Purpose: This study is to grasp factors influencing the quality of life in older adults after total knee replacement. Methods: This study was conducted with 165 older adults who had TKR at four orthopedic hospitals in D city. Data were analyzed using one-way ANOVA, independent t-test, Pearson's correlation, and stepwise multiple linear regression with SPSS 19.0 software. Results: Pain and depression were negatively correlated with range of motion, social support, while sense of coherence was positively correlated with quality of life. Sense of coherence (43%, ${\beta}=.40$), pain (8%, ${\beta}=-.30$), and depression (3%, ${\beta}=-.20$) on the Physical Component Summary in the quality of life have significant explanatory power of 54%. Sense of coherence (49%, ${\beta}=.44$), social support (6%, ${\beta}=.25$), and depression (3%, ${\beta}=-.22$) on the Mental Component Summary in the quality of life have significant explanatory power of 58%. Conclusion: This study suggests developing a program to improve the quality of life in older adults who had TKR, considering factors such as sense of coherence.

본 연구대상자는 D시 소재 C, D, S, W 정형외과 전문병원에서 퇴행성 슬관절염으로 편측 슬관절전치환술을 받고 퇴원한 후 외래로 통원치료를 받는 수술 후 1년 이내의 노인을 대상으로 하였다. 의사소통이 가능하고 인지기능 장애 및 신경정신과적 질병이 없으며, 슬관절 기능과 통증에 영향을 줄 수 있는 질환이 없는 노인으로 본 연구의 목적을 이해하고 연구참여를 서면으로 동의한 대상자로 하였다.

데이터처리

• 대상자의 삶의 질에 영향을 미치는 요인을 확인하기 위해 stepwise multiple regression으로 분석하였다.

이론/모형

Yesavage와 Sheikh [19]가 개발한 Geriatric Depression Scale Short Form (GDSSF)을 Kee [20]가 번역하여 타당도와 신뢰도를 검증한 한국판 단축형 노인 우울 척도(Geriatric Depression Scale Short Form Korean Version, GDSSF-K)를 사용하였다. 이 도구는 15문항으로 0~15점의 범위를 가지며, ‘예’ 0점, ‘아니오’ 1점으로 측정하여 점수가 높을수록 우울증상이 심함을 의미한다.
통합성은 Antonovsky [12]가 개인의 통합성을 측정하기 위해 개발한 13문항의 도구를 Kim [22]이 한국적 상황에 맞게 번안하여 타당도를 검증한 도구를 사용허락을 받고 사용하였다. 이해력 5문항, 관리력 4문항, 의미부여 4문항의 3개 하위요인으로 구성되며 7점 척도로 가능한 점수의 범위는 13~91점이다.

성능/효과

본 연구결과 슬관절전치환술을 받은 노인들은 통증과 우울이 적을수록 삶의 질이 높고, 관절가동범위가 넓고 사회적 지지가 많을수록 삶의 질이 높은 것으로 나타났다. 또한, 통합성이 높을수록 삶의 질이 높은 것으로 나타났으며, 삶의 질에 통합성은 가장 높은 설명력을 가지는 것으로 분석되었다. 그러므로 삶의 질을 높일 수 있는 통증을 조절하고, 우울을 예방하고 관리할 수 있는 중재프로그램과 관절가동범위를 향상 시킬 수 있는 신체활동 및 운동 프로그램을 포함하여, 사회적 지지를 강화하고 통합성을 증진시키는 삶의 질 향상 프로그램을 개발하여 활용해 볼 것을 제언한다.
본 연구는 슬관절전치환술을 받은 노인의 삶의 질에 영향을 미치는 요인을 파악하기 위해 시도되었다. 본 연구결과 슬관절전치환술을 받은 노인들은 통증과 우울이 적을수록 삶의 질이 높고, 관절가동범위가 넓고 사회적 지지가 많을수록 삶의 질이 높은 것으로 나타났다. 또한, 통합성이 높을수록 삶의 질이 높은 것으로 나타났으며, 삶의 질에 통합성은 가장 높은 설명력을 가지는 것으로 분석되었다.
신체적 건강보다 삶의 질이 더 높게 나타나 것은 수술 후 신체적 기능이 향상됨에 따른 것으로 추측된다. 본 연구의 다중회귀분석 결과 슬관절전치환술을 받은 노인의 신체적, 정신적 건강의 삶의 질에 대해 통합성, 사회적 지지, 통증, 우울 등이 유의한 영향요인으로 나타났으며, 가장 높은 영향 요인은 ‘통합성’으로 나타났다. 선행연구에서 슬관절전치환술을 시행한 환자를 대상으로 2년 후 시점에서 삶의 질을 측정한 결과 MCS는 환자의 65%, PCS는 환자의 89%에서만이 삶의 질 개선을 보였으며, 5~18%의 환자는 여전히 슬관절 기능상태가 나쁜 것으로 보고되었다[17].

후속연구

또한 주요 연구결과를 토대로 슬관절전치환술의 긴 회복기간을 고려하여 수술 후 1년 이내의 외래 통원 환자를 대상으로 함으로써 수술 후 가정 및 지역사회에서의 신체 및 정신적 영역의 간호중재 및 체계화된 재활 프로그램 구성의 기초자료를 제공하는 점에서 연구의 의의가 있다. 그러나 본 연구의 대상자는 일부 지역에 국한되어 있고 임의 표출하여 선정하였으므로 연구결과를 다른 지역에 확대 해석하는 데에는 제한이 있다. 따라서 무작위 표집을 통한 추후 연구를 제언한다.
