 방법,기대 효과, 비밀 유지, 철회 가능성 등을 설명한 후 참여 의지를 밝힌 대상자에게 설문조사를 시행하였다. 둘째, 병원간호사회 협조를 구한 후 간호부서장 대상으로 전자우편을 통해 감염관리간호사 배치 유무를 조사하였다.
대상 데이터
2차는 대한감염관리간호사회 회원이 감염관리간호사로 근무하고 있지 않은 150병상 이상인 총 318개 의료기관에 설문지를 배부하여 감염관리간호사 배치 여부를 조사하였으며, 응답한 91개 의료기관 중 52개에 감염관리간호사가 배치되어 있었으며, 감염관리간호사가 배치된 52개 의료기관에 설문지를 재배부하여 14개 의료기관이 응답하였다. 1, 2차 연구대상 모집 결과, 감염관리간호사가 근무하고 있으면서 150병상 이상인 330개 의료기관 중 200개 의료기관이 응답하였으며, 불성실하게 응답한 1곳을 제외한 199개 의료기관의 자료를 최종 분석에 이용하였다.
본 연구대상은 감염관리간호사가 배치된 150병상 이상의 국내 의료기관을 대상으로 하였다. 1차 연구대상 모집을 위하여 대한감염관리간호사회 회원이 감염관리간호사로 근무하고 있는 150병상 이상인 총 278개 의료기관에 설문지를 배부하였고, 186개 의료기관에서 응답하였다.

의료기관의 규모에 따른 감염관리 활동실태와 수준 분석은 x2 test, t-test로 분석하였으며, 의료기관 특성에 따른 감염관리 활동 수준은 ANOVA와 t-test로 분석하고 사후 검증은 Scheffé를 이용하였다. 감염관리간호사가 인식하는 의료기관 감염관리 활동 수준에 영향을 미치는 요인은 다중회귀분석을 이용하여 분석하였다.

성능/효과
감염관리간호사가 인식하는 의료기관 감염관리 활동 수준에 영향을 미치는 요인을 분석한 결과 의료기관 인증 평가, 근무 형태, 감염관리실 인력수가 유의한 요인으로 확인되었고요인의 모형 설명력은 37.5%였다(Table 5).
본 연구결과를 통하여 감염관리간호사 경력과 담당 병상 수, 의료기관의 감염관리 활동 측면에서 일부 개선이 이루어졌음을 알 수 있었다. 그러나 규모가 작은 의료기관에서 여전히 겸임의 근무 형태가 존재하고, 감염관리 업무를 함에도 불구하고 교육 지원을 받지 못하거나 감염관리에 적합한 자격증을 보유한 감염관리간호사가 적음을 알 수 있었다. 또한 음압격리실 구비, 손위생 모니터링, 소독제 승인 절차, 소독과 멸균 모니터링과 같은 감염관리 활동은 의료기관의 규모에 따라 유의한 차이를 보여 규모가 작은 의료기관에서 감염관리 시설과 설비, 경험과 지식을 갖춘 감염관리간호사가 부족함을 확인하였다.

후속연구
또한 연구 기간 중 감염관리 관련 의료법과 인증기준의 강화, 감염관리 인프라에 대한 수가 신설로 인해 국내 의료기관의 감염관리 인력, 시설, 장비 측면에서 변화가 있었다. 이에 지역별, 의료기관 규모별, 의료기관 종별로 대표성 있는 표본을 표출하여 감염관리 인력, 시설, 장비, 활동 실태와 활동 수준에 대한 반복연구와 감염관리 인프라 투입의 효과를 측정하는 연구를 제안한다.
